Anaphylaxis.

Anaphylaxis is a sudden onset, immediate reaction that implies a risk of death. Think of a "rule of 2s" for anaphylaxis, which implies that reactions usually begin within 2 minutes to 2 hours after injection, infusion, ingestion, contact, or inhalation. Fatalities can be from asphyxiation from laryngeal or oropharyngeal swelling, collapse from hypotensive shock, cardiac arrest, or acute severe bronchoconstriction that causes respiratory failure and arrest. When there is activation of mast cells and basophils in anaphylaxis, chemical mediators are detectable. The preformed mediators from mast c

Anaphylaxis.

Anaphylaxis is a systemic, life-threatening disorder triggered by mediators released by mast cells and basophils activated via allergic (IgE-mediated) or nonallergic (non-IgE-mediated) mechanisms. It is a rapidly evolving, multisystem process involving the integumentary, pulmonary, gastrointestinal, and cardiovascular systems. Anaphylaxis and angioedema are serious disorders that can lead to fatal airway obstruction and culminate in cardiorespiratory arrest, resulting in hypoxemia and/or shock. Often, these disorders can be appropriately managed in an outpatient setting; however, these conditi

Anaphylaxis.

Anaphylaxis is an acute, systemic reaction mediated by immunoglobulin E hypersensitivity. Release of bioactive factors causes vasodilation and bronchiole constriction that can lead to hypotensive shock and asphyxiation. Differential diagnosis includes acute asthma, localized angioedema, syncope, and anxiety/panic attacks. Diagnostic tests lack specificity. Clinical diagnosis is based on demonstration of specific airway or cardiovascular compromise within proximity of allergen exposure. Treatment includes epinephrine, antihistamines, fluid resuscitation, and airway management. Prevention focuse

Anaphylaxis.

Anaphylaxis occurs commonly in community settings. The rate of occurrence is increasing, especially in young people. Understanding potential triggers, mechanisms, and patient-specific risk factors for severity and fatality is the key to performing appropriate risk assessment in those who have previously experienced an acute anaphylactic episode. The diagnosis of anaphylaxis is based primarily on clinical criteria and is valid even if the results of laboratory tests, such as serum total tryptase levels, are within normal limits. Positive skin test results or increased serum specific IgE levels

Anaphylaxis.

The clinical syndrome of anaphylaxis may be produced by a number of mechanisms. The life-threatening clinical features are angioneurotic oedema, bronchospasm, cardiovascular collapse and pulmonary oedema. The initial treatment consists of general resuscitation plus adrenaline. In shock, colloid solutions should be used to restore blood volume. The pulmonary oedema that occurs is a membrane oedema, and should be treated with artificial ventilation, positive end expiratory pressure, and volume replacement. After a reaction, all available tests should be used to determine the cause, and the patie

Anaphylaxis.

BACKGROUND: Anaphylaxis is the quintessential critical illness in emergency medicine. Symptoms are rapid in onset and death can occur within minutes. Approximately 1500 patients die annually in the United States from this deadly disorder. It is imperative, therefore, that emergency care providers be able to diagnose and appropriately treat patients with anaphylaxis. Any delays in recognition or initiation of therapy can result in unnecessary increases in patient morbidity and mortality. DISCUSSION: Recent literature, including updated international anaphylaxis guidelines, has improved our und

Anaphylaxis.

Anaphylaxis is an acute multisystem allergic reaction that is potentially fatal. Anaphylactic episodes are most commonly caused by foods or drugs, but in many instances have no known cause. Each physician should be equipped in office for therapy of the acute event. The drug of choice, which should be administered immediately, is epinephrine. Although there is some debate as to the preferred injection site, it is clear that of sites studied to date, injection in the lateral thigh (vastus lateralis) produces the most rapid rise in serum level. Any patient predisposed to anaphylactic episodes sho

Anaphylaxis.

The syndrome of anaphylaxis is a life-threatening event in which the potential for patient morbidity and mortality is high. An understanding of the pathophysiology of anaphylaxis, the most serious of the allergic disorders, is paramount for its diagnosis. In addition to these elements, this article discusses newly recognized causes of anaphylaxis and reviews its treatment.

Occupational anaphylaxis-Data from the anaphylaxis registry.

BACKGROUND: Epidemiologic data on occupational anaphylaxis is scarce, and there is a need of more knowledge about work-related anaphylactic episodes. METHODS: Based on the data of the Anaphylaxis Registry, we identified cases related to occupational exposure and analyzed the elicitors, demographics, severity of clinical reaction and management. RESULTS: Since 2017, 5851 cases with an information about the occupational relation of the anaphylactic episode were registered whereby 225 (3.8%) were assigned to be caused by an occupational allergen. The vast majority of these occupational anaphyla

Refractory Anaphylaxis and Near-Fatal Anaphylaxis: Two Overlapping Clinical Observations of Severe Anaphylaxis.

