Management of stenosis of the subglottic larynx remains one of the most challenging problems facing the laryngologist. This report describes our results treating severe subglottic stenosis. Five patients, 2 males and 3 females, with acquired stabilized laryngotracheal stenosis were treated during the period 1981-1991. Ages ranged from 13 to 62 years. 25 patients, 15 children and 10 adults, with the clinical diagnosis of acute epiglottitis were studied. Patients severely ill on admission, 13 children and 1 adult, were immediately treated with tracheotomy and antibiotics; the remaining patients with antibiotics only. A case of a female patient with extensive rheumatoid arthritis who presented to the Accident and Emergency Department with life-threatening stridor is described. Although clinical involvement of the larynx is found in over a third of patients with severe rheumatoid arthritis, acute airways obstruction is fortunately a very rare complication. Stridor is probably precipitated in the acute situation in such patients as a result of upper respiratory tract infection.

Subglottic stenosis is a disorder characterized by narrowing of the airway below the glottis or apposing edges of the true vocal cords. In a broad definition, the term may be used to describe airway compromise in the regions of the larynx or the trachea or both. In children, the stenosis is usually due to scar formation secondary to prolonged airway intubation, rather than to external trauma. In selected patients with obstructive sleep apnea, the uvulopalatopharyngoplasty procedure may be performed to remove excessive tissue in the oropharyngeal airway.
