Single-embryo transfer.

The high incidence of multiple pregnancy is a major side effect of assisted reproductive technology that has recently given rise to justified criticism from different sides. The reasons for the high incidence of multiple pregnancies are discussed and a brief review is given of the steps that have been proposed to solve or at least reduce the problem. In this article, attention is particularly focused on the prevention of twin pregnancies because in terms of perinatal morbidity and associated costs, they outweigh the effect of high-order multiple pregnancies when expressed in absolute figures.

Outcomes and cost analysis of single-embryo transfer versus double-embryo transfer.

BACKGROUND: Studies had compared single-embryo transfer to double-embryo transfer with cleavage stage embryos and found that while single-embryo transfer was less costly, it was also associated with a lower live birth rate than double-embryo transfer. A single blastocyst transfer has been shown to improve the live birth rate per cycle compared to single-embryo transfer at cleavage stage. OBJECTIVES: To compare live birth rates and real costs of elective single-embryo transfer to double-embryo transfer and to determine the incremental cost-effectiveness ratio of these two strategies in an unse

Conjoined twins post single embryo transfer of a euploid embryo in a frozen embryo transfer cycle.

A patient of advanced maternal age with infertility underwent assisted reproductive technology treatment, followed by a single euploid embryo transfer in a hormonal replacement therapy frozen cycle. The implanted embryo resulted in a thoracophagus conjoined twin pregnancy, joined at the thorax, and was diagnosed at 11 weeks. The pregnancy was terminated. This is the first case of conjoined twins resulting from a euploid embryo transfer in a frozen cycle.

Biofluid aspects of embryo transfer.

Embryo transfer (ET) is the last stage of extracorporal fertilization during which the embryo is placed in the uterine cavity with a medium-filled catheter 2-3 days after in vitro fertilization. While fertilization in the laboratory occurs at very high rates (> 90%), the overall success of the procedure (i.e., take home baby) is still very low (< 25%) and assumed to be mainly due to implantation failure. A computational model was developed to simulate ET within the uterine cavity by a fluid-filled catheter inserted into a two-dimensional channel with oscillating walls. The results showed that

Clinical practice of embryo transfer.

In this review, several embryo transfer methods are considered, together with factors involved in achieving an effective transfer. The approach most used is transcervical intrauterine transfer. This is described in detail, together with the many variables influencing success, e.g. technical ability and training of personnel, catheter choice, value of a previous 'dummy transfer' and the need to minimize trauma during transfer and so prevent damage to the uterine lining, bleeding and uterine contractions. These factors can each negatively impact on pregnancy rates. Emphasis is put on quality, de

Ideal frozen embryo transfer regime.

PURPOSE OF REVIEW: This review aims to compare evidence on four criteria (embryo implantation, obstetric outcomes, patient convenience, and IVF-unit efficiency) by analyzing published research on different endometrial preparation methods for frozen embryo transfer (FET). RECENT FINDINGS: While the artificial-FET cycle provides advantages in scheduling and implantation, it falls short in ensuring optimal obstetric outcomes. In contrast, natural-FET ensures embryo implantation conditions if ovulation is correctly identified. Supplementing with exogenous progesterone shields against low corpus l

Sonographer-guided frozen embryo transfer vs ultrasound-assisted frozen embryo transfer - A randomised controlled study.

BACKGROUND: The use of ultrasound to guide placement of the embryo during in vitro fertilisation (IVF) is important, but there are times where a good image cannot be obtained. Having a trained sonographer perform the ultrasound may therefore improve the success of embryo transfer. AIM: To determine whether the routine use of a sonographer to guide embryo transfer is superior to standard care. MATERIALS AND METHODS: Randomised, controlled, prospective clinical study in a private infertility clinic. There were 113 women aged <38 years undergoing frozen embryo transfer (donor egg/embryo exclude

Comparison of Pregnancy and Neonatal Outcomes Between Fresh Embryo Transfer and Frozen-Thawed Embryo Transfer.

This study compared the pregnancy and neonatal outcomes between fresh embryo transfer and frozen-thawed embryo transfer (FET). These patients were split into two groups: the fresh embryo transfer group and the FET group. The general conditions, pregnancy outcomes, and neonatal outcomes between these groups were compared. The influencing factors of fetal macrosomia occurrence were explored as well. Compared with the fresh embryo transfer group, the FET group had a significantly higher mean age (32.59 ± 4.77 vs. 31.90 ± 4.71, p < 0.05) and lower multiple pregnancy rate (21.2% vs. 26.9%, p < 0.05

Non-invasive evaluation of embryo quality for the selection of transferable embryos in human in vitro fertilization-embryo transfer.

The ultimate goal of human assisted reproductive technology is to achieve a healthy pregnancy and birth, ideally from the selection and transfer of a single competent embryo. Recently, techniques for efficiently evaluating the state and quality of preimplantation embryos using time-lapse imaging systems have been applied. Artificial intelligence programs based on deep learning technology and big data analysis of time-lapse monitoring system during in vitro culture of preimplantation embryos have also been rapidly developed. In addition, several molecular markers of the secretome have been succ
