Ideomotor apraxia: a review.

Ideomotor apraxia (IMA) is a disorder traditionally characterized by deficits in properly performing tool-use pantomimes (e.g., pretending to use a hammer) and communicative gestures (e.g., waving goodbye). These deficits are typically identified with movements made to verbal command or imitation. Questions about this disorder relate to its diagnosis, anatomical correlates, physiological mechanisms involved, and the patients in whom IMA is best characterized. In this review, utilizing information presented at an international workshop, we summarize the present state of knowledge about IMA. We

Ideomotor apraxia and functional ability.

The impact of ideomotor apraxia (IMA) on functional ability has been a relatively neglected topic in research. This has been due to the continued focus on performance on gesture imitation and pantomime of tool-use, together with widespread acceptance of anecdotal evidence that IMA has no effect when directly manipulating objects. An increasing number of studies have shown that IMA does in fact result in increased clumsiness when handling objects and may contribute to disability in everyday life. However the effect seems relatively mild compared to the stark abnormalities on gesture imitation a

Ideomotor apraxia in Huntington's disease.

The pattern of movement errors in ideomotor apraxia suggests an abnormality in selection and sequencing of component movements. Individuals with Huntington's disease were evaluated prospectively for the presence of apraxia, and aspects of motor and cognitive function were correlated with apraxic errors. Based on a conservative apraxia rating, ideomotor apraxia occurred in three (33%) of nine patients with a mean duration of disease of 10.4 years. Only two (22%) individuals made no apraxic errors, however, and the group as a whole made apraxic errors in 26% of movements. Apraxia was associated

Motor learning in ideomotor apraxia.

Using Luria's motor sequence test, we studied the learning ability of 34 right-handed patients with unilateral hemisphere lesions. Patients with ideomotor apraxia needed 4 to 6 trials in the test, while the remainder needed only 1 or 2. Ideomotor apraxics also required more time to complete the test. The lesions of patients who failed to master this test were not always found in the frontal lobes. These results suggest that the ability to learn motor sequences is impaired in apraxic patients and that the left hemisphere of the brain plays the major role in learning a new motor sequence. Patien

Pantomime comprehension and ideomotor apraxia.

In a prior study it was shown that apraxic patients with posterior infarctions that included the parietal lobe could not discriminate between gestures. In this study these observations were replicated using a nonverbal paradigm in which the subjects did not have to discriminate between gestures, but instead had to comprehend their meaning. Pantomimed acts on videotape were shown to six apraxic-aphasic patients, seven nonapraxic-aphasic patients, and six normal subjects. Four drawings were also shown, one of which matched the pantomime (for example, if the pantomime was of hammering, one drawin

Two forms of ideomotor apraxia.

Destruction of parietal areas containing visuokinesthetic motor engrams, where motor acts may be programmed, should be distinguishable from apraxia induced by disconnection of these parietal areas from frontal motor areas. Destruction should result in inability to distinguish well-performed from poorly performed movements, whereas disconnection should not. We gave movement and act-discrimination tasks to apraxic and nonapraxic patients with anterior lesions or nonfluent aphasia, and to patients with posterior lesions or fluent aphasia. On both tasks, the performance of posterior/fluent patient

Axial movements in ideomotor apraxia.

Non-symbolic axial movements were examined and compared to oral and limb movements in a group of 60 aphasic patients (15 of each major subgroup) with exclusively left-sided brain damage. The contention in the literature that axial movements are preserved in patients with ideomotor limb apraxia was not confirmed.

Sensorimotor learning in ideomotor apraxia.

Sensorimotor learning ability in patients with ideomotor apraxia was investigated, using as subjects, eight patients with ideomotor apraxia, 8 with aphasia without ideomotor apraxia, and 8 normal controls. The aphasia, apraxia, and normal control groups were matched for age, sex, and education. The aphasia and apraxia groups were chosen to control for lesion size and scores on Kimura's recurring figure test, the Token test, and intelligence. The mirror-aiming test was performed and the learning effect in terms of decreases in total time, the number of errors, and the times for errors were exam

Ideomotor apraxia in Alzheimer's disease.

Ideomotor apraxia, tested on verbal command and by imitation, was checked in 23 patients suffering from dementia of Alzheimer's type of different severity and in 17 age-matched controls. A significant deterioration of ideomotor praxis could be shown even in mild dementia. Correlations of ideomotor apraxia and aphasia, tested by the Token test were found to be significant.

Ideomotor apraxia: a call to action.

Although ideomotor apraxia (IM) has been a topic of investigation since the early 20th century, progress in studying the disorder has been hindered by important (and sometimes unarticulated) differences in the underlying theoretical models of various investigative groups. As a result, it is difficult to draw conclusions about the expected performance of IM patients on various tests of praxis (e.g. pantomime, single object use, gesture imitation, and naturalistic action); the relationship of IM to ideational apraxia (IA); the degree to which specific error types are diagnostic of one or the oth

Motor Adaptation Deficits in Ideomotor Apraxia.

OBJECTIVES: The cardinal motor deficits seen in ideomotor limb apraxia are thought to arise from damage to internal representations for actions developed through learning and experience. However, whether apraxic patients learn to develop new representations with training is not well understood. We studied the capacity of apraxic patients for motor adaptation, a process associated with the development of a new internal representation of the relationship between movements and their sensory effects. METHODS: Thirteen healthy adults and 23 patients with left hemisphere stroke (12 apraxic, 11 nona

Callosal ideomotor apraxia in Alzheimer's disease.

BACKGROUND/OBJECTIVE: Impaired ability to perform skilled movements with the left upper limb in patients with corpus callosum injury has been well described (callosal apraxia) with some displaying spatial-temporal errors primarily in response to verbal commands (verbal callosal disconnection apraxia), with imitation, and when using actual tools (callosal ideomotor apraxia). Additionally some patients with callosal injury also make content errors when selecting and using the incorrect tool with their left upper limb (callosal conceptual apraxia). Interestingly, patients with Alzheimer's disease

Defective motor learning in ideomotor apraxia.

Liepmann suggested that the left hemisphere contained the engrams for motor sequences. Other investigators have suggested that ideomotor apraxia may be caused by either a destruction of these engrams or a disconnection of these engrams from motor systems in the nondominant hemisphere. If these hypotheses are correct, then ideomotor apraxics should not only show a defect on previously learned motor tasks but also a defect in new motor learning. Nine right-handed, hemiparetic, aphasic apraxics were given six trials on a rotary pursuit meter. Eight right-handed hemiparetic, aphasic, nonaprixic pa

Ideational and ideomotor apraxia: a qualitative analysis.

This paper reviews research into both ideational and ideomotor apraxia focusing on the qualitative aspects of limb apraxia. The criteria based on a distinction between different type of movements which have been used to distinguish between ideational (IA) and ideomotor apraxia (IMA) are reviewed together with an evaluation of the most frequent methods of testing and their effects on patients responses. Finally a list of the types of response reported in the literature is presented and their relationship with IA and IMA is proposed.

Neuropsychological and neuroanatomical dimensions of ideomotor apraxia.

Fifty-five right-handed men with left hemisphere stroke were systematically investigated for ideomotor apraxia of various body parts. Standardized aphasia and apraxia examinations in all cases, and appropriately timed CT in 28 cases, were used for analysis of psychological and anatomical properties of ideomotor apraxia. It is not possible to ignore the modality of eliciting the movement (command or imitation) and the body part being moved (buccofacial, limb or whole body) from any comprehensive theory about ideomotor apraxia. Different levels of performance are seen with different body parts t
