Denial mechanisms in myocardial infarction: their relations with psychological variables and short-term outcome.

From a selected sample of 97 males suffering from a first myocardial infarction, 67 patients were studied to ascertain the influence of denial mechanisms (DM) on their cardiological and psychological outcome. There were no differences among high deniers and low deniers with respect to the cardiological outcome, but high deniers showed less anxiety and depressive reactions both in the coronary unit and 1 month later, and also presented less psychopathology in general. In the last evaluation, one year after leaving the hospital (N = 52), there was no difference among deniers and non deniers in d

[The Life Style Index: correlations with psychological distress and hostility].

The Life Style Index (LSI) was designed to assess defense mechanisms, assuming that their use is related to specific emotional states and diagnostic concepts. Aiming to further investigate the psychometric properties of the Greek version of the LSI, the aim of the present study was to investigate the relationship of specific defense mechanisms with dimensions of psychological distress and hostility features in three different populations. The sample comprised 1261 adults (410 healthy participants, 723 medical patients and 128 psychiatric patients). Along with defense mechanisms (LSI), Psycholo

Denial in physical illness.

Denial is a concept often encountered in literature describing patients' psychological responses to physical illness. Definitions and theories of denial have drawn on clinical, cognitive, psychodynamic, organic, and interpersonal frameworks. Denial is related to other concepts such as lack of insight, self-deception, and anosognosia. Empirical studies have yielded mixed results with regard to the adaptive properties of denial. These results require interpretation in the light of: (1) the definitional complexities of denial; (2) the diversity of methods used for its assessment; and (3) the choi

Pitfalls in anorexia nervosa research: The risk of artifacts linked to denial of illness and methods of preventing them.

One of the most serious problems faced by researchers studying eating disorders is denial of illness in individuals with anorexia nervosa. Importantly, the term "denial" not only has different meanings, but in the case of anorexia nervosa its very nature still remains obscure. It is not even known whether it is deliberate or unintentional. Denial of illness in anorexic patients has serious consequences for evaluation of the reliability of information obtained from those individuals. Indeed, the most frequently used screening questionnaires, such as the Eating Attitudes Test (EAT) (Garner & Gar

Anxiety and depression in chronic hemodialysis: some somatopsychic determinants.

AIMS: Depression and anxiety are so common in hemodialysis (HD) patients that we found it useful to study the respective contributions of the subjective somatic sensations and of the objective medical comorbidity to psychological distress. We also hypothesized that denial has a protective effect against anxiety and depression, and that alexithymia is, on the contrary, a risk factor. MATERIAL AND METHODS: In a cross-sectional design, we investigated relationships between psychological distress and somatic complaints, Charlson comorbidity index, denial and alexithymia, in a group of 54 patients

Extensive basal cell carcinoma of the face: an extreme case of denial.

Patients with head and neck cancer are particularly susceptible to using denial as a coping mechanism. While some forms of denial may help patients achieve better levels of physical functioning, persistent denial can serve as a major barrier to treatment. We report a case of extreme denial by a 60-year-old woman with an extensive basal cell carcinoma of the face that had been neglected for more than 20 years. We present this case to raise awareness of the potential danger of denial, and we discuss strategies that physicians can undertake to properly manage patients who engage in it. Since the

Denial of pregnancy: a literature review and discussion of ethical and legal issues.

Denial of pregnancy is an important condition that is more common than expected, with an incidence at 20 weeks gestation of approximately 1 in 475. The proportion of cases persisting until delivery is about 1 in 2500, a rate similar to that of eclampsia. Denial of pregnancy poses adverse consequences including psychological distress, unassisted delivery and neonaticide. It is difficult to predict which women will develop denial of pregnancy. There are a number of forms of denial of pregnancy, including psychotic and non-psychotic variants. Denial of pregnancy is a 'red flag' that should trigge

Anosognosia during Wada testing.

Anosognosia, the verbally explicit denial of hemiplegia, is more often reported after right- than left-hemisphere lesions. However, this asymmetric incidence of anosognosia may be artifactual and related to the aphasia that often accompanies left-hemisphere lesions. Anosognosia has been attributed to psychological denial and the emotional changes associated with hemispheric dysfunction. Eight consecutive patients undergoing intracarotid barbiturate (methohexital) injections as part of their presurgical evaluations for intractable epilepsy were assessed for anosognosia after their hemiplegia an

Psychological Assessment in Anorexia Nervosa: A Single Case Study on Predictive Factors of Dropout.

Anorexia nervosa (AN) has the highest mortality rate among psychiatric disorders. Adult AN patients have a chronic history of treatment dropout due to denial of their psychological and physical disease states, which may be connected to defense mechanisms. We developed an assessment protocol to evaluate the psychological functioning of patients undergoing a psychodynamic approach for eating disorders (PAED), aimed at identifying the psychological factors associated with intervention success or dropout. We analyzed the case of an adult patient who quit treatment at the start and discussed her ps

Exploring illness denial of LVAD patients in cardiac rehabilitation and their caregivers: A preliminary study.

