[Percutaneous gastro-enterostomy].

Percutaneous gastrostomies or gastroenterostomies serve for temporary or permanent enteric feeding in patients with obstruction or functional derangement of the esophagus or hypopharynx. In addition, this radiological procedure may be indicated for small bowel decompression. The authors present their experience in 71 patients. Insufflation of air through a nasogastric tube or catheter is the preferred method for gastric distension. The inferior margin of the left lobe of the liver and the transverse colon are localized sonographically and fluoroscopically prior to puncture. Either Seldinger or

[Enterostomy in Crohn disease].

Indications, technique and prognosis of 129 enterostomies in Crohn's disease are reported. Advanced perianal or entero-genital fistulas represented the predominant indication (60% of primary, 46% of repeated stomas). 85% of stomas were created in combination with intestinal resections. Preferred type of stoma was a loopileostomy (76% of primary stomas), whereas colostomies were avoided whenever possible because of increased complications. Patients initially presenting with rectal involvement or perianal fistulas were prone to need a stoma during the course of their disease while intraabdominal

Enterostomy complications in infants.

A retrospective review of 48 infants, less than one year of age, with enterostomies was carried out to analyse the complications. Early complications were encountered in 3 patients (5%). Stoma complications occurred in 7 patients (14.6%) and revision was required in 2 cases. Total or near total parenteral nutrition was needed in 23 patients (48%). Of the 25 who tolerated oral feeding, 16 (64%) required extra sodium in their diet. Stoma-closure in 37 patients had a leakage rate of 8% and no mortality. Careful attention to technical details, diligent postoperative stoma care and early closure of

Feeding by tube enterostomy.

Feeding by tube enterostomy is safe, economical and well tolerated by most patients. It will ameliorate or reverse the catabolic effects of protein-calorie malnutrition. Feeding tube enterostomy is indicated in patients with the combined findings: a need for long term nutritional therapy; inability to ingest adequate nutrients by mouth, and a normally functioning gastrointestinal tract. These feedings are most efficaciously delivered into the stomach and jejunum and the surgeon must be thoroughly familiar with the differences in commercial formulations prior to their use.

Complications of newborn enterostomies.

AIM: To evaluate the occurrence and severity of enterostomy complications in newborns suffering from different intestinal disorders. METHODS: A 10-year retrospective cohort study (2008-2017) investigated newborns that underwent enterostomy formation and reversal for different intestinal disorders. Only infants less than 28 d old at the time of enterostomy creation were included in the study (corrected age was applied in the cases of preterm neonates). The patients were divided into two groups according to their underlying diseases. Group 1 included infants suffering from necrotizing enterocol

Postoperative intussusception in children with enterostomy.

Postoperative intussusception (POI) is an uncommon but important cause of postoperative intestinal obstruction. We describe three cases of intussusception, which developed after major abdominal surgery. Two of the patients were premature newborns whose conditions were complicated by necrotizing enterocolitis and accepted ostomy. The other case involved a patient with Hirschsprung's disease who had had a previous colostomy and accepted Duhamel pull through procedure. Postoperative intussusception is a rare complication of pediatric abdominal surgery and may occur in premature infants. The ostom

Characterization of neonatal and infant enterostomy fluids.

The composition of gastrointestinal (GI) fluids is crucial for the dissolution, solubilization, and absorption of orally administered drugs. Disease- or age-related changes in GI fluid composition could significantly affect the pharmacokinetics of oral drugs. However, limited studies have been conducted on the characteristics of GI fluids in neonates and infants due to practical and ethical challenges. The current study collected enterostomy fluids from 21 neonate and infant patients over an extended period of time and from different regions of the small intestine and colon. The fluids were ch

Mucous fistula refeeding in neonates with enterostomies.

BACKGROUND/PURPOSE: Neonates with intestinal pathology may require staged surgery with creation of an enterostomy and mucous fistula (MF). Refeeding (MFR) of ostomy output may minimize fluid and electrolyte losses and reduce dependence on parenteral nutrition (PN), though a paucity of evidence exists to support this practice. The purpose of this study was to assess the outcomes of infants undergoing MFR and document associated complications. METHODS: With REB approval, infants with intestinal failure undergoing MFR between January 2000 and December 2012 were identified. A chart review was con

A reappraisal of the Mikulicz enterostomy in infants and children.

We performed the Mikulicz procedure in 46 pediatric patients. Thirty-five were high-risk patients, 20 of whom had necrotizing enterocolitis. High risk was defined by the presence of peritonitis, intestinal perforation, poorly demarcated intestinal gangrene, or severe associated systemic illness. The remaining 11 patients had the procedure performed for technical reasons, most commonly a discrepancy in the size of the proximal distal limb ratio greater than 4:1. The procedure consisted of intestinal resection with double-barreled enterostomy, crushing of the spur between stomas, and subsequent

Pancreatico-enterostomy for isolated main pancreatic duct disruption.

BACKGROUND: We present our experiences with isolated main pancreatic duct injuries due to blunt trauma, managed by pancreatico-enterostomies. METHODS: This is a retrospective study of seven patients, one female and six males who presented between 1997 and 2005, whose ages ranged from 10 to 54 years. Three were due to motor vehicle accidents, two due to blunt assault, one pedestrian vehicle accident and one go-cart accident. Four presented acutely and were managed surgically within 24h; two were delayed by 3 days and one by 14 days. Six had pre-operative CT scans; one had an ERCP confirming du

Sutureless enterostomy for extremely low birth weight infants.

OBJECTIVE: In recent years, improved survival rates of extremely low birth weight infants (ELBWIs) have led to an increasing number of enterostomy performed for those with meconium obstruction of prematurity (MOP)1,2, spontaneous intestinal perforation (SIP)3,4. To prevent serious stoma-related complications such as stoma side perforation, prolapse, fall and surgical site infection, we introduce our new "sutureless enterostomy" technique. METHODS: We present the procedures in detail. We reviewed the medical records of twelve patients who underwent "sutureless enterostomy" in our neonatal inte

The timing of enterostomy reversal after necrotizing enterocolitis.

BACKGROUND/PURPOSE: Newborns undergoing surgery for necrotizing enterocolitis (NEC) often require a stoma. The study purpose is to determine if the timing of stoma closure impacts the postoperative course. METHODS: After obtaining institutional review board approval, records of patients with NEC who received a stoma between 2003 and 2007 at 2 pediatric institutions were reviewed. Data collected included time interval between stoma creation and closure, indication for closure, postoperative complications, time to feeds, and length of neonatal intensive care unit and hospital stays. Surgery for necrotising enterocolitis: primary anastomosis or enterostomy?

The ideal surgical management of neonates with necrotising enterocolitis (NEC) is still a matter of debate. The purpose of this study was to compare the results of bowel resection with primary anastomosis with the results of bowel resection with enterostomy. Sixty-three neonates with NEC had a bowel resection in the acute phase of the disease in the period between February 1990 and March 2001. Thirty-four of them (54%) underwent resection of the bowel with primary anastomosis (Group A), and 29 (46%) had resection with enterostomy (Group B). Group A had a lower gestational age and lower birth w
