[Clinical study on post-stroke urinary retention treated with acupuncture at the twelve jing-well points and bladder function training].

OBJECTIVE: To explore the methods for the therapeutic effect improvement in the treatment of post-stroke urinary retention. METHODS: Sixty-three patients of post-stroke urinary retention were randomized into an observation group (32 cases) and a control group (31 cases). The routine clinical medication of neurology and basic rehabilitation were adopted in the two groups. Additionally, in the control group, the intermittent urinary catheterization and bladder function training were applied. The duration and frequency of catheterization were determined by the autonomic urination and residual ur…

Acute Urinary Obstruction in a Tetraplegic Patient from Misplacement of Catheter in Urethra.

A male tetraplegic patient attended accident and emergency with a blocked catheter; on removing the catheter, he passed bloody urine. After three unsuccessful attempts were made to insert a catheter by nursing staff, a junior doctor inserted a three-way Foley catheter with a 30-mL balloon but inflated the balloon with 10 mL of water to commence the bladder irrigation. The creatinine level was mostly 19 µmol/L (range: 0-135 µmol/L) but increased to 46 µmol/L on day 7. Computerized tomography urogram revealed that the bilateral hydronephrosis with hydroureter was extended down to urinary bladder

Nursing interventions to reduce the risk of catheter-associated urinary tract infection. Part 1: Catheter selection.

BACKGROUND: The urinary system is the most common site for all hospital-acquired infections, accounting for approximately 40% of all nosocomial infections. The US Centers for Medicare & Medicaid Services has enacted 2 policies that have focused considerable attention on the optimal use of indwelling catheters in the acute and long-term care settings and the prevention of complications including catheter-associated urinary tract infection (CAUTI). OBJECTIVES: This is the first of a 2-part Evidence-Based Report Card reviewing current evidence pertaining to nursing actions for prevention of CAUT

Neurogenic bladder dysfunction due to Behçet's disease.

BACKGROUND: Behcet's disease is a chronic multisystem condition of unknown origin. Genitourinary tract involvement with genital apthous ulcers, epididymitis, urethritis and recurrent cystitis is encountered commonly. Neurogenic bladder dysfunction due to neural involvement of the syndrome, has been reported extremely rarely. METHODS: A case of neurogenic bladder dysfunction due to Behcet's disease is presented. The patient was 17-year-old young male with a long history of the disease and neurological affection, but with neglected lower urinary tract symptoms and urinary incontinence. The pati

The epidemiology of kidney and urinary tract infections in pediatric chronic kidney disease: prevalence, risk factors and association with GFR decline in the Chronic Kidney Disease in Children Study.

BACKGROUND: Urinary tract and kidney infections are potential complications in pediatric chronic kidney disease (CKD), especially among patients with congenital anomalies of the kidney and urinary tract (CAKUT). We characterized the prevalence of self-reported infections from childhood through young adulthood; evaluated risk factors; and quantified the association with CKD progression. METHODS: Infections in the past year were self-reported at annual Chronic Kidney Disease in Children (CKiD) study visits. Age-specific infection prevalences were stratified by sex and CKD diagnoses (glomerular,

SCI Model Systems Complicated Urinary Tract Infection (cUTI) Guidelines: International Consensus on cUTI Likelihood Based on Symptoms.

BACKGROUND: Complicated urinary tract infection (cUTI) is prevalent among people with spinal cord injury and disease (SCI/D). Diagnostic guidelines are neither consistent nor evidence based. OBJECTIVES: To establish consensus around symptoms-based diagnostic and decision-making criteria for cUTI for SCI/D. METHODS: A representative sample of clinicians from PM&R, infectious disease, urology, and primary care within the United States (phase 1) and internationally (phase 2) participated in this study. Phase 1 involved focus groups and interviews to refine a decision-making paradigm for cUTI ba

[Features of surgical treatment of urinary incontinence in women with urethral destruction].

Urethral destruction in women is a rare pathology which is result of traumatic damage of the urethra during pelvic fractures, sexual trauma, iatrogenic effects (operative vaginal delivery, prolonged catheterization of the bladder, complicated surgical interventions on the urethra, etc.). The main symptom of urethral destruction is involuntary urinary incontinence. The relevance and complexity of this problem are determined by the fact that the choice of surgical treatment for urethral destruction is still unsolved. This requires such kind of intervention, which will be aimed not only at restor

Urinary catheter-associated infections.

Catheter-acquired urinary infection is the most common device-associated healthcare-acquired infection. Although most patients are asymptomatic, symptomatic infection may occur and is associated with increased morbidity and costs. Long-term indwelling catheters are associated with more complex microbiology and greater morbidity than short-term catheters. The most effective way to prevent these infections is to restrict indwelling urinary catheter use to limited indications, and to discontinue use of a catheter as soon as feasible. Alternate means of managing bladder emptying, including externa

Postoperative catheterization and prophylactic antimicrobials in children with hypospadias.

A prospective study of 78 children who underwent 84 operations for correction of hypospadias was done. Of these, 54 had a transperineal indwelling Foley catheter for ten days after surgery and 30, a transurethral catheter. Forty-five randomly selected children received prophylactic antimicrobial therapy (sulfamethoxazole), and the remaining 39 children served as controls. Incidence of urinary tract infection was significantly higher in the control group (10 of 39) as compared with the treated group (3 of 45) in spite of the higher incidence of vesicoureteral reflux in the treated group. Herpes zoster-associated acute urinary retention: a case report.

