Mapping and estimating the population at risk from lymphatic filariasis in Africa.

Lymphatic filariasis remains a major public health problem in Africa and is 1 of the World Health Organization's 6 diseases targeted for global eradication. However, no detailed maps of the geographical distribution of this disease exist, making it difficult to target control activities and quantify the population at risk. We hypothesized that the distribution lymphatic filariasis is governed by climate. The climate at sites in Africa where surveys for lymphatic filariasis had taken place was characterized using computerized climate surfaces. Logistic regression analysis of the climate variabl

Short report: peak period of filarial transmission.

This study was designed to determine the peak period of infective mosquito biting and thus the greatest potential for filarial transmission to occur. It was found that biting density, natural infection, and infectivity rates of Culex quinquefasciatus were significantly higher in the third quadrant of the night (from midnight to 3:00 AM) than at other times. This was true in both an urban and a rural environment. Avoidance of mosquito bite by any means during this time period might reduce and limit filarial transmission to a great extent.

Efficient assessment of filariasis endemicity by screening for filarial antigenaemia in a sentinel population.

We have previously reported that a monoclonal antibody-based antigen detection assay (AD12) is sensitive and specific for Bancroftian filariasis in Egypt. The purpose of the present study was to demonstrate the use of this assay in a sentinel population as a means of efficiently screening for filariasis endemicity. Antigen testing was performed with finger-prick blood collected during the day from 743 schoolchildren (ages 11-16 years). The school draws students from 5 villages in Qalubia Governorate, 35 km north-east of Cairo, Egypt. The prevalence of filarial antigenaemia in the school was 17

Studies on the filariasis vector--Culex quinquefasciatus at Kanchrapara, west Bengal (India).

A nine month entomological and parasitological study was conducted between December 1991 and August 1992, at Kanchrapara, a town of the district North 24-Parganas, West Bengal. The town was found to be endemic for bancroftian filariasis. Microfilaria rate among the human population was 1.9 per cent (35 of 1200 persons). Average man hour density of C. quinquefasciatus was considerably high (31.10) and a higher vector prevalence was seen in summer. The infection and infectivity rates of the natural population of the vector were 2.3 and 0.28 per cent respectively. 9.2 and 1.7 per cent of the huma

[Historical epidemiology of bancroftian filariasis in southwest islands of the Indian Ocean].

Bancroftian filariasis arose in the South-West Indian Ocean Islands with human settlements. During the XIXe century, most of the islands were infected but the prevalence and clinical features of the disease were different from an Island to an other. The vectors are Cx. quinquefasciatus, An. gambiae, An. arabiensis, An. funestus. Even if Culex are proven vectors most of the transmission is due to anophelines. The introduction of the parasite was followed by an explosion of the diseases with dramatic clinical features. But since the beginning of the century the disease retreats. Whatever this is

Vascular abnormalities in experimental and human lymphatic filariasis.

Whereas clinical descriptions of grotesque lymphedema and standard light microscopy in human filariasis have elucidated the natural progression of this disease, the link between the nematode and vascular abnormalities including elephantiasis remains poorly understood. Accordingly, we examined the nature and distribution of lymphatic and blood vascular derangements in a variety of tissues and organs from 37 ferrets acutely and chronically infected with Brugia malayi and in 15 patients with Wuchereria bancrofti or Brugia malayi infestation (resected skin, subcutaneous tissue, and lymph nodes) us…

[A survey of filariasis in the Comoro Islands, in Great Comoro and Mohéli].

The filariasis of W. bancrofti with nocturnal periodicity is, together with malaria, the principal endemic disease in the Comoros Islands. Well studied on Mayotte island, which remained a French Territorial Collectivity, its prevalence and distribution are less known in the Grand Comoros, Anjouan and Moheli islands which form the Federal Islamic Republic of the Comoros. The study of the nocturnal microfilariae in the Grand Comoros and Moheli in 1987 is the opportunity to assess the situation of the disease on this archipelago.

How Does the Proportion of Never Treatment Influence the Success of Mass Drug Administration Programs for the Elimination of Lymphatic Filariasis?

BACKGROUND: Mass drug administration (MDA) is the cornerstone for the elimination of lymphatic filariasis (LF). The proportion of the population that is never treated (NT) is a crucial determinant of whether this goal is achieved within reasonable time frames. METHODS: Using 2 individual-based stochastic LF transmission models, we assess the maximum permissible level of NT for which the 1% microfilaremia (mf) prevalence threshold can be achieved (with 90% probability) within 10 years under different scenarios of annual MDA coverage, drug combination and transmission setting. RESULTS: For Ano

Lymphatic filariasis in Papua New Guinea: distribution at district level and impact of mass drug administration, 1980 to 2011.

BACKGROUND: Lymphatic filariasis (LF) caused by Wuchereria bancrofti is present at high prevalence in some parts of Papua New Guinea. However, there has been no rigorous data-based representative assessment of nationwide prevalence of LF. The LF programme has been daunted by the scope of the problem, and progress on mass drug administration (MDA) has been slow and lacking in resources. METHODS: A systematic literature review identified LF surveys in Papua New Guinea between 1980 and 2011. Results were extracted by location, time period and test used (blood slide, immunochromatographic test (I…

Maternal filarial infection: association of anti-sheath antibody responses with plasma levels of IFN-γ and IL-10.

Maternal filarial infection influences the risk of acquiring infection and development of immunity in children. Here we have analysed the blood samples of 60 mothers (24 infected and 36 uninfected) and their corresponding cord bloods to assess the impact of maternal infection on the anti-sheath antibodies and cytokine production in neonates born from them. About 69·4% of non-infected mothers and their cord bloods showed the presence of anti-sheath antibodies, while only 16·6% of the cord bloods from infected mothers were positive for it. The IL-10 level was significantly high in cord bloods of

Insecticide resistance in Culex quinquefasciatus from Zanzibar: implications for vector control programmes.

BACKGROUND: Zanzibar has a long history of lymphatic filariasis (LF) caused by the filarial parasite Wuchereria bancrofti, and transmitted by the mosquito Culex quinquefasciatus Say. The LF Programme in Zanzibar has successfully implemented mass drug administration (MDA) to interrupt transmission, and is now in the elimination phase. Monitoring infections in mosquitoes, and assessing the potential role of interventions such as vector control, is important in case the disease re-emerges as a public health problem. Here, we examine Culex mosquito species from the two main islands to detect W. ba…

The epidemiological delimitation of lymphatic filariasis in an endemic area of Brazil, 41 years after the first recorded case.

In 2002, the status of lymphatic filariasis (LF) in the district of Jaboatão, in north-eastern Brazil, was reviewed, 41 years after the first case of the disease was recorded in the area. Current data from Jaboatão were compared with older information from the district that had been published, as official reports on filariasis cases and mosquitoes and as the result of population surveys, between 1948 and 1997. Although the first microfilaria-positive cases were detected in the district in 1959, it was not until 1993 that the national Ministry of Health categorized Jaboatão as an endemic area.
