Tumors of the cerebral aqueduct.

Two cases of tumor of the cerebral aqueduct are described. Case 1 is a pilocytic astrocytoma in a 16-year-old girl with a two-year history of intermittent increase of intracranial pressure. The tumor was completely confined within the lumen of the aqueduct. Case 2 is a subependymoma of a 68-year-old man. The tumor extended beyond the aqueduct to the periaqueductal gray matter and produced signs and symptoms suggesting normal pressure hydrocephalus. The literature contains 18 other cases of tumor of the aqueduct: 13 gliomas and five vascular malformations. All, except one, produced clinical man…

Endoscopic anatomy of the cerebral aqueduct.

OBJECTIVE: What is known about the cerebral aqueduct is derived mainly from the legacy of classic histology and from the most recent advanced neuroimaging technologies. In fact, although this important structure is frequently glimpsed by neurosurgeons, only limited anatomic contributions have been added by microsurgery to its direct in vivo description. A review of our surgical experience in navigating the fourth ventricle prompted us to revisit the classical anatomic descriptions of the aqueduct and compare them using the novel perspective of neuroendoscopy. METHODS: We reviewed video record…

Cavernous Angioma of the Cerebral Aqueduct.

BACKGROUND: Among the rare intraventricular cavernomas, purely intra-aqueductal cavernomas are exceptionally rare. CASE DESCRIPTION: A 62-year-old patient presented with progressive headache, memory loss, gait instability, and urinary incontinence. Magnetic resonance imaging showed the presence of a mass lesion located in the lumen of the cerebral aqueduct, associated with triventricular hydrocephalus. CONCLUSIONS: We discuss the rationale that led us to treat hydrocephalus with neuroendoscopy, which offered the possibility to directly inspect the intra-aqueductal lesion, make the diagnosis …

Split cerebral aqueduct: a neuroendoscopic illustration.

PURPOSE: Forking of the cerebral aqueduct is a developmental malformation that is infrequently encountered by neurosurgeons as a rare cause of hydrocephalus, sometimes with a delayed onset. The etiology of an apparently forked aqueduct might be different. However, neuroendoscopy can often be the optimal treatment. The purpose of this study was to review the literature by analyzing the anatomical, functional, diagnostic, and therapeutic features of this unusual condition and adding our personal cases. METHODS: We present a case of forking of the cerebral aqueduct that was detected in vivo and …

Cerebral aqueduct stenosis presenting with limb pain.

Two children are reported with hydrocephalus and aqueduct stenosis who presented with back and limb pains. Neither had the classic symptoms of headache and vomiting. The children had enlarged heads and later developed ataxic gait and papilloedema. The cause of the pains is uncertain but similar symptoms have been reported in subjects with benign intracranial hypertension and may relate to spinal nerve root pouch distension. Operative ventricular drainage resulted in rapid improvement of all symptoms in both children.

Magnetic resonance imaging of the cerebral aqueduct.

1.5 Tesla MRI examinations were evaluated for aqueductal configuration, hydrocephalus and flow-related signal void in 70 patients with juxtaaqueductal pathology and in 20 normal controls. In the 70 cases with pathology, the aqueduct was obliterated or distorted in 34, dilated in 3, normal in 29 and not evaluable in 4. A definite flow-related signal void indicated CSF movement within the aqueduct in all normal examinations. Flow-related signal void was absent in some, but not all, patients with aqueductal obliteration and distortion. CSF turbulence can create an intra-aqueductal signal void in

The Cerebral Aqueduct Compliance: A Simple Morphometric Model.

BACKGROUND AND OBJECTIVES: This work aimed to identify different configurations of the adytum of the cerebral aqueduct suggesting its safe neuroendoscopic navigation. This concept is intimately connected to the physiological aqueductal dilatability or compliance, which is relatively ignored in the literature. A better knowledge of the extent of physiological aqueductal dilatability might better define the ideal diameter and safer features of dedicated flexible endoscopes. METHODS: The study includes 45 patients operated on using a flexible scope with a 3.9-mm diameter, where the structural el…

Cerebral cysticercosis with aqueductal obstruction. Case report.

