[Early cytomegalovirus retinitis].

We report the clinical course of cytomegalovirus (CMV) retinitis associated with acquired immunodeficiency syndrome (AIDS) from the initial onset. The patient was a 40-year-old human immunodeficiency virus antibody-positive male with hemophilia A. He was diagnosed as having AIDS on the basis of pneumocystis carinii pneumonia. Ophthalmoscopic examination disclosed a small white punctate lesion at the macular area in his right eye. Because the lesion enlarged gradually with hemorrhages, it was suspected to be CMV retinitis. However, further examination was impossible due to his severe general co

Cytomegalovirus Retinitis in infancy.

PURPOSE: To describe the presentation of cytomegalovirus retinitis (CMVR) in a series of infants. METHODS: Immunocompromised infants with either HIV or systemic cytomegalovirus (CMV) were examined for CMVR. Ocular involvement was recorded and monitored by digital imaging. RESULTS: Five infants were detected to have CMVR. All the infants demonstrated changes within the macula. One infant progressed from a fine granular pattern to fulminant CMVR. CONCLUSION: Infants under a year with CMVR have a predilection for the disease to present at the macula, in contrast to the presentation in adults, …

Classification Criteria for Cytomegalovirus Retinitis.

PURPOSE: The purpose of this study was to determine classification criteria for cytomegalovirus (CMV) retinitis. DESIGN: Machine learning of cases with CMV retinitis and 4 other infectious posterior/ panuveitides. METHODS: Cases of infectious posterior/panuveitides were collected in an informatics-designed preliminary database, and a final database was constructed of cases achieving supermajority agreement on diagnosis using formal consensus techniques. Cases were split into a training set and a validation set. Machine learning using multinomial logistic regression was used in the training s

[Cytomegalovirus retinitis in immunocompetent patients].

BACKGROUND: Cytomegalovirus (CMV) retinitis usually affects severely immunosuppressed individuals. We report two immunocompetent patients who developed CMV retinitis. CASES: Case 1 was a 65-year-old man who was referred to us with blurred vision and floaters of 2 weeks duration in his left eye. Slit-lamp biomicroscopy showed keratic precipitates, aqueous cells, and vitreous opacity in his left eye. Funduscopic examination revealed yellow-white retinal lesions with arterial sheathing in the superotemporal midperiphery. Case 2 was a 63-year-old man who presented with a 2-week history of blurred

[Histopathological findings in cytomegalovirus retinitis].

PURPOSE: We examined eyeballs collected from autopsied acquired immunodeficiency syndrome patients with cytomegalovirus (CMV) retinitis, and analyzed the precise pathogenesis of CMV retinitis. MATERIAL AND METHODS: Eyeballs were fixed with 10% buffered formalin embedded in paraffin. CMV antigens were investigated by histopathological and immunohistochemical analyses. Histopathological findings were compared with funduscopic images. RESULTS: CMV antigens remained in the necrotic area of the retina and many CMV immediate early antigens existed in intact parts of the inner retina showing almost

The diagnosis of cytomegalovirus retinitis.

Immunosuppressed patients are at risk for developing cytomegalovirus retinitis. This disorder is the most common cause of vision loss in patients with the acquired immunodeficiency syndrome (AIDS). Cytomegalovirus retinitis is probably the result of hematogenous spread of the virus to the retina after systemic reactivation of a latent cytomegalovirus infection. Although the ophthalmic infection may initially be asymptomatic, the retinal necrosis it produces may result in both loss of visual field and decreased visual acuity. Routine screening of these patients is required for early diagnosis. …

[Vitreous involvement in cytomegalovirus retinitis].

In our outpatient department we often see patients with vitreous inflammation. Recent reports emphasize that vitreous inflammation is very rare, so we looked for patients with cytomegalovirus (CMV) retinitis and vitreous inflammation. We investigated 359 HIV-positive patients for at least 1 year; all patients reached the stage of AIDS disease. We included patients without any retinal signs of CMV retinitis at the beginning or CMV recurrence during the study. A slit-lamp investigation, binocular funduscopy and b-picture ultrasound were done at regular intervals. The results show an increase in

Clinical aspects of cytomegalovirus retinitis.

Cytomegalovirus (CMV) retinitis is a sight-threatening opportunistic infection of the retina that occurs in immunosuppressed individuals. The diagnosis is based primarily on the clinical ophthalmic examination, with laboratory confirmation of systemic CMV infection. The retinitis has several characteristic appearances and a characteristic course of clinical evolution. The response to therapy with 9-[(1,3-dihydroxy-2-propoxy)methyl]guanine (DHPG, ganciclovir) has represented a significant improvement in the management of this disease. An understanding of the clinical presentation, clinical cour
