PURPOSE: Bladder neck closure (BNC) is an important component of reconstructive urological surgery, especially in the management of neurogenic bladder. To our knowledge we present the largest series of patients who have undergone this procedure.

A novel technique of urinary diversion was designed by interposition of an intussuscepted ileal segment between the ureters and the rectosigmoidal pouch, thus preventing ureteral reflux as well as stenosis at the uretero-enteric anastomosis, possible occurrence of urocolonic carcinoma, and frequent evacuation or incontinence. Since all the procedures were limited to the lower abdominal cavity, the surgical invasiveness was compatible with that of an ileal conduit. Our early experience in 15 patients showed that this technique can be considered for those in whom the urethra is not available.

BACKGROUND: We report the rare case of a chorioretinitis occurring after intravesical BCG instillation to cure in situ carcinoma.
HISTORY AND SIGNS: A 57-year-old man was treated for bladder carcinoma with BCG instillations. Visual acuity was 20/20 in the right eye and 20/40 in the left eye. Bilateral chorioretinitis was discovered three months after initiation of intravesical BCG therapy.
THERAPY AND OUTCOME: No progression of ocular lesions could be seen after a 6 months therapy with rifampicin, isoniazide and ethambutol.

Urethral irregularity (impeding catheterization) and failure to achieve continence are common complications in surgery for urinary incontinence. We describe a surgical technique using an anterior bladder wall flap that is sutured to the posterior wall in an onlay fashion creating a flap valve mechanism. We report a case of true carcinosarcoma involving the urinary bladder, ureter, and renal pelvis in an eighty-year-old man. The patient underwent transurethral resection of the bladder tumor and left nephrectomy, followed by combination chemo- and radiotherapy. He died eighteen months after the nephrectomy.

The effects of focused high energy shock waves (SW) on the implanted urinary bladder cancer in rabbits were examined. The bladder cancer was exposed to 2000 to 8000 shots of focused SW under ultrasound guidance. Sodium-calcium exchange has been suggested to play a pivotal role in the regulation of cytosolic free calcium (Caf) by epithelial cells.
