Coccidioidomycosis.

Coccidioidomycosis is a systemic disease caused by Coccidioides immitis and C posadasii spp, which are predominant in arid zones of the American continent, mainly in the Southwestern United States and the northern states of Mexico, as well as other regions with different environmental conditions. Some countries of Central and South America are also endemic zones. Most infected patients are asymptomatic. Disseminated disease develops in less than 5% of clinically affected individuals. Culture, biopsy, and DNA probes are used for fungus identification. Prognosis is related to low antibody detect…

Coccidioidomycosis.

The difficulties in managing this potentially horrific disease, with its myriad manifestations, are immense, because host factors dramatically impact outcome. Coccidioidomycosis should warrant great respect among clinicians, because, even with dramatic improvements in therapies, outcomes remain poor. Although there have been outstanding successes with these new therapies, tragic losses after years of immense patient suffering still occur. Coccidioidomycosis is a geographically restricted fungus but is one that inflicts tremendous suffering on affected patients. In addition, because of travel a…

Coccidioidomycosis.

Coccidioidomycosis is a fungal infection caused by Coccidiodes. The fungus has a limited geographic distribution but the disease can be seen anywhere. Primary infection is usually self-limited; however, when a large dose of fungus is inhaled or in an immunosuppressed host, the patient may develop overwhelming infection, including ARDS, in a brief period. While serodiagnosis can help to suspect the diagnosis, particularly in the setting of overwhelming disease, diagnostic efforts should focus on identifying the organism in body fluids or tissues. Several treatment options are available. Amphote…

Coccidioidomycosis.

Coccidioidomycosis is usually acquired by inhalation of Coccidioides immitis in certain areas of the Western Hemisphere. However, the disease may occur far away in individuals who have visited or lived in, then departed from, the endemic areas. The disease which can affect normal and immunocompromised individuals, has many manifestations resembling those of many diseases. The diagnosis is usually not difficult and can be accomplished by histopathological, cultural, and serological methods. Therapy can be surgical and/or medical. The latter can make use of parenteral amphotericin B and its lipi…

Coccidioidomycosis.

Coccidioidomycosis is a systemic fungal infection endemic to the southwestern United States and other parts of the western hemisphere. Although producing a wide range of disorders in healthy persons, immunosuppression predisposes to especially severe disease. Thus, a knowledge of the pathogenesis of coccidioidal infections and its relation to the normal immune responses is useful to understand the diversity of problems that Coccidioides immitis may cause. Diagnosis usually requires laboratory studies such as fungal culture or specific serologic testing. Fortunately, many patients do not need t…

Coccidioidomycosis.

Although rapid population growth in the Southwestern United States and travel to and through the area are increasing the potential for exposure to Coccidioides immitis, prevalence rates have declined in some endemic areas, probably because of environmental factors. With the iatrogenic immunosuppression of organ transplantation and the immunosuppression inherent in AIDS, more opportunistic infections with this organism are to be expected. The variety of cutaneous manifestations continues to challenge the dermatologist's acumen. Spherule-derived coccidioidin is an improved epidemiologic tool, an
