STUDY DESIGN: Fourteen cadaveri specimens were sectioned to analyze the internal architecture of the human calcaneus. We described the arrangement and orientation of trabecular patterns within the calcaneus and made multiple measurements of its cortical thickness.

Fractures of the calcaneus generally occur in the setting of high-energy trauma, resulting in complex, three-dimensionally oriented fracture patterns. The authors present the results of another investigation of a group of casualties with fractures of the calcaneum treated by open reposition and stabilization with a splint and screws in 1994 to 1996, followed up after an interval of 57-74 months after the injury, on average after 66 months. From August 1992 to March 1997, 66 patients with 71 displaced intraarticular calcaneal fractures were prospectively examined after an operative treatment using an extended lateral approach and the ASIF calcaneal plate followed by early functional postoperative treatment (mean follow-up 25 months, retrieval rate 96%). Calcaneal stress fracture is an injury that particularly affects athletes and runners owing to repeated stress on the lower extremities without sufficient recovery time. Medial and lateral heel pain during activity raises suspicion of the injury, but radiological examinations are required for a definitive diagnosis. The treatment is usually conservative, involving rest and dietary correction, and a pharmacological or surgical approach is only relevant in some few cases.
