Blood pressure variability of two ambulatory blood pressure monitors.

OBJECTIVE: There are no data on the evaluation of blood pressure (BP) variability comparing two ambulatory blood pressure monitoring monitors worn at the same time. Hence, this study was carried out to compare variability of BP in healthy untreated adults using two ambulatory BP monitors worn at the same time over an 8-h period. METHODS: An Accutorr device was used to measure office BP in the dominant and nondominant arms of 24 participants.Simultaneous 8-h BP and heart rate data were measured in 24 untreated adult volunteers by Mobil-O-Graph (worn for an additional 16 h after removing the Sp

The quest for accurate wearable blood pressure monitors.

The automated cuff-oscillometric blood pressure (BP) measurement currently is the standard method for evaluating hypertension using office, home and ambulatory measurements. It eliminated observer-related issues of the auscultatory method and made out-of-office BP monitoring feasible. But there are issues with this method, mainly due to the limited BP information they provide, the use of an inflatable cuff, and because measurements are provided only in static conditions. Cuffless wearable devices can provide complete information of the individual's BP profile and behavior, for days, weeks, mon

Measurement variation among 12 electronic home blood pressure monitors.

Although there are AAMI and BHS standards for accuracy of electronic home electronic blood pressure monitors (HBPM), patient composition differences and differences in manufacturer's algorithm for calculation of the systolic and diastolic measurement may result in measurement differences between monitors. The aim of this study was the measurement of differences among HBPM. Paired comparisons were performed between the Omron 712c electronic home monitor and each of 12 other HBPM (Sunbeam 7654, Sunbeam 7623, Omron 711, Omron 432c, A&D-UA767, Lumiscope 1085M, Omron 725CIC, Assure A30, Lumiscope 1

Accuracy of home blood pressure readings: monitors and operators.

OBJECTIVE: To evaluate the accuracy of automated digital blood pressure monitoring devices and operators in the community. Also, we tested the effects of a simple education program, and looked for arm-arm differences. DESIGN: Subjects who had bought their own automated digital blood pressure monitor were recruited via an advertisement in the local newspaper. On arrival, they were asked to record their blood pressure exactly as they would at home. The investigator noted any technique deficiencies then corrected them. Blood pressures were then recorded by the investigator and the subject, on op

Accuracy of drug store blood pressure monitors: an observational study.

OBJECTIVES: Publically accessible blood pressure monitors are widely used, but little information is available on their accuracy. We compared blood pressure readings of 17 drug store monitors with those obtained using a validated home monitor (Omron BP742CAN) and both with those taken at home using the Canadian Hypertension Education Program protocol. MATERIALS AND METHODS: Duplicate readings were taken using the drug store monitor (VitaStat, n=6, and PharmaSmart, n=11) on the left arm and the Omron on the right in three participants: two normal and one untreated hypertensive patient. We used

How and why do patients use home blood pressure monitors?

INTRODUCTION: Little is known about how and why patients use home blood pressure monitoring (HBPM). We investigated from where patients obtain their monitor, their reasons for using HBPM, and their frequency of performing blood pressure (BP) measurements. We also examined whether those using HBPM provide written reports of measurements to their doctor and whether they have ever had their monitor checked. METHODS: We conducted a cross-sectional mail survey of adult patients with hypertension enrolled in a practice-based research network of 24 primary care practices throughout the state of Nort

Accuracy of ambulatory blood pressure monitors in routine clinical practice.

BACKGROUND: To determine the extent of discrepancies between ambulatory blood pressure measurement (ABPM) devices and mercury sphygmomanometry in pre-use testing in routine clinical practice and whether such discrepancies are associated with patient characteristics and subsequent 24-h ABPM readings. METHODS: A retrospective analysis of a database of 683 prospectively collected records was performed. The study population comprised of patients referred for ABP monitoring at a Sydney teaching hospital. Patients had same-arm sequential measurements with a mercury sphygmomanometer and Spacelabs 90

Comparison of two oscillometric blood pressure monitors in subjects with atrial fibrillation.

OBJECTIVE: To compare blood pressure readings obtained with two commonly used oscillometric monitors: Omron HEM 711 AC (OM) and Welch-Allyn 52000 series NIBP/oximeter (WA) with mercury sphygmomanometers (Merc) in subjects with atrial fibrillation. METHODS: We recruited 51 hemodynamically stable subjects with atrial fibrillation. Fifty four subjects in normal sinus rhythm served as controls. Supine blood pressure readings in each arm were recorded simultaneously using one monitor and Merc. The second monitor then replaced the first and readings were repeated. Merc was then switched to the oppo

Ambulatory blood pressure monitoring: mean blood pressure and blood pressure load.

