Internal Carotid Artery Hypoplasia Misidentified as Internal Carotid Artery Dissection.

Agenesis or hypoplasia of the internal carotid artery (ICA) may easily be confused with dissection or occlusion. We report a case of a 24-year-old female with complaint of acute left-hand hypoesthesia and a history of occasional intermittent numbness of her right hand with myoclonic jerking. Because previous imaging studies over 2 years were interpreted as occlusion of the left ICA secondary to carotid dissection, the treating physician had prescribed anticoagulant therapy. During transcranial Doppler (TCD) examination, the spectral waveform was unexpectedly normal, prompting a repeat review o

Internal carotid artery dissection.

The purpose of this review is to increase the awareness of internal carotid artery dissection (ICAD), a potentially serious and probably underdiagnosed condition. ICAD is a not uncommon cause of stroke in young patients. ICAD may occur spontaneously or as a result of trauma. However, the "spontaneous" dissection is often preceded by a trivial trauma. The typical patient presents with ipsilateral headache or neck pain, ipsilateral Horner's syndrome and delayed ischemic symptoms from the ipsilateral hemisphere or retina. Conventional angiography, the gold standard for diagnosis, tends to be repl

[Internal carotid artery dissection].

Arterial dissection results from bleeding into the vessel wall. Some cases are associated with cervical trauma or have evidence of an underlying vascular disease; many occur without any history of injury or detectable arterial disease. Among the cervical cephalic arteries, the extracranial segment of the internal carotid artery is the vessel most commonly involved; intracranial carotid dissections are much rare. Carotid dissection occurs predominantly in young or middle-aged adults and shows no sex predominance. Although clinical manifestations can be extremely diverse (from isolated headache

Internal carotid artery hypoplasia.

Hypoplasia of the carotid arteries is a rare congenital anomaly which when clinically symptomatic presents as cerebral ischemia or hemorrhage. Patients with this anomaly commonly have associated variations of the circle of Willis, an increased incidence of intracranial aneurysms and extensive networks of collateral vessels to accommodate the diminished carotid flow. Carotid artery hypoplasia should not be confused with other conditions which may have a similar appearance such as arterial dissection, vasculitis, and tubular fibromuscular hyperplasia.

[Internal carotid artery revascularization].

Carotid endarterectomy remains the standard revascularization technique for the prevention of ischemic stroke resulting from severe carotid stenosis. Surgery is highly beneficial in patients with a symptomatic stenosis of 70% or greater that is not a total or near-total occlusion. The benefit becomes more diluted in patients with a symptomatic 50-69% stenosis, and surgery has no effect, or even increases the risk of stroke, in those with a less than 50% stenosis. Surgery has also been shown to reduce the risk of stroke in asymptomatic patients with a 60-99% stenosis, but the absolute benefit i

Internal carotid artery epistaxis.

Epistaxis from the internal carotid artery (ICA) or bleeding from the ICA at the skull base is a rare, frightening, and difficult management problem. We present five cases, with a variety of causes--in all of which the patients survived massive hemorrhage--and suggest a protocol for management of the condition.

External carotid endarterectomy with internal carotid artery transposition flap angioplasty for symptomatic internal carotid artery occlusion.

The external carotid artery is an important collateral pathway for cerebral perfusion when the internal carotid artery is occluded. After internal carotid artery occlusion, there is a definite risk of ipsilateral neurological events. The authors retrospectively examined their experience with endarterectomy of the external carotid artery for symptomatic internal carotid artery occlusion. Results based on the authors' experience and on historical data show external carotid endarterectomy to be a safe procedure. Obliteration of the cul-de-sac appears to be a very important factor in the preventio

Ophthalmic artery originating from contralateral internal carotid artery associated with ipsilateral agenesis of internal carotid artery.

Congenital absence of the internal carotid artery is a rare anatomical variant. The primarily reported origins of the ophthalmic artery associated with this variant are the posterior communicating, middle cerebral, and external carotid arteries. We report the case of a patient without an internal carotid artery whose ophthalmic artery originated from the contralateral internal carotid artery. The superior hypophyseal and prechiasmal arteries may contribute to collateral flow to the orbit from the contralateral internal carotid artery.

[A Case of Carotid Endarterectomy for the Internal Carotid Artery Stenosis Associated with Twisted Internal Carotid Artery].

