[Cryoglobulinemia].

Cryoglobulins are proteins that precipitate when cooled and dissolve when heated. Cryoglobulinemia may be classified into Type I (monoclonal), Type II (mixed), and Type III (polyclonal). About 5 to 10% of myeloma proteins and 7 to 20% of primary macroglobulinemia proteins are cryoprecipitable (Type I cryoglobulin), and more than half of these have no symptoms attributable to cryoglobulin nature of their M-component. The most common symptoms of Type I cryoglobulinemia include Raynaud's phenomenon, necrosis of the toes or legs, vascular purpura, and urticaria. In Type I cryoglobulinemia due to p…

[Cryoglobulinemia].

Cryoglobulinaemia may be discovered by systematic blood examination or by the finding of a purpura reflecting a leukocytoclastic angiitis or vascular manifestations. An immunochemical study is necessary to determine whether the cryoglobulin thus found is purely monoclonal or mixed, with a monoclonal component (usually IgM) and a polyclonal IgG component, or is composed of polyclonal immunoglobulins. The principal complications of cryoglobulinaemia involve the kidneys and the nervous system. Monoclonal cryoglobulins may reveal Kahler's disease or Waldenström's macroglobulinaemia. Mixed cryoglob

Cryoglobulinemia.

Cryoglobulins are immunoglobulins that precipitate in vitro at temperatures below 37 ̊C. Cryoglobulin-associated disease is heterogeneous, a … systemic autoimmune diseases, malignant neoplasms and cases with no identified aetiology (essential cryoglobulinemia). Treatment should be modulated according to the predominant etiopathogenesis (vasculitis or hyperviscosity), the severity of internal organ involvement and, especially, the associated underlying disease. Due to the complex aetiological, clinical an …

[Cryoglobulinemia].

Cryoglobulinaemias contain seric immunoglobulins which precipitate at low temperatures in some tissues. Based on immunohistochemical analysis of their components (monoclonal or polyclonal), 3 types of cryoglobulinaemias have been identified, and a classification has been proposed. The presence of a paraprotein within the cryoglobulinaemia is due to the proliferation of B cells. Type I is mainly associated with a lymphoproliferative syndrome (myeloma and others...) whereas type II (mixed cryoglobulinaemia) and type III are the result of viral and bacterial infectious diseases, and of autoimmune

[Mixed cryoglobulinemia].

The authors describe two clinical cases of cryoglobulinemia. A 70 years old woman, having skin ulcers on lower limbs, arthralgias, paresthesias and constitutional symptoms, for about 10 months. Exams revealed mild anemia, elevation of the biological parameters of inflammation and aminotransferases, positive cryoglobulin and rheumatoid factor in serum, and a severe reduction in C4 complement fraction. Hepatitis C virus (HCV) serology was negative. Idiopathic mixed cryoglobulinemia was diagnosed and corticosteroid therapy started. Given the lack of response, cyclophosphamide and plasmapheresis w…

Cryoglobulinemia Vasculitis.

Cryoglobulinemic vasculitis (CryoVas) is a small-vessel vasculitis involving mainly the skin, the joints, the peripheral nervous system, and the kidneys. Type I CryoVas is single monoclonal immunoglobulins related to an underlying B-cell lymphoproliferative disorder. Type II and III cryoglobulins, often referred to as mixed cryoglobulinemia, consist of polyclonal immunoglobulin (Ig)G with or without monoclonal IgM with rheumatoid factor activity. Hepatitis C virus (HCV) infection represents the main cause of mixed CryoVas. The 10-year survival rates are 63%, 65%, and 87% in HCV-positive mixed …

Mixed cryoglobulinemia.

Mixed cryoglobulinemia (MC), type II and type III, refers to the presence of circulating cryoprecipitable immune complexes in the serum and manifests clinically by a classical triad of purpura, weakness and arthralgias. It is considered to be a rare disorder, but its true prevalence remains unknown. The disease is more common in Southern Europe than in Northern Europe or Northern America. The prevalence of 'essential' MC is reported as approximately 1:100,000 (with a female-to-male ratio 3:1), but this term is now used to refer to a minority of MC patients only. MC is characterized by variable…

[Mixed cryoglobulinemia].

Cryoglobulins are immunoglobulins that persist in serum, precipitate with cold temperature, and resolve when rewarmed. Cryoglobulinemia are part of systemic vasculitis, and they are in the subgroup of immune complex type vasculitis. Mixed cryoglobulinemia are associated with malignant B lymphoproliferative disorders, auto-immune and chronic infectious diseases, particularly hepatitis C virus infection. Mixed cryoglobulinaemia has a variable prognosis, depending mainly on renal involvement, systemic extension, and severity of hypertension. In pilot studies, alpha interferon seemed promising to …

Cryoglobulinemia in a child.

