New metabolites of the drug 5-aminosalicylic acid. II. N-formyl-5-aminosalicylic acid.

1. A new metabolite of the drug 5-aminosalicylic acid (5-ASA) has been found in urine from pigs and in plasma of humans. The metabolite has been isolated from pig urine using an XAD-2 column and purified using preparative h.p.l.c. 2. The metabolite has been identified as N-formyl-5-ASA (5-formamidosalicylic acid) using 1H- and 13C-n.m.r. spectrometry and mass spectroscopy and the structure was confirmed by chemical synthesis. 3. N-Formyl-5-ASA is stable in human plasma and in potassium phosphate buffers between pH 3.0 and 9.0. It is hydrolysed below pH 3.0. 4. N-Formyl-5-ASA was readily formed…

Stability of 5-aminosalicylic acid suspension.

The stability of 5-aminosalicylic acid in a suspension prepared extemporaneously for administration by enema was studied. The suspension was prepared (tragacanth was the suspending agent) and then placed in 3-ounce amber glass prescription bottles; five bottles were stored at room temperature and five were refrigerated for 90 days. Using samples from each bottle, concentrations of 5-aminosalicylic acid were determined in triplicate by high-performance liquid chromatography at 0, 30, 60, and 90 days. Throughout the study period, all samples retained more than 90% of the day 0 concentration of 5…

Synthesis, characterization and in vitro release of 5-aminosalicylic acid and 5-acetyl aminosalicylic acid of polyanhydride--P(CBFAS).

A novel polyanhydride, poly[(5-carboxybutyl formamide)-2-acetyl salicylic anhydride] (P(CBFAS)), with 5-aminosalicylic acid (5-ASA) incorporated into the polymer backbone was synthesized and characterized by infrared, (1)H-nuclear magnetic resonance, differential scanning calorimetry, vapor pressure osmometry, etc. The polyanhydride was subjected to degradation and simultaneously released 5-ASA and its derivative 5-acetyl aminosalicylic acid (5-acetyl ASA) in vitro under various conditions. The factors influencing the release profiles of 5-ASA and 5-acetyl ASA, including polymer molecular weig…

Pancreatitis after rectal administration of 5-aminosalicylic acid.

A patient treated with sulfasalazine for new-onset ulcerative colitis developed self-limited pancreatitis. Rechallenge with 5-aminosalicylic acid (5-ASA) in enema form (Rowasa) again induced pancreatitis. Recent case reports suggest that the salicylate component of sulfasalazine can lead to the development of pancreatitis with oral 5-ASA administration. This patient's course demonstrates further that rechallenge with 5-ASA in a rectal form may also lead to pancreatitis.

Possible mode of action of 5-aminosalicylic acid.

Despite the extensive use of sulfasalazine (SAS) and/or 5-aminosalicylic acid (5-ASA) in patients with inflammatory bowel disease and, more recently, rheumatoid arthritis, their mode of action has not been elucidated so far. None of the numerous pharmacological and biochemical effects described, including immunosuppressive, antifolate, and modulatory actions on lymphocyte and leukocyte functions, could be defined unequivocally as mediating their beneficial activity. Recently, interest has focused on actions of SAS and 5-ASA on the various enzymes of the arachidonic acid cascade. Mucosa of pati…

Safety of topical 5-aminosalicylic acid in pregnancy.

OBJECTIVE: To assess the safety and efficacy of topical 5-aminosalicylic acid preparations for therapy of distal colitis during pregnancy. METHODS: Nineteen pregnancies in sixteen consecutive patients with proven distal colitis were identified prospectively at a tertiary care center. All patients were given trials of weaning off medication and all failed. All subjects were on maintenance topical 5-aminosalicylic acid therapy at the time of conception. They were followed throughout their pregnancy and thereafter. Their children were also closely examined and followed by a pediatrician. RESULT…

Para-aminosalicylic acid: the return of an old friend.

Para-aminosalicylic acid (PAS) is one of the last remaining drugs available to treat extensively drug-resistant (XDR) tuberculosis. Good outcomes (81% 5 year survival) were documented when PAS was first used with streptomycin, yet results of PAS with remaining potentially effective drugs for the treatment of XDR tuberculosis are poor (mortality 30-90%). In this Review, we assess published work regarding recommendations for PAS dosing in relation to efficacy and tolerance. PAS 20 g daily acoompanied only by streptomycin is better in prevention of streptomycin resistance than PAS 10 g or 5 g dai…

Topical 5-aminosalicylic acid: a treatment for aphthous ulcers.

A double-blind placebo controlled trial of 5% 5-aminosalicylic acid (5-ASA) cream for the treatment of oral aphthous ulcers was carried out on 22 subjects. The cream or a placebo (11 patients each) was applied to the ulcers three times daily for up to 14 days. Daily discomfort was reduced by half (P less than 0.01) and less pain (P less than 0.05) was experienced by the treated group. Treatment with 5-ASA shortened healing time (7 vs. 11 days, P less than 0.01) and reduced the difficulty in eating (P less than 0.05). No significant side-effects were reported. We believe 5-ASA cream to be an ef…
