Perianal Crohn disease.

Crohn disease is a chronic inflammatory disease characterized by sharply demarcated segments of gastrointestinal involvement from mouth to anus. Its perineal manifestations are among the most devastating and mutilating complications. They occur at any time and may precede the intestinal manifestations. Their most common presentations are perineal ulceration, fistula, and abscess. Proliferative and polypoid morphology of the cutaneous lesions mimicking warts and condyloma are rarely described. We report a 25-year-old woman with a 4-month history of confluent plaques of the perineal region with

[Familial Crohn disease].

Authors report two cases of Crohn's disease in a brother and a sister. Female showed first manifestation at 8 years of age. Male started at 12 years. Onset was acute in the girl with ileo-cecal and colonic involvement and extraintestinal manifestations. The boy had an insidious onset with ileal involvement and striking rectal and perianal symptoms. In both cases, clinical course was chronic and relapsing. HLA A and B phenotypes were studied in both patients and their parents. Different factors concerning origin of disease, as well as relationship to other diseases among relatives are discussed

Nutrition in Crohn disease.

Nutrition plays an important role in the pathogenesis, treatment, and morbidity of Crohn disease. Approximately two thirds to three fourths of hospitalized patients with active disease and one fourth of outpatients with Crohn disease are malnourished. Malnutrition, which can be present even when Crohn disease is in remission, can affect growth, cellular and humoral immunity, bone density, and wound healing. Decreased nutrient intake, malabsorption, drug-nutrient interactions, anorexia, and protein-losing enteropathy can all contribute to the protein-calorie malnutrition and other specific nutr

[Treatment of Crohn disease].

Crohn's disease is a chronic inflammatory disease of the bowel for which there is no curative treatment. The purpose of treatment is to reduce mortality to the absolute minimum and to give the patients a normal quality of life. Acute episodes of low severity are treated with sulfasalazine and its most recent derivatives such as 5-aminosalicylate. More severe episodes require oral corticosteroid therapy which, when prescribed in adequate doses, results in clinical remission in over 90% of the cases. To this must be added parenteral nutrition in patients with very severe symptoms or when a major

[Echotomography in Crohn disease].

The authors report their experience in 40 patients with intestinal Crohn's disease, who underwent both abdominal ultrasound scanning and double-contrast small bowel enema. The various echographic patterns are described, found in the different phases during the evolution of Crohn's disease, underlying the importance of ultrasound in evaluating the real degree of bowel wall thickening and the exo-enteral space, only indirectly appreciated with X-ray examination.

[Appendectomy and Crohn disease].

The Authors, on the basis of the casuistry of the Verona Surgical Centre, concerning the interventions for Crohn's disease from 1973 up to 1983, and comparing the data of literature, study the problem of appendicectomy in Crohn's disease. They suggest the operation of appendicectomy both when there are signs of appendicopathy in course, joined to a pathology of the last ileum ansa, and in presence of apparently sound appendix and clear signs of ileum Crohn's disease. They think it opportune to avoid appendicectomy only in presence of clear signs of involvement of caecum by the morbid process.

[Crohn disease--surgical standards].

Surgery for Crohn's disease is restricted to the treatment of complications. Even then, nearly all patients with Crohn's disease must be operated on at least once in lifetime. Surgical concepts base on the right timing for the operation, interventional drainage of abscesses, accurate pre-operative work-up to determine the extent of inflammation, and bowel conserving operation techniques. Respecting these principles, surgery for Crohn's disease can be performed with low complication rates restoring in most cases patients' quality of life.

[Isolated jejunal Crohn disease].

Crohn Disease (CD) is a chronic granulomatous inflammatory disorder of not well known etiology. It may affect the whole digestive tract, segmentally, from the mouth to the anus although the rectum is usually not involved. Digestive symptoms of CD depend on the location and extension of the disease, its fistulating or stenosing evolution as well as the presence of local or systemic complications. 40% of the patients affected with CD have simultaneous terminal ileal and colonic involvement, 30% have terminal ileitis and 25% have granulomatous colitis. Isolated esophageal, gastroduodenal and peri

[Enterostomy in Crohn disease].

