Gender-affirming care in South Africa: A cross-sectional survey of transgender and gender-diverse people in the Eastern and Western Cape provinces, South Africa.

BACKGROUND: Transgender and gender-diverse (TGD) people face significant discrimination in the South African (SA) health system, limiting their access to HIV services and gender-affirming care, which supports an individual's gender identity when it does not align with their sex assigned at birth. Despite the critical role of these services for TGD people, access to care remains understudied in SA. OBJECTIVES: To describe TGD people and their access to and need for social, legal and medical transition, including psychosocial care, hormone therapy and surgery, as well as HIV services, in the Ea

Incidence of prolonged transitional neonatal hypoglycemia and associated factors among neonatal admissions in South Gondar public hospitals, North-Central Ethiopia: a prospective cross-sectional study.

BACKGROUND: Unlike in developed countries, neonatal morbidity and mortality are the leading challenges associated with easily preventable and treatable disorders during the neonatal period in low- and middle-income countries. However, evidence-based data about prolonged transitional hypoglycemia and associated factors are highly limited in Ethiopia and resource-limited countries. METHODS: An institution-based prospective cross-sectional study was conducted at public hospitals in South Gondar in neonatal intensive care units (NICUs). The data were entered and analyzed using SPSS version 23. De

Technical Performance Score Predicts Partial/Transitional Atrioventricular Septal Defect Outcomes.

BACKGROUND: Repair of partial or transitional atrioventricular septal defects (P/TAVSDs) has excellent outcomes; however, late reinterventions remain a concern. Technical performance score (TPS) measures residua after repair and has been associated with early/mid-term outcomes after congenital cardiac operation. Our study investigates TPS as a predictor of outcomes after P/TAVSD repair. METHODS: This was a single-center retrospective review of P/TAVSD repair from July 2000 to November 2015. Intraoperative and discharge TPS were assigned based on echocardiographic criteria: class 1, no residua

Effects of surrogate mothering on physiologic stabilization in transitional newborns.

This experimental study was conducted to determine whether surrogate mothering during the first hours of life will affect physiologic stabilization in the transitional newborn. Two groups of 8 normal newborns were studied. The treatment was given on cue and consisted of rocking, cuddling, visual and verbal interaction, and non-nutritive sucking to satiety. The control group received routine nursing care. The experimental group's mean axillary temperature correlated with increasing age (r = +.91, p less than .001) and leveled at 2 hours of life at 98.2 degrees F (36.8 degrees C). Control group

A prospective cohort analysis from Germany shows transition into adulthood is an underestimated vulnerable period for children with overweight/obesity.

BACKGROUND: Even though the transition from adolescence to adulthood is a vulnerable and crucial period for young individuals with chronic conditions, it remains poorly studied in obesity. We comprehensively characterized the transition of children with overweight/obesity from childhood to adult care. METHODS: Data from an adulthood follow-up of the Leipzig childhood obesity cohort (N = 209, mean age at follow-up 24.9 years) were analyzed and related to the respective childhood data. Assessments comprised anthropometrics, oral glucose tolerance testing, carotid ultrasonography, 24 h blood pre

Development and Validation of the Transition Readiness Assessment Instrument in Type 1 Diabetes "On TRAck".

OBJECTIVES: Transition to adult care is challenging for youth with type 1 diabetes (T1D) and their caregivers. We have developed the diabetes-specific "On TRAck" transition readiness scale, and in this study we assess its reliability and validity compared with TRANSITION-Q, a generic transition readiness questionnaire. METHODS: We systematically created 3 versions of On TRAck: adolescent, parent and health-care provider (HCP) versions (for case managers and physicians). Among adolescents 13 to 18 years of age with T1D at a single academic centre, we conducted an exploratory factor analysis an

Impact of Transition to Insulin U-500 during Pregnancy on Continuous Glucose Monitoring-Derived Glycemic Metrics among Women with Type 2 Diabetes.

OBJECTIVE: Insulin U-500 is a highly concentrated regular insulin that is five times stronger than standard U-100 and is used to treat severe insulin resistance. Glycemic data from continuous glucose monitoring (CGM) in relation to initiation of U-500 insulin in pregnancy with type 2 diabetes are limited. We examined CGM-derived glycemic measures before and after insulin U-500 transition during pregnancy among women with type 2 diabetes. STUDY DESIGN: We conducted a case-crossover study evaluating glycemic measures before and after transition to insulin U-500 during pregnancy among women with

NICU music therapy: song of kin as critical lullaby in research and practice.