Cryoglobulinemia is a rare condition. We report a case of cryoglobulinemia in 1.5-year-old boy, who presented with bluish black discoloration of hands and feet. Serum was positive for cryoglobulins. The serum cryoglobulin level was 70 μg/ml (normal <20 μg). Serum protein electrophoresis revealed the presence of M-spike. Serum immunofixation and assessment of clonality could not be done as these specialized investigations are not available here. Investigations did not reveal any systemic involvement. He was treated with anticoagulants, broad-spectrum antibiotics, and systemic steroids with whic…

[Type I cryoglobulinemia].

HISTORY AND CLINICAL FINDINGS: For 2 years a 52-year-old man had repeated bouts of purpura, arthralgia and fever. He was known to have abnormal monoclonal gammaglobulins, type IgG-lambda and vasculitis when he had another bout with acute renal failure and necrotizing ulcers in the legs. TESTS: Several laboratory tests were abnormal: erythrocyte sedimentation rate (122 mm), haemoglobin level (9.1 g/dl), white cell count (32,000/microliters), platelet count (562,000/microliters), creatinine level (4.1 mg/dl) and liver enzyme activities. He also had proteinuria (4.5 g daily) and nephritic urinar

Proteomic Profiling of Cryoglobulinemia.

OBJECTIVE: We aimed to explore and identify candidate protein biomarkers of cryoglobulinemia (CGE) in disease control patients with negative cryoglobulin (DC) or healthy controls (HCs). METHODS: The tandem mass tag (TMT)-labeled serum quantitative proteomics approach was used to identify differentially expressed proteins between the CGE and DC groups. Ingenuity pathway analysis was used for functional annotation of differentially expressed proteins. Biomarker candidates were validated in another cohort using the parallel reaction monitoring (PRM) method. Apolipoprotein A1 (APOA1), apolipoprot…

[Cryoglobulinemia in Sjögren's syndrome].

As many as 130 patients with Sjögren's disease (SD) were examined for blood cryoglobulins during 1977-1982. Cryoglobulinemia was discovered in 25 (19.2%) patients with SD. The clinical manifestations such as severe xerostomia, appreciable increase of the parotid salivary glands, hepatosplenomegaly, purpura, polyneuropathy, lesions of the lungs and kidneys were mostly detectable in SD patients with cryoglobulinemia. Ten cryoprecipitates of SD patients with cryoglobulinemia showed the monoclonal immunoglobulins IgMk-9 and IgA-1. All the patients had high titers of antinuclear antibodies and 90% …

[Articular manifestations of cryoglobulinemias].

Manifestations are found in the joints in 27-50 percent of patients with cryoglobulinaemia. They include arthralgia, or less frequently arthritis of distal oligo-articular topography, predominantly in the lower limbs. These manifestations consist of pure synovitis - nonexudative, nondestructive, and usually recurrent. Cold is rarely a precipitating factor. The manifestations are indicative of cryoglobulinaemia when they are associated with other symptoms of the disorder, among which Raynaud's disease and purpura of the lower limbs are the most frequent. Cryoglobulin is demonstrated only if the

Mixed cryoglobulinemia: new concepts.

The most documented extrahepatic manifestation of hepatitis C virus (HCV) infection is mixed cryoglobulinemia (MC). MC is characterised by the presence of temperature-sensitive protein complexes: in type II MC, cryoglobulins are composed of a monoclonal rheumatoid factor (usually, IgMkappa) against polyclonal IgG. In type III MC, all components are polyclonal. The presence of microheterogeneity and other new types of cryoglobulins is a novel and recent observation. The production of different autoantibodies and circulating immune complexes, including the cryoglobulins, are responsible for syst…

Cryoglobulinemia Complicated by Pneumonia.

This study describes a 64-year-old female with a history of hepatitis C and cryoglobulinemia, who presented with respiratory symptoms, including dry cough, shortness of breath, and fever, alongside joint pain and fatigue. Initial workup revealed interstitial pneumonia, supported by chest imaging, and the patient was treated for pneumonia with standard antibiotic therapy. Despite no renal involvement, a hallmark of cryoglobulinemia, further testing confirmed elevated serum cryoglobulin levels. The patient was diagnosed with cryoglobulinemia complicated by pneumonia, a rare but significant manif…

Cryoglobulinemia: An update in 2019.

Cryoglobulinemia is defined as the persistent presence in serum of abnormal immunoglobulins (Igs) that precipitate at low temperatures and dissolve again upon warming. Cryoglobulins may be composed only of a monoclonal Ig (simple type I cryoglobulinemia), of a monoclonal Ig bound to the constant domain of polyclonal Ig heavy chains (mixed type II cryoglobulinemia), or only of polyclonal Igs (mixed type III cryoglobulinemia). The manifestations of type I cryoglobulinemia are often related to intravascular obstruction, whereas those seen in the mixed cryoglobulinemias often originate in true imm…
