Preview Clostridial myonecrosis (gas gangrene) is an uncommon sequela of traumatic injury. Because the infection can become rapidly fatal, early recognition and expeditious treatment are critical. In addition to wide surgical debridement and treatment with broad-spectrum antibiotics, hyperbaric oxygen therapy has been shown to have synergistic benefit. Dr Stephens presents an illustrative case and discusses current data regarding effective management.

A 66-year-old woman presented with left lower abdominal pain and paresthesias of the left thigh of 6 hours duration. 10 years previously she had undergone massive abdominal irradiation for retroperitoneal lymphoma. 3 days prior to admission she underwent rectal biopsy as part of a work-up for suspected inflammatory bowel disease. Within a few hours she developed fulminating gas gangrene of the left thigh and lower abdomen. In spite of aggressive surgical debridement she died 12 hours after admission. The incidence of gas gangrene is decreasing, but the mortality is still high. The diagnosis re…

Gas gangrene is not a disease of the past. Despite improved awareness, earlier care of trauma victims, new antibiotics, and advanced monitoring techniques, histotoxic clostridia continue to cause loss of life and limb. A 20-year literature review on gas gangrene (Part I) indicates that a combined therapy approach with early recognition, surgical intervention, appropriate antibiotics, and hyperbaric oxygen (HBO) provides optimal care. Part II, a 15-year clinical experience, appears to be the largest English-language series reported using the combined therapy of antibiotics, surgery, and hyperba…
