[EXCORIATION (SKIN-PICKING) DISORDER].

Excoriation (Skin-Picking) disorder is a clinically recognized condition which was recently included in the Diagnostic and Statistical manual of the American Psychiatric Association (DSM) - fifth edition, as OCD (obsessive compulsive disorder) related disorder. The disorder's official status has been achieved due to its high frequency and unique clinical picture involving both mental and physical impairment. In this article, we would like to present a concise review of the literature together with an illustrative case. Epidemiological surveys show a prevalence of 3% to 5% for the general popul

Self-Trephination in Cranial Excoriation Disorder.

BACKGROUND AND IMPORTANCE: Trephination is a procedure in which a small hole is made in the skull. Rare cases of self-trephination by individuals seeking medical benefit have been reported. Excoriation disorder is a compulsive skin-picking condition in which an individual self-inflicts cutaneous lesions. Left untreated, severe excoriation disorder can pose significant health risks. CLINICAL PRESENTATION: Here, we describe 5 patients who presented with self-trephination due to a severe form of compulsive cranial excoriation at 2 neighboring academic medical centers over a 4-year period. We rev

Low-Dose Naltrexone for Excoriation Disorder.

Excoriation (skin-picking) disorder (ED) is a condition characterized by the repeated compulsion to pick at the skin, causing physical trauma and psychiatric distress. Patients often desire to cease skin-picking behavior but are unable to do so. Multiple treatment modalities are effective for ED, including naltrexone. Previous reports of naltrexone for ED were at a high dose of 50 mg. The efficacy of low-dose naltrexone (LDN) at 4.5 mg in managing ED has not been reported. We present a case of a 51-year-old female with ED who was evaluated in the pain clinic for fibromyalgia management. Her me

Severe bullous pemphigoid with excoriation disorder.

Bullous pemphigoid is the most common autoimmune blistering skin disease. Pathogenesis involves autoantibodies that attack the basement membrane, resulting in blisters and intense pruritus. We present a case of bullous pemphigoid with concurrent excoriation disorder in a woman in her 50s. The suspected diagnosis of bullous pemphigoid was confirmed through direct immunofluorescence testing on a specimen obtained via punch biopsy, then treated with vancomycin and steroids. In addition, cross tapering from duloxetine to fluoxetine was used to treat the patient's excoriation disorder. The concurre

Prevalence of skin picking (excoriation) disorder.

Skin picking (excoriation) disorder is a mental health condition characterized by repetitive picking of one's skin leading to tissue damage as well as functional impairment and/or distress. A convenience sampling of 10,169 adults, aged 18-69 years, representative of the general US population, completed a survey to establish occurrence of skin picking disorder. 213 participants (2.1%) (55.4% female) identified as having current skin picking disorder and 318 (3.1%) (54.1% female) reported lifetime skin picking disorder (i.e. current or past). Those with current skin picking disorder were signifi

Trichotillomania and excoriation disorder: Primary care implications.

Trichotillomania and excoriation disorder are two psychiatric conditions that have similar origins to obsessive compulsive disorder. Both can cause patients significant distress and lead to medical consequences. It is important that primary care NPs identify and refer these individuals for psychiatric treatment.

Excoriation disorder: impulsivity and its clinical associations.

Excoriation disorder is the repetitive scratching or picking of skin that leads to physical damage, distress, and functional impairment. Skin picking has been associated with impulsivity and problems with inhibition. We hypothesized that problems in these areas could be disease severity markers. We recruited 73 adults meeting DSM-5 criteria for excoriation disorder, and 50 adult controls. Those with excoriation disorder were categorized as either "high impulsive" (HI) or "low impulsive" (LI) using either a neurocognitive task of motor impulsivity (Stop Signal Task) or the Barratt Impulsiveness

Investigating the familiality of trichotillomania and excoriation disorder.

Trichotillomania (TTM) and excoriation disorder (ED) run in families and are thought to have shared etiological underpinnings. Only a few small studies have compared the family history of individuals with TTM, ED, and both conditions. To better understand shared predispositions, we examined self-reported family history of mental health disorders using cross-sectional survey responses from a genetics study of probands ages 4-66 years with TTM only (n = 69), ED only (n = 34), and both conditions (n = 70). Individuals with TTM only reported higher rates of having a first-degree relative with TTM

Recurrent Midface Deep Ulcer Caused by Excoriation Disorder.