Music therapy can improve neonatal function and reduce anxiety in parents during neonatal intensive care unit (NICU) stays. Live music entrained to an infant's observed vital signs, provided by a certified music therapist with First Sounds RBL (rhythm, breath, and lullaby) training, enhanced bonding for infant-parent dyads and triads. The author's song of kin intervention, which employs parent-selected songs, is compared to the presentation of a well-known folk theme ("Twinkle") in 272 neonates. Culturally based, parent-selected, personalized musical tunes provided in song, as a noninvasive in

An approach to heroin use disorder intervention within the South African context: a content analysis study.

BACKGROUND: The field of heroin use disorder intervention has been in transition in South Africa since the outbreak of the heroin epidemic. Yet despite growing evidence of an association between heroin users' use of supplementary intervention services and intervention outcomes, heroin use disorder intervention programmes in South Africa generally fail to meet international research-based intervention standards. METHODS: Semi-structured interviews with ten heroin use disorder specialists were conducted and the interviews were subjected to content analysis. RESULTS AND DISCUSSION: In terms of

'We Need Help Too'-The Forgotten Stakeholders: Exploring the Needs of Carers During Their Young Persons' Transition From Child to Adult Eating Disorder Services.

BACKGROUND: Carers are essential in their young person's recovery during transition from child to adult eating disorder services. Despite re … mics. CONCLUSIONS: Carers continue to articulate concerns that corroborate and reinforce the wider transitional care literature, with few carers feeling supported in the process. Prioritising the co-development of interventions supporting carers wellbeing during transition is key to ensure carers distress is not perpetuated.

Navigating adulthood with maple syrup urine disease: patient and caregiver perspectives on healthcare transition and independent living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

The Efficacy of Bracing in Nonoperative Care of Medial Femoral Condyle Osteochondritis Dissecans: A Study From the Research in Osteochondritis Dissecans of the Knee (ROCK) Study Group.

BACKGROUND: Stable osteochondritis dissecans (OCD) lesions are primarily treated nonoperatively. Nonoperative treatment includes several recommendations, including the use of an unloader brace. However, little is known about the clinical effectiveness of unloader bracing and its influence on OCD healing, return to sports, and patient-reported outcomes (PROs). The purpose of this investigation was to evaluate the impact of unloader bracing on patients with stable medial femoral condyle (MFC) OCD lesions treated nonoperatively. PURPOSE: To investigate the influence of valgus unloader bracing on

Magnet Status in Nurse Practitioners' Care Assessments and Hospital Recommendations.

BACKGROUND: Nurse practitioners (NPs) are a growing segment of the acute care hospital workforce, yet little is known about how organizational factors, such as Magnet designation, influence their workplace experiences. OBJECTIVES: We examined whether employment in a Magnet hospital was associated with NPs' strong recommendation of their hospital and whether this relationship was explained by NPs' assessments of care quality, patient safety, and care transitions. METHODS: In this cross-sectional study, we analyzed data from 774 NPs across 425 US hospitals using the 2024 Penn Nurses4All Survey

Life transition stages of family caregivers for patients with chronic illness: A grounded theory study.

BACKGROUND: Family caregivers play a crucial role in providing continuous support to individuals with chronic illnesses. Their responsibilities extend beyond the well-being of patients, influencing both patient health outcomes and their own. Caregiving entails significant responsibilities and sacrifices, requiring caregivers to navigate evolving roles and care needs over time. Existing literature highlights the emotional and psychological challenges faced by caregivers, including role transitions and stress. However, most studies focus on caregiving experiences rather than the specific transit

Strategies to Improve Patient-Centered Care for Drug Use-Associated Infective Endocarditis: JACC Focus Seminar 2/4.

Drug use-associated infective endocarditis (DUA-IE) is a major cause of illness and death for people with substance use disorder (SUD). Investigations to date have largely focused on advancing the care of patients with DUA-IE and included drug use disorder treatment, decisions about surgery, and choice of antibiotics during the period of hospitalization. Transitions from hospital to outpatient care are relatively unstudied and frequently a key factor of uncontrolled infection, continued substance use, and death. In this paper, we review the evidence supporting cross-disciplinary care for peopl

Supporting Youth from Pediatric- to Adult-Oriented HIV Care Across Two Metro Sites in the United States: Results from the iTransition Pilot Trial.

In the United States, youth are disproportionately affected by HIV and have poorer health outcomes than adults. Health care transition (HCT) from pediatric/adolescent- to adult-oriented HIV care is associated with disruptions to youths' care retention, medication adherence, and viral suppression. However, no evidence-based interventions exist to improve HIV-related HCT outcomes. Accordingly, our team designed and implemented the iTransition intervention to support youth and providers in navigating HIV-related HCT. We conducted a pilot trial of iTransition in two cities in the United States wit
