We have investigated various types of blood culture bottles which are mainly used at present and posed problems present in the blood culture bottles. First, there are differences between resin and ecosorb in the ability to adsorb and inactivate antibiotics in the blood. Second, the delay in placing the bottle (into which blood was inoculated) to the automatic instrument (delay in the start of incubation) greatly affects the automatic detection by BACTEC system and shows false negatives. Third, when the same blood is incubated in plural bottles (aerobic and anaerobic bottles), the differences a…

OBJECTIVE: Assess clinical outcomes, knowledge, and attitudes about shortage mitigation interventions during a nationwide blood culture bottle shortage. We hypothesized that efforts to target ordering of blood cultures would not change patient-oriented outcomes.
DESIGN: Retrospective, noninferiority, multicenter study.
SETTING: Tertiary, rural healthcare system.
PATIENTS: Eligible patients had any culture ordered before and after mitigation strategies.
INTERVENTIONS: Mitigation strategies were rapidly implemented to preserve the availability of blood culture bottles for high-risk patients.…

There are no hard-and-fast rules for judging whether an organism is a pathogen or contaminant. Experience is critical. In the end, the physician is responsible for determining whether an organism is a contaminant or a pathogen on the basis of laboratory and clinical data, which often include patient history, physical examination, body temperature, peripheral leukocyte count and differential, clinical course, and culture results. When blood culture results are given to a nurse, she or he should communicate this information to the attending physician as soon as possible, with greatest emphasis p…

A blood culture system was developed in which a lysing agent (either Tween 20, one of several other polyoxyethylene adducts, digitonin, or Triton X-100) is added to the blood culture medium. Of 33 Triton compounds, 9 lysed human blood, as did 7 of 21 polyoxyethylene compounds and digitonin, all at a concentration of 0.05%. Under the specific test conditions, three of the hemolytic polyoxyethylene compounds and digitonin had no inhibitory effect. All of the Triton compounds had at least some inhibitory effect on the most sensitive of the pathogenic bacteria that were tested, Streptococcus pneum…

For blood cultures in septicemic patients it is advisable to use two broth culture bottles. One bottle should be incubated with air, the other without (for anaerobes). Supplemented Peptone Broth II (Becton-Dickinson) and BHI Roche were superior for culturing aerobic and anaerob bacteria in comparison to other blood culture media. They are commercially available, and with these media it is not necessary to inoculate two bottles with blood from one patient. Supplemented Peptone Broth II is also suitable very well for the isolation of Campylobacter fetus and has a redoxpotential which is especial…

Spiral filaments seen in the blood cultures of two patients with fever and jaundice were initially thought to be leptospires; these were later proved to be artefacts. An investigation was carried out to exclude the possibility of laboratory contamination of the culture media and to find out how these objects were produced. The significance of the findings is discussed in relation to the possibility of a mistaken diagnosis in routine laboratories which have a limited experience of leptospires.

BACKGROUND: Blood cultures are central to diagnosing bloodstream infections and guiding appropriate treatment. Because microbial concentrations in blood are low, culturing is necessary for any downstream analysis, and rapid detection of microbial growth is essential. Automated systems, such as those for blood culture (ie, blood culture automats) facilitate rapid detection, but are not equally available worldwide; their implementation in low-income and middle-income countries (LMICs) is hindered by complex supply chains, low availability of equipment and consumables, such as blood culture bottl…

BACKGROUND: The optimal use of blood cultures to determine the etiology of febrile episodes in neutropenic children has not been well-defined.
METHODS: Single volume blood cultures using the Pediatric ISOLATOR System (ISO), were compared with variable, weight-based culture volumes using the BACTEC 9240 Culture System (BAC). Additionally the value of routinely inoculating the BACTEC MYCO/F LYTIC culture vial (MFL) as well as the BACTEC AEROBIC/F culture vial (AF) was examined.
RESULTS: A total of 2620 cultures had both ISO and BAC inoculated; 182 cultures were positive (7.0% of cultures); 97.…

BACKGROUND: Harboring a high mortality, the incidence of sepsis is increasing; thus detection, identification and susceptibility tests of the involved microorganisms become urgent.
METHODS: We reviewed the records from January 2013 until July 2014 of a total of 4110 blood culture bottles taken from adult patients in a private tertiary hospital.
RESULTS: Growth of microorganisms was observed in 559 bottles (12.6%). We emphasize that 2648 blood cultures (60%) were taken in two paired aerobic and anaerobic bottles drawn at the same time (1324 pairs); from these, growth was observed in 182 inocu…

The clinical and laboratory issues important in pediatric blood cultures are similar to those in adult blood cultures with a few noteworthy exceptions. The collection of an uncontaminated specimen and an ample volume of blood is more difficult, especially in neonates. In addition, children often have previously received oral antibiotics or a broad-spectrum parenteral antibiotic. The relative frequencies of the pathogens causing bacteremia in children are different in important ways from in adults. Haemophilus influenzae b, although much less common than in the past, is still an important pedia…

Blood cultures are among the most commonly ordered tests in hospitals, frequently carried out as a matter of routine whenever a patient has a fever. However, only 5% of these are positive, and nearly half could be avoided given the lack of clinical indication. This article proposes a structured approach to determining the indication for taking blood cultures, based on the probability of detecting bacteremia according to symptoms, the suspected site of infection and laboratory parameters, on the availability of diagnostic alternatives, and on the potential impact of the result on patient manage…

Blood culture contamination (BCC) has historically been a major issue with clinical and financial implications. Recent studies have shown that initial specimen diversion devices (ISDDs) can help reduce BCC, but a recent large study by C. Vieni, M. Legried, J. Mainella, K. Bute, et al. (J Clin Microbiol 64:e00085-26, 2026, https://doi.org/10.1128/jcm.00085-26) demonstrates that a major concern with ISDD implementation is blood culture collector dissatisfaction with the utilization of ISDDs and subsequent avoidance when use is optional. A variety of local factors may limit the potential impact o…

Increase in cryptococcal infection has been noticed after acquired immunodeficiency syndrome pandemic. Cryptococcus neoformans can be isolated from blood in the process of dissemination to brain. We report a case of cryptococcal fungaemia in a patient whose cerebrospinal fluid was negative for Cryptococcus neoformans. Retrospective analysis revealed human immunodeficiency virus seropositivity of the patient. He was treated with amphotericin B and fluconazole. Antiretroviral therapy was started, however, the patient succumbed to the infection.

Infective endocarditis (IE) is a systemic septic disease that is a microbial infection of the endothelial surface of the heart. Despite advances in medical, surgical, and critical care interventions, IE remains a life-threatening illness. Therefore, it is important to promptly diagnose it using the modified Duke criteria. These criteria integrate factors predisposing patients to the development of IE, the blood-culture isolates and persistence of bacteremia, and echocardiographic findings, along with other clinical and laboratory information. Positive blood culture and a positive echocardiogra…