Ambulatory blood pressure monitoring (ABPM) is commonly used to diagnose pediatric hypertension. Using ABPM, hypertension is usually defined as a mean BP greater than the 95th percentile for height. A BP load >30% (% of BP readings greater than the 95th percentile) is also used for the diagnosis of hypertension. The objective of this study was to determine the agreement between mean BP greater than the 95th percentile and 30% BP load for the diagnosis of hypertension using ABPM. All ABPM records (n =1,009) of patients referred for hypertension to a pediatric center were retrieved. Scans were e

[What are the therapeutic consequences of evaluating hypertensive patients with 24-hour blood pressure monitors and blood pressure self-measurement?].

Arterial hypertension is an important risk factor for excessive cardiovascular morbidity and mortality due to its high prevalence of about 20% in the adult population. Causal readings, which have been obtained for diagnosis and control of treatment in hypertension are of limited value. They are not reproducable due to physiologic variability of blood pressure, which causes a rise of blood pressure, if a straining situation (e.g. in doctor's office) occurs (white-coat-hypertension). Furthermore, no correlation between causal readings and signs of endorgan-damage (EOD) such as left ventricular h

The Post-Marketing Real-World Outpatient Clinical Validation of Electronic Blood Pressure Monitors.

This study evaluated the accuracy of a variety of home upper-arm oscillometric blood pressure monitors (BPMs) from 448 participants against the FDA-certified XYZ110 auscultatory device using simultaneous measurements. Descriptive statistics were used to summarize the key findings of the study. Device performance was evaluated referencing international standards (ISO 81060-2:2013), where a mean absolute difference of ≤5 mmHg was defined as "Accurate." In addition, survey questionnaires completed by a subset of participants were analyzed to provide supplementary insights. A total of 448 consecut

Empowering Low-Income Patients with Home Blood Pressure Monitors to Improve Hypertension Control.

INTRODUCTION: Effective management of hypertension (HTN) is a priority in primary care. With telehealth now considered a staple care delivery method, uninsured and low-income patients without home blood pressure (BP) monitors may need additional attention and resources to achieve successful HTN control. METHODS: This prospective study at an underserved community clinic assessed the impact of distributing free BP monitors on patients' HTN control and therapy adherence. Enrollees were randomized into 2 groups, both completing 4 primary care physician (PCP) visits over a 6-month study period. In

Improving Access to Home Blood Pressure Monitors at a Federally Qualified Health Center.

Background: Self-monitoring of blood pressure (BP) clinically decreases BP. However, cost can limit access, especially in underserved populations. Objective: This mixed-methods pilot study aims to determine the impact of providing home BP monitors free of charge to patients at a federally qualified health center (FQHC) by quantifying the effect on BP and surveying patients to measure satisfaction and engagement. Methods: One hundred eighty patients with clinically diagnosed hypertension received BP monitors. Patient charts were reviewed to collect demographics and office BP readings 3 months b

A simulation-based evaluation of nine oscillometric blood pressure monitors for self-measurement.

OBJECTIVE: To evaluate the performance of nine self-measurement oscillometric blood pressure monitors using a simulator. METHODS: For each monitor, simulation data from 48 sets of simulated waveforms with four simulations for each set were used for analysis. The waveforms represent a wide range of blood pressure. The monitor-simulator blood pressure differences were analyzed according to the 1992 Association for the Advancement of Medical Instrumentation (AAMI) and 1993 British Hypertension Society (BHS) protocols, except that corrections were made to take into account simulation variability.

Home Blood Pressure Monitoring.

The diagnosis and management of hypertension have been based primarily on blood pressure (BP) measurement in the office setting. Higher out-of-office BP is associated with an increased risk of cardiovascular disease, independent of office BP. Home BP monitoring (HBPM) consists of the measurement of BP by a person outside of the office at home and is a validated approach for out-of-office BP measurement. HBPM provides valuable data for diagnosing and managing hypertension. Another validated approach, ambulatory BP monitoring (ABPM), has been considered to be the reference standard of out-of-off

Home blood pressure monitoring.

Blood pressure measurements made in the physician's office have been the standard for diagnosing hypertension and determining the efficacy of antihypertensive drug therapy. However, these measurements do not necessarily reflect a patient's usual blood pressures in other circumstances, such as at work or in the home. Accurate blood pressure measurements performed by the patient or a family member in an out-of-office setting can be integrated with office readings to assist in the management of hypertension.

Ambulatory blood pressure monitoring.

Non-invasive measurement of blood pressure in ambulatory humans began in the 1960s. Ambulatory devices have been modified over the years and are now pocket-sized, with almost noiseless pumps. Their accuracy must be validated by independent laboratories using a standardised protocol. Twenty-four-hour ambulatory monitoring offers considerable advantages over conventional measurement, including avoidance of error associated with conventional measurement, elimination of white-coat effect and the provision of a series of blood pressure readings over the time period rather than a one-off measurement

Blood pressure measurement and monitoring.

Recent developments in blood pressure measurement have emphasized recordings taken outside the physician's office. Such readings can recognize the spuriously high levels recorded in "white coat" hypertension. Nocturnal and early morning readings can detect silent complications and impending cardiovascular accidents.
