We present a paediatric institutional experience with laparoscopic gastrostomies (LG) and evaluate its appropriateness as the recommended method for gastrostomy placement. We also sought to evaluate the efficacy of a simple technique for LG and collected information on long-term follow-up after LG. LG was performed in 112 children over a 6-year-period. The procedure involves visualization of the stomach through an umbilical port and a second epigastric gastrostomy site to select and anchor the stomach with sutures prior to the placement of a low profile gastrostomy feeding device (LPGD). PURPOSE: To describe a modification in the Georgeson technique for laparoscopic gastrostomy tube (LGT) placement and compare the rate of reoperation for disruption of the gastrostomy tract after LGT by our modified technique to our own series of percutaneous endoscopic gastrostomy (PEG) tubes as well as the published rates of reoperation for LGTs and PEGs.
METHODS: In 2003, we modified our technique for LGT to include laparoscopically placed sutures to secure the stomach to the abdominal wall. A retrospective review was performed on all children undergoing LGT placement and PEGs from March 20

In a review of 125 percutaneous endoscopic gastrostomies (PEG) and 88 Stamm gastrostomies performed at Duke University Medical Center since 1978, the average operating room time for PEG (50 +/- 20 min) was shorter than for Stamm (96 +/- 26 min) (p less than 0.0001). General anesthesia was administered in only 13% of PEG placements compared with 64% of Stamm gastrostomies. The cost of PEG was about $1000 less than for Stamm gastrostomies. The average time after surgery until use of the feeding tube was 1.8 days for PEG compared with 3.4 days for Stamm (p less than 0.0001). The overall complicat

Radiologic gastrostomy has a higher success rate and a lower complication rate and offers a greater choice of tubes than percutaneous endoscopic gastrostomy (PEG). The position and configuration of the stomach and colon are clearly seen under fluoroscopy, and ultrasound can be used to locate the liver. Radiologic gastrostomy procedures can be performed when there are oropharyngeal tumors, or esophageal strictures and stents, and can be performed under local anesthesia alone. Peroral push-gastrostomies are preferable for palliative care and for patients with neurogenic dysphagia, but percutaneo

Endoscopic gastrostomy is one procedure that is recommended for feeding or for gastric drainage, when the permanence of the feeding tube is required for a long time. This has arisen as a better alternative than surgical gastrostomy, because of its simplicity, efficiency, lower costs, and scarce morbidity-mortality. The most commonly recommended technique consists of gastric punction by percutaneous route and traction of the feeding tube from the mouth with the aid of an endoscopy. The indications are the necessity of feeding patients in a critical state who have gastric disorders and in whom p

For gastrostomy after thoracic esophagus extirpation its distal stump was used. It is confirmed, that transesophageal gastrostomy has a number of advantages. Pezzer's catheter can be used as gastrostomic tube which self-fixes in given position and secures reliable gastrostoma sealing. Suturing of the stomach to parietal peritoneum around gastrostoma is not more necessary. Deformation and reduction of the stomach size are excluded and stomach is kept ready for subsequent plastic replacement of the oesophagus. The operation gets oncologically more radical as a result of removal of paraesophageal

Although percutaneous endoscopic gastrostomy (PEG) has become a common technique for the placement of gastrostomy tubes, gastrostomy can be performed via the laparoscopic approach with minimal trauma or by using a percutaneous gastrostomy kit. In this report, we describe two procedures for laparoscopic gastrostomy using standard instruments. Standard laparoscopic techniques are used to create a pneumoperitoneum. In the first method, three transparieto-transgastric U stitches are placed to surround the site selected for the gastrostomy. A Foley catheter is inserted through the abdominal and gas

Percutaneous gastrostomy was developed over the last years as an alternative method to surgical and endoscopic procedures, either for enteral feeding or for drainage of secretions or fluid collections communicating with the upper gastrointestinal (GI) tract. January 1990 through January 1992, 15 percutaneous gastrostomies and 2 percutaneous jejunostomies were performed at the Gastroenterologic Radiology Division of INT, Milan, Italy. The catheters were always inserted under local anesthesia, generally using special gastrostomy sets. Catheter insertion was possible in all the patients who under

BackgroundGastrostomy has been established as the standard procedure for administering long-term enteral nutrition in individuals with swallowing disturbances. Percutaneous gastrostomy is a less-invasive approach than open surgical gastrostomy, and can be accomplished via endoscopy (percutaneous endoscopic gastrostomy or PEG) or sonographic or fluoroscopic guidance (percutaneous radiological gastrostomy or PRG). Both techniques have different limitations, advantages, and contraindications. In order to determine the optimal technique for long-term nutritional supplementation many studies have b

In an attempt to facilitate long-term care and patient acceptance of gastrostomy feeding, the technique of permanent Janeway gastrostomy was modified. Using an auto-stapling device, a full-thickness gastric tube (6 cm long and 1.5 cm in diameter at its base) is created from the anterior wall of the stomach, based on the greater curve, with special attention to its vasculature. The base is invaginated into the stomach wall to create a tight valve. After skin closure, the terminal 1 cm is excised and the mucosa is sutured flush with the skin. A #10 French tube is inserted and positioned perpendi

Despite its simplicity, gastrostomy has been associated with a relatively high incidence of complications. We have described two unusual complications: inadvertent duodenostomy, resulting in gastric outlet obstruction, and the migration of a gastrostomy feeding tube into the midportion of the jejunum, resulting in obstruction of the small bowel.

Gastrostomy tube placement is a well-established technique for providing long-term feeding or decompression to patients with dysphagia or the need for gastric venting. However, direct transabdominal access to the stomach is not feasible in some patients, such as those without a safe percutaneous route to the stomach, with conditions that may inhibit tract healing, and/or at high risk of peritonitis. For these patients, percutaneous transesophageal gastrostomy (PTEG) is a safe and effective technique for providing gastric access for enteral feeding or decompression. PTEG placement involves ultr

Tube feeding is commonly used in providing enteral nutrition to the patient with dysphagia. The percutaneous endoscopic gastrostomy (PEG) is an alternative to the nasogastric tube (NG) and the surgical gastrostomy. The advantages of PEG are short procedure time, the avoidance of using an operating theatre and general anaesthesia and reduced cost. The main indication for PEG is dysphagia due to neurological disease and cancer in the pharynx and oesophagus. A gastrostomy is cosmetically more acceptable than NG, self extubation is more difficult for the confused patient, tube diameter is larger a

Probes for the percutaneous endoscopic gastrostomy (PEG) are placed by pull-through methods or by direct puncture of the stomach. Depending on the length of the interior branch of the probe, gastral or (duodeno-) jejunal feeding is possible; the latter may also be achieved by direct percutaneous endoscopic jejunostomy (PEJ). Small calibre endoscopes can be introduced via large-bore PEG probes; after the extraction of such a probe the resulting fistulous tract itself may also be used für percutaneous endoscopy by using pediatric endoscopes. The usual external fixation of the probes with compres

We present the case of a woman on whom a percutaneous endoscopic gastrostomy (PEG) was performed through the sinus tract of a previous surgical gastrostomy for supraglottic obstructing malignancy. Five years after the induction of the surgical gastrostomy, she experienced a peristomal leakage, leading to severe necrotizing fasciitis, with skin irritation and inflammation. Despite extensive treatment to heal the abdominal wall close to the feeding tube, it recurred 3 months later, without any obvious cause. It was thus decided to perform a new gastrostomy in a nearby normal skin area, but, sinc