An 87-year-old woman presents with a 4-week history of urinary incontinence during which she had been treated for disseminated herpes zoster virus (HZV). On physical exam painful vesicles involving the entire vulvar region with mainly right sacral distribution were found. A catheterized volume exceeded 600 ml of retained urine after the patient failed to void spontaneously. Multichannel voiding-pressure urodynamic studies revealed an acontractile neurogenic bladder with overflow incontinence. The patient was discharged on a conservative regimen with arrangement for visiting nurse services to p

A prospective microbiologic study of bacteriuria in patients with chronic indwelling urethral catheters.

Bacteriuria is common in chronically catheterized patients and is associated with both acute and chronic complications. Of 605 consecutive weekly urine specimens from 20 chronically catheterized patients, 98% contained bacteria at high concentrations and 77% were polymicrobial. The mean interval between new episodes of bacteriuria was 1.8 weeks. Most species of bacteria caused five to seven new episodes of bacteriuria per 100 weeks of catheterization. Even though access to the catheter lumen was similar, the duration of bacteriuric episodes varied greatly by species. Of the episodes of bacteri

[Secondary uroliths after surgical correction of urinary bladder extrophy].

The analysis of long-term results of complex surgical correction of urinary bladder extrophy in 83 patients has shown that 21 (25.3%) patients had concrements in the urethra. Multiple bacteriological tests detected urease-producing flora which was managed with specific antibacterial therapy. Two patients were exposed to extracorporeal shock-wave lithotripsy. Low-invasive lithotripsy was low effective and all the patients were operated. Chemical structure of the removed concrements was determined with x-ray difractometry. Recurrent concrements were found in 7 (33.3%) of 21 patients who had not

Intravaginal slingplasty (IVS): an ambulatory surgical procedure for treatment of female urinary incontinence.

A new ambulatory procedure for treatment of female urinary incontinence (intravaginal slingplasty, IVS) was performed on 50 patients. In all patients the surgical procedure was carried out under local anaesthesia and without postoperative urinary catheterization. The technique has been elaborated from previous experimental and clinical studies and aims at restoration of the pubourethral ligament and the suburethral vaginal hammock. Thirty-eight patients suffered from genuine stress incontinence as objectively verified and 12 patients had symptoms and signs of both urge and stress incontinence.…

Cloacal exstrophy reconstruction for urinary continence.

From 1978 to 1989 we treated 16 patients with cloacal exstrophy, including 8 who have been treated since birth. To achieve urinary continence bladder reconstruction was required in 12 patients, including 3 ileocystoplasties, 4 gastrocystoplasties and 5 gastric continent urinary reservoirs. All 12 patients underwent simultaneous bladder neck/urethral continence procedures. Of the 12 patients with completed reconstruction 10 were evaluable for continence. All 10 patients are dry for periods of 3 or more hours, and 5 are totally dry day and night. All but 1 patient are totally dependent on clean

Acute Renal Shutdown Following Femoral Central Venous Catheterization: Iatrogenic or Spontaneous Rupture of the Inferior Epigastric Artery.

Femoral central venous catheterization is a commonly performed procedure in the intensive care unit or ward in a hospital setting. Unfortunately, the venous catheterization procedure can cause insertion-related complications, commonly referred to as mechanical complications, which can range from being clinically insignificant to life-threatening if untreated. The femoral vein is chosen due to its low risk for complications such as bleeding, which are easy to control when the patient is on anticoagulants. Although the infection rate is high with femoral central line insertion, we prefer it over

Postvoid residual urine in women with stress incontinence.

AIMS: To confirm the idea that women with stress incontinence can have elevated postvoid residual urine (PVR) and to examine the correlation between PVR obtained with catheterization versus that with BladderScan (BS). MATERIALS AND METHODS: This is a prospective study involving 902 women referred to our urogynecology clinics because of symptoms of lower urinary tract dysfunction. Women were selected if they met all of the following conditions: (1) A main complaint of stress urinary incontinence; (2) A diagnosis of urodynamic stress incontinence; and (3) No previous pelvic surgery, advanced pe

Not all nosocomial Escherichia coli bacteriurias are catheter associated.

We prospectively determined what proportion of nosocomial E. coli bacteriurias are associated with urinary catheters. Only 46% (95% CI 37–56%) of nosocomial E. coli bacteriurias were catheter-associated. Compared to bacteriuric patients with catheters, non-catheterized patients were less likely to be male and have renal insufficiency or a recent urogenital procedure.

Status and tasks of today's urodynamics.

The current situation of urodynamics and the ways of improving and reinforcing our investigational and treatment modalities are presented. Correct evaluation of the bladder dysfunction depends on sound evaluation of a patient's history, naked-eye observation and careful urodynamic investigation. The clinical and social significance of the following items is specifically discussed: urinary incontinence of elderly people, clean intermittent catheterization and artificial urethral sphincter.

The management of urinary incontinence in the male neurological patient.

PURPOSE OF REVIEW: Urinary incontinence in male neurological patients is a very frequent problem but treatment remains challenging. Thus, we summarize and highlight the latest developments in the management of urinary incontinence in this specific patient population. RECENT FINDINGS: Intermittent self-catheterization, antimuscarinics, intradetrusor injections with onabotulinumtoxinA, augmentation cystoplasty, urinary diversion, and artificial urinary sphincter are the cornerstones of the armamentarium for treating neurogenic urinary incontinence. However, with the exception of onabotulinumtox