Twisted carotid artery bifurcation is a variant in which the internal carotid artery(ICA)courses mediall to the external carotid artery(ECA). Here, we report a case of ICA stenosis, which is a complication of twisted ICA, treated with carotid endarterectomy(CEA). A 75-year-old woman was brought to our hospital with left-sided hemiparesis. MRI diffusion-weighted image revealed right frontal lobe infarction and high-grade stenosis at the origin of the right ICA. The ICA was located inside in relation to the ECA, manifesting as twisted ICA. We diagnosed her with cerebral infarction, caused by the

Anomalous External Carotid Artery-Internal Carotid Artery Anastomosis in Two Patients with Proximal Internal Carotid Arterial Remnants.

Two angiographic instances of anomalous external carotid artery (ECA) and internal carotid artery (ICA) anastomosis are described, each occurring at the C2-3 level and bearing remnants of proximal ICA. The ICA remnant of one patient (identifiable immediately upon bifurcation of the common carotid artery) was hypoplastic, and that of the other patient was an occluded arterial stump. These features are not typical of non-bifurcating ICA. The occipital artery originated from an anomalous connection in one instance and from the main trunk of the ECA (just past the ECA-ICA connection) in the other.

External carotid artery stenting in symptomatic internal carotid artery occlusion.

We report the case of a patient with an anterior ischemic stroke due to tandem occlusion of the left M2 segment and ipsilateral internal carotid artery (ICA), with concomitant severe stenosis of the ipsilateral external carotid artery (ECA) and contralateral ICA, and moderate stenosis of the left vertebral artery (VA); as thrombectomy was not possible, stenting of the right ICA was performed. Two days after significant recovery, the patient showed neurological deterioration when in upright position, and brain magnetic resonance imaging confirmed decreased cerebral blood flow on the left hemisp

Intracavernous internal carotid artery pseudoaneurysm.

Epistaxis is commonly encountered in otorhinolaryngologic practice. However, severe and recurrent epistaxis is rarely seen, especially that originating from a pseudoaneurysm of the intracavernous internal carotid artery (ICA). We herein present the case of a 32-year-old man who was involved in a motor vehicle accident and subsequently developed recurrent episodes of profuse epistaxis for the next three months, which required blood transfusion and nasal packing to control the bleeding. Computed tomography angiography revealed a large intracavernous ICA pseudoaneurysm measuring 1.7 cm × 1.2 cm ×

Spontaneous internal carotid artery dissection.

Once considered uncommon, spontaneous dissection of the carotid artery is an increasingly recognized cause of stroke, headache, cranial nerve palsy, or ophthalmologic events, especially in young adults. Even in the presence of existing signs and symptoms, the diagnosis can be missed by experienced physicians of all specialties. We report a case of spontaneous internal carotid artery dissection in a 38-year-old woman with a cortical stroke and visual disturbances as initial symptoms. The diagnosis was confirmed by magnetic resonance imaging/angiography and by angiography. Prompt anticoagulation

Extracranial internal carotid artery aneurysms.

Extracranial internal carotid artery aneurysms are uncommon. The symptoms of these aneurysms vary according to their location and size. Larger aneurysms may present as cervical or parapharyngeal masses, which may or may not be pulsatile or tender; there may be an associated systolic bruit. Pharyngeal hemorrhage, epistaxis, or bleeding from the ear are rare manifestations. Ischemic symptoms are a frequent mode of presentation of these aneurysms. Arterial diseases, particularly atherosclerosis, may predispose the vessel to the formation of the aneurysm. Sometimes trauma is the cause. We report h

Internal Carotid Artery Occlusion: Management.

PURPOSE OF REVIEW: Acute internal carotid artery occlusion (ICAO) is associated with high rates of morbidity and mortality, whereas chronic ICAO can present insidiously with recurrent strokes. In this review, we discuss the medical and surgical management approaches among patients with acute, subacute, and chronic ICAO. RECENT FINDINGS: We reviewed the recent literature regarding clinical presentation of acute and chronic cases of ICAO, and discuss the current data, accepted guidelines, and prospects. Surgical, endovascular, or a combination (hybrid) revascularization has been shown to be eff

Right internal carotid artery agenesis.

Congenital absence of one or both carotid arteries is a rare anomaly. It is usually discovered incidentally by computed tomography, or magnetic resonance imaging of head and neck taken for some other reason. Most of the patients are asymptomatic, although there is a close association of intracranial aneurysms and subarachinoid hemorrhage in many patients. Here we report a case of right internal carotid agenesis in a young female associated with recent onset amnestic syndrome.