Indications, technique and prognosis of 129 enterostomies in Crohn's disease are reported. Advanced perianal or entero-genital fistulas represented the predominant indication (60% of primary, 46% of repeated stomas). 85% of stomas were created in combination with intestinal resections. Preferred type of stoma was a loopileostomy (76% of primary stomas), whereas colostomies were avoided whenever possible because of increased complications. Patients initially presenting with rectal involvement or perianal fistulas were prone to need a stoma during the course of their disease while intraabdominal

Renal Amyloidosis and Crohn Disease.

Background: Secondary amyloidosis, a rare complication of Crohn disease (CD), is triggered by persistent systemic inflammation. Kidney involvement is the most frequent manifestation and is often characterized by nephrotic syndrome and kidney failure. This complication usually appears in patients with long-standing disease and is associated with increased morbidity and mortality risk. Diagnosis is by microscopic amyloid observation of tissue biopsy, and when the diagnosis is confirmed, the therapeutic objective is disease activity control. Response assessment is challenging because of a lack of

Cutaneous Manifestations of Crohn Disease.

Awareness of the extraintestinal manifestations of Crohn disease is increasing in dermatology and gastroenterology, with enhanced identification of entities that range from granulomatous diseases recapitulating the underlying inflammatory bowel disease to reactive conditions and associated dermatoses. In this review, the underlying etiopathology of Crohn disease is discussed, and how this mirrors certain skin manifestations that present in a subset of patients is explored. The array of extraintestinal manifestations that do not share a similar pathology, but which are often seen in association

Gingival involvement in Crohn disease.

BACKGROUND: Although the oral manifestations of Crohn disease are well-established, there is little specific documentation of the gingival involvement. CASE DESCRIPTION: The authors describe four patients with significant gingival involvement and identify clinical signs and symptoms of the disease involving the gingivae, along with other oral manifestations. Patients had persistent gingival lesions manifesting as pustular ulcerations, erythema, swelling and cobblestoning. The authors also discuss the differential diagnosis, treatment options and prognostic factors. CLINICAL IMPLICATIONS: Pat

[Crohn disease and laparoscopic surgery].

AIM OF THE STUDY: To evaluate possibilities of laparoscopic surgery in the treatment of Crohn's disease. METHODS: A retrospective evaluation of the group of 14 patients, operated on in the period of 2/1997 to 2/2003. RESULTS: In the group of 7 women and 7 men, laparoscopy was used for ileocecal resection in six cases (43%), resection of ileotransversoanastomosis in four cases (29%) and abdominal-peritoneal amputation of rectum once (7%). The surgical intervention lasted 125 min on the average. The post-operation complications were not recorded and the conversion was not necessary. The post-o

[Ultrasound diagnosis of Crohn disease].

In inflammatory bowel diseases sonography can depict the transformations in the bowel wall structure as well as extramural changes. Certain patterns of altered stratification can be defined which can be helpful in determining disease activity. Crohn's disease is extending very often into the mesenteries and to the serosa, omentum and peritoneum on the other side. Sonography will detect fistulas, abscesses, omental cake and ascites for example. Extraintestinal manifestations like those of the biliary tree, the pancreas, the kidneys can be seen. Color doppler sonography will be able to define di

[Crohn disease of the esophagus].

INTRODUCTION: Since the landmark report by Crohn, Ginzburg and Oppenheimer in 1932, the spectrum of involvement of Crohn's disease has expanded to include the entire digestive tract from mouth to anus. The involvement of the esophagus by Crohn's disease is very rare. METHODS: We report a 41-year-old male patient with severe Crohn's disease of the distal esophagus with a fistula to the right bronchial system. RESULTS: After a long period of nutritional support we performed a transthoracic esophagectomy and a Nissen fundoplication. Postoperatively his course was complicated by pneumonia and wi