Excoriation disorder, or skin-picking disorder, is a compulsive condition that leads to self-inflicted tissue damage. The authors present a case of a 43-year-old female with a large and persistent facial wound exposing the left malar bone that remained unresponsive to multiple medical interventions. Imaging revealed a 4×3x4 cm defect extending into the intraoral cavity. Despite surgical debridement and reconstruction, wound dehiscence persisted until psychiatric evaluation confirmed excoriation disorder. Treatment included cervicofacial and subperiosteal rotation flaps, acellular dermal matrix

Group therapy for excoriation disorder: Psychodrama versus support therapy.

BACKGROUND: Excoriation disorder (ED) is characterized by recurring excoriation of the skin resulting in tissue damage, usually associated with emotional deregulation. Psychotherapy is a valuable treatment; however, no studies emphasize the patients' interactional aspect, nor the potential benefit of group treatment. METHODS: We recruited a convenience sample of 38 individuals with ED according to DSM-5 criteria, in which 19 individuals proceeded to treatment, 10 with psychodrama group therapy (PGT), and 9 with support group therapy (SGT) in an open pilot study. RESULTS: The entire sample pr

Characteristics of trichotillomania and excoriation disorder across the lifespan.

Trichotillomania (hair-pulling disorder) and excoriation (skin-picking) disorder are body-focused repetitive behaviors, which often first present in adolescence and cause distress and impairment into adulthood. Few studies have examined the clinical characteristics of the co-occurrence of these conditions across the lifespan. We examined cross-sectional survey responses collected from April 2018-February 2020 to evaluate the relationship between trichotillomania, excoriation disorder, and their co-occurrence. Responses from individuals with trichotillomania (n = 50), excoriation disorder (n =

Excoriation disorder vs skin-centered body dysmorphic disorder: A clinical and therapeutic update.

Skin-centered body dysmorphic disorder (BDD) and excoriation disorder (ED) are categorized under Obsessive Compulsive and Related Disorders in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) and are characterized by excessive or inappropriate skin picking. Patients with skin-centered BDD and ED often first present to dermatology. These conditions are important to recognize and appropriately diagnose, as perpetuation of the disorder is inevitable without appropriate psychiatric treatment. These conditions are associated with increased morbidity and BDD is associated with incre

Prevalence and predictors of hair pulling disorder and excoriation disorder in Tourette syndrome.

Trichotillomania/hair pulling disorder (HPD) and excoriation/skin picking disorder (SPD) are childhood-onset, body-focused repetitive behaviors that are thought to share genetic susceptibility and underlying pathophysiology with obsessive-compulsive disorder (OCD) and Tourette syndrome (TS). We sought to determine the prevalence of DSM-5 HPD and SPD in TS patients, and to identify clinical factors most associated with their co-morbidity with TS. Participants included 811 TS patients recruited from TS specialty clinics for a multi-center genetic study. Patients were assessed using standardized,

A genomic study of trichotillomania and excoriation disorder in families.

Trichotillomania and excoriation disorder are obsessive-compulsive related disorders that are often subclassified together as body-focused repetitive behavior (BFRB) disorders. While previous research suggests shared genetic factors, the genetic architecture of these BFRBs remains incompletely understood. Probands with trichotillomania and/or excoriation disorder and both of their biological parents were recruited for an ongoing genetic study of parent-offspring trios with BFRBs. Genome-wide array data were generated in 110 families (334 individuals total) to investigate the role of both commo

Review of Management Considerations for Excoriation Disorder in Older Adults.

Excoriation disorder (ED) is defined as compulsive skin picking that results in skin damage and emotional distress. Optimal management of ED includes individualized treatment plans consisting of psychotherapies, behavioral and pharmacologic interventions. Compared with younger populations, older adults are at increased risk for poor outcomes, such as infection and hospitalization, and require unique management considerations. Risk factors that contribute to disease burden include age-related changes to the skin and underlying medical and psychiatric comorbidities. A literature review was condu

Assessment and treatment of trichotillomania (hair pulling disorder) and excoriation (skin picking) disorder.

Recommendations are provided for the assessment and treatment of trichotillomania (hair pulling disorder, or HPD) and excoriation disorder (skin picking disorder, or SPD), two body-focused repetitive behavior (BFRB) disorders, based on their severity, comorbidities, and behavioral style. Habit reversal training (HRT) and stimulus control are first-line behavioral treatments that can be used in cases of all severity levels and may be particularly helpful when pulling or picking is performed with lowered awareness/intention.
