Having passed the 30th anniversary of the first implantation of a Björk-Shiley convexo-concave tilting mechanical valve, recognition of the life-threatening complication of strut fracture is not widespread. The authors report the case of a 48-year-old man with acute-onset chest pain and dyspnea found to have strut fracture and disk embolization of a 26-year-old Björk-Shiley prosthetic aortic valve. The value of echocardiography in the diagnosis of this condition is discussed.

Measurement of plasma 17-hydroxyprogesterone by a simple competitive protein-binding assay has proved of value in the diagnosis of congenital adrenal hyperplasia due to 21-hydroxylase deficiency. Children with untreated adrenal hyperplasia had high resting levels from as early as the 6th day of life, and children treated with suppressive doses of cortisone showed a rise after adrenal stimulation with tetracosactrin. The method also promises to enhance the ease and precision with which the condition can be managed.

Spontaneous coronary artery dissection is a rare clinical condition with a wide range of clinical presentations ranging from asymptomatic cases to life-threatening ventricular arrhythmias and sudden cardiac death. The exact pathophysiological mechanism has not been fully established yet. We would like to present a survivor of sudden cardiac death presenting with ventricular fibrillation due to spontaneous coronary artery dissection. The prompt evaluation, medical management, surgical myocardial revascularisation and value of off-pump coronary artery bypass surgery are discussed.