The authors report a case of cerebral cysticercosis which presented with generalized nonspecific neurological signs and symptoms attributed to acute aqueductal obstruction, with concomitant intracranial hypertension. These were characteristic intracranial calcifications along with angiographically demonstrated signs of hydrocephalus. Contrast encephalography clearly demonstrated aqueductal obstruction. Pathologically, the aqueductal obstruction was shown to be due to parasitic invasion of the brain stem with compression of the aqueduct. The presence of typical intracranial calcification in con

Hydrocephalus with cerebral aqueductal dysgenesis and craniofacial anomalies.

A pre-term infant was born with multiple limb and craniofacial anomalies, including craniosynostosis and a nasopharyngeal adenohypophysis. Severe hydrocephalus of the lateral and third ventricles was attributed to cerebral aqueductal dysgenesis. Scattered aqueductules and ependymal rosettes were found throughout the midbrain and pontine tegmentum. An hypothesis to explain the unique constellation of craniocerebral findings is discussed.

Two-Dimensional Ultrasound Evaluation of the Fetal Cerebral Aqueduct: Improving the Antenatal Diagnosis and Counseling of Aqueductal Stenosis.

OBJECTIVE: To describe a technique for the visualization and measurement of cerebral aqueduct diameter through a 2D sagittal median plane, and to report its aspect and measurement in fetuses with aqueductal stenosis (AS). METHODS: This was a cross-sectional study of 207 morphologically normal fetuses in low-risk pregnancies between 20 and 36 weeks of gestation. The cerebral aqueduct was visualized transvaginally in a midsagittal plane, and measurements of its greatest diameter (ampulla) were taken independently by an expert and a nonexpert sonographer. In addition, the aqueduct morphology fro…

An In-Vitro Experimental Investigation of Oscillatory Flow in the Cerebral Aqueduct.

BACKGROUND: The cerebrospinal fluid filling the ventricles of the brain moves with a cyclic velocity driven by the transmantle pressure, or … here, for the first time, in vitro experiments involving an MRI-informed experimental model of the cerebral aqueduct flow. METHODS: Dimensional analysis is used in designing a scaled-up model of the aqueduct flow, with physical similarity maintained by adjusting the flow frequency and the properties of the working fluid. High-resolution MRI images are used to gen …

Cerebral Ventriculomegaly With Abnormality of the Aqueduct of Sylvius: A Case Series.

Fetal cerebral ventriculomegalies are brain anomalies with potentially poor neurodevelopmental prognosis. They are amenable to prenatal imaging, isolated or associated with other cerebral and/or extra-cerebral abnormalities. Most are obstructive due to a focal or diffuse dysgenesis of the ventricular system, disturbing the cerebro-spinal fluid outflow. Triventricular dilatation is the most common, predicting a lesion at the level of the aqueduct of Sylvius (AS). Although variants in several genes have recently been linked to ventriculomegaly, their role remains unclear, compromising our abilit

Spontaneous resolution of obstructive hydrocephalus from blood in the cerebral aqueduct.

Obstructive hydrocephalus is a neurological emergency that needs to be immediately identified and treated. It very rarely resolves without treatment. We report about an 86-year-old man with right frontal stroke who developed obstructive hydrocephalus caused by blood in the cerebral aqueduct. The patient had sudden and immediate clinical improvement and a repeated head computed tomography (CT) scan showing spontaneous resolution of hydrocephalus. Spontaneous resolution of obstructive hydrocephalus is possible when the cause is minimal blood in the cerebral aqueduct without any blood in the four…

Cyclophosphamide-induced agenesis of cerebral aqueduct resulting in hydrocephalus in mice.

Источник пометил публикацию как отозванную

The present work was undertaken to reveal the mechanism of cerebral aqueduct agenesis found to result in hydrocephalus following intrauterine exposure to model teratogen, cyclophosphamide, in murine fetuses. A single dose of 10-mg/kg body weight cyclophosphamide was injected intaperitoneally to pregnant mice on day 10, 11 or 12 of gestation. Fetuses were collected through abdominal incision on day 18 and studied for various malformations of brain and cranium including hydrocephalus. Incomplete development and failure of canalization of the cerebral aqueduct were detected when serial sections o…