Implantable left ventricular assist devices (LVADs) are increasingly used for patients with heart failure. Despite this evidence, the psychological processes affecting implanted patients and their caregivers are only marginally studied. While common emotional reactions, such as anxiety and depression already receive attention, very little is known about the course of caregiver strain, illness denial, and their reciprocal relation. This preliminary study aims at exploring these critical topics in a cardiac rehabilitation setting, besides offering indications for future research. A total of 44 L

Maladaptive Denial of Severe Pain and Acute Orthopedic Injuries in a Patient With a Schizoaffective Disorder.

Persistent denial of severe and acute pain following orthopedic injuries has not been previously reported. We present a case of a 24-year-old woman with a history of schizoaffective disorder who suffered severe pain secondary to acute orthopedic injuries who insisted, "I am fine! There is nothing wrong with me." Her maladaptive denial resulted in an initial refusal of necessary medical/surgical care, but she eventually accepted the necessary treatments despite her persistent belief she did not need such care. Her verbalizations and behaviors were characterized by active avoidance and angry rea

Anorexia Nervosa and First-Person Perspective: Altruism, Family System, and Body Experience.

Based on the case study of Jeanne, the objective of this article is to study patterns of specific subjectivity in anorexic subjects. We seek to identify, in a first-person perspective, the core vulnerability features of anorexic existence, beyond the dimension of food alone. The identification of a psychopathological structure results in a better understanding of Jeanne's clinical situation and helps formulate psychotherapeutic and prophylactic recommendations. We suggest that so-called "denial" is a psychological mechanism that should be reconsidered. Denial is not a mechanism pertaining to a

Early psychological aspects of severe hand injury.

We investigated the incidence and nature of psychological symptoms occurring during the first two months after severe hand injuries. 94% of patients had significant symptoms at some point early in rehabilitation, including nightmares (92%), flashbacks (88%), affective lability (84%), preoccupation with phantom limb sensations (13%), concentration/attention problems (12%), cosmetic concerns (10%), fear of death (5%), and denial of amputation (3%). Two months later, flashbacks (63%) remained pronounced. Nightmares (13%), affective lability (48%), concentration/attention problems (5%), fear of de

[Several factors to distinguish anosognosia from denial after a brain injury].

This Article deals with the question of ignorance after a serious brain injury. The main purpose of this research is to present some factors that are critical for the differential diagnosis of anosognosia (organic etiology) and denial (psychological etiology) in order to improve the rehabilitation strategies of brain injuried population. In the present work we report the case study of MG (Havet-Thomassin, 2000). MG is a 32 Year old right handed man who has undergone a severe brain injury after a car accident. Neuropsychological approach of ignorance is mainly landed from questionnaire assessme

A comparison of psychological resilience and related factors in Chinese firstborn and only children.

The objective of this research is to compare psychological resilience in Chinese firstborn and only children and its relation to perceived parenting styles and child abuse. The results found that total psychological resilience score, and resilience subscales such as emotion control and goal focus mean scores of the firstborn child was significantly lower compared to the only child (Ps<0.05). The total psychological resilience scores of both male and female in firstborn children was significantly lower, when compared respectively to only children (Ps<0.01). Emotional warmth and understanding in

On the contribution of unconscious processes to implicit anosognosia.

Mograbi and Morris present a review of the literature on anosognosia of hemiplegia and dementia, including Alzheimer's disease. Their review focuses on aspects of implicit anosognosia. The authors' viewpoint is supported by the presentation of a general model on implicit anosognosia in Alzheimer's disease. The notions have important implications for clinical management, in particular, failure of treatment in Alzheimer's patients.

The interaction between implicit and explicit awareness in anosognosia: emergent awareness.

The dissociation between implicit and explicit forms of awareness has been described in various neurological diseases. The way in which these forms of awareness integrate in order to permit the necessary unitary experience of self remains unclear. Here, the hypothesis that a form of emergent awareness (i.e., the emergence of a verbal acknowledgment of deficits as a consequence of attempting to act) may represent a link between implicit and explicit components is proposed for discussion.

Denial, anosodiaphoria, and emotional reactivity in anosognosia.

A central flaw in models of anosognosia is to consider metacognitive awareness and affective regulation and responsiveness as separate functional modular subsystems. This line of reasoning leads to an "either or" conceptualization of the probable causes of implicit awareness in anosognosic patients. Neuroscience research and clinical observations of patients suggest that anosognosia is often associated with a change in the affective status of the individual as well as a change in their explicit verbal descriptions of themselves. Studying anosognosic patients over time and including measures of

More work on lack of awareness and insight in healthy people and psychiatric patients will assist model building.

We comment on the work of Mograbi and Morris and their newly developed Cognitive Awareness Model in terms of metacognition and awareness of disability in health aging, dementia, and psychosis. It is suggested that further research comparing implicit awareness of disability and objective metacognitive processes would be useful for theory development, as well as further understanding cognitive models of insight in the clinical domain.

Health professionals are unaware of anosognosia.

"Implicit awareness" may be inferred from compliance with medical treatment, even when the patient explicitly denies the need for treatment. Such compliance may cause medics and other health professionals to underestimate the frequency of anosognosia and its effects on the lives of patients and carers. We report survey data showing that health professionals do indeed consider anosognosia following stroke to be relatively uncommon and unimportant, in contrast with evidence on its true frequency and impact. Mograbi and Morris' emphasis on the distinction between implicit and explicit awareness m