The terms refractory anaphylaxis and near-fatal anaphylaxis have gained prominence in the last 5-10 years. Refractory anaphylaxis has a clinical justification in that patients often do not respond to successive doses of adrenaline. Near-fatal anaphylaxis facilitates the study of fatal anaphylaxis, which is 10 times less frequent. In terms of epidemiology, refractory anaphylaxis and near-fatal anaphylaxis overlap, since refractory anaphylaxis can prove fatal, and the more severe the anaphylaxis (including cardiorespiratory arrest), the greater the need for adrenaline and fluid infusion. However

Venom Anaphylaxis.

Allergy to venoms of stinging insects (bees, yellow jackets, hornets, wasps, and stinging ants) causes large local reactions or systemic reactions, including anaphylaxis. Diagnostic evaluation includes skin testing and/or serum venom-specific IgE and basal serum tryptase (to identify underlying mast cell disorders). Patients with sting anaphylaxis should be prescribed epinephrine and venom immunotherapy which prevents allergic reactions in up to 98% of patients. Most patients may safely discontinue venom immunotherapy after 5 years, although high-risk patients need to continue indefinitely. Fu

Idiopathic anaphylaxis.

Idiopathic anaphylaxis (IA) is defined as anaphylaxis without any identifiable precipitating agent or event. The clinical manifestations of IA are the same as allergen-associated (immunologic) anaphylaxis and include urticaria, angioedema, hypotension, tachycardia, wheezing, stridor, pruritus, nausea, vomiting, flushing, diarrhea, dysphagia, light-headedness, and loss of consciousness. Patients usually tend to have the same manifestations on repeated episodes. IA is a prednisone-responsive disease that is ultimately a diagnosis of exclusion. Approximately 40% of patients are atopic. Serum tryp

9. Anaphylaxis.

Anaphylaxis is an acute-onset, potentially fatal systemic allergic reaction. It is usually triggered by an agent such as an insect sting, food, or medication, through a mechanism involving IgE and the high-affinity IgE receptor on mast cells or basophils. Less commonly, it is triggered through other immunologic mechanisms, or through nonimmunologic mechanisms. It often occurs in community settings. Anaphylaxis episodes range in severity from those that are mild and resolve spontaneously to those that are fatal within minutes. The clinical diagnosis is based on a meticulous history and physical

Food anaphylaxis.

Anaphylaxis is a life-threatening allergic reaction, and food is one of the most common responsible allergens in the outpatient setting. The prevalence of food-induced anaphylaxis has been steadily rising. Education regarding food allergen avoidance is crucial as most of the fatal reactions occurred in those with known food allergies. The lack of a consensus definition for anaphylaxis has made its diagnosis difficult. Symptoms affect multiple organ systems and include pruritus, urticaria, angioedema, vomiting, diarrhoea, abdominal cramps, respiratory difficulty, wheezing, hypotension, and shoc

[Idiopathic anaphylaxis].

Idiopathic anaphylaxis (IA) has been described as a clinical entity by North America authors. The symptoms are brisk and relapsing episodes of angioedema, urticaria, bronchospasm, digestive symptoms, cardio-vascular collapse. Adults, females more than males, are mainly concerned. The seriousness is linked to laryngeal angioedema or to hypotension. Nevertheless lethality is rare. The first step of the procedure is aimed at the detection of several rare pathologies mimicking IA: the paroxystic capillary leak syndrome induced by a monoclonal gammapathy or by a release of cytokines, a carcinoid sy

Idiopathic anaphylaxis.

Anaphylaxis represents the maximal variant of an immediate-type allergic reaction involving the whole organism with manifestations in different organ systems. IgE-mediated mast cell and basophil activation is the major pathomechanism; however, immune complex and pseudo-allergic reactions also may lead to the same symptomatology. The most common elicitors are drugs, additives, occupational substances, animal venoms, aeroallergens, and contact urticariogens but also physical factors (cold, heat, ultraviolet light, exercise). When no eliciting factors can be detected, the term "idiopathic anaphyl

Latex anaphylaxis.

In the past decade, immediate hypersensitivity to latex has been accepted as a serious problem. The proteins that are present in natural rubber are considered to be the responsible antigens, and the hypersensitivity responses induced include contact and generalized urticaria, angioedema, rhinitis, conjunctivitis, bronchospasm, life threatening anaphylaxis and death. So far, children with spina bifida or severe urogenital defects, health care workers, and rubber industry employees appear to be at greater risk than the general population. Since 1991 several reports and cross-reactivity studies h
