Despite the continuous technical advancements around health information standards, a critical component to their widespread adoption involves political agreement between a diverse set of stakeholders. Countries that have addressed this issue have used diverse strategies. In this vision paper we present the path that Chile is taking to establish a national program to implement health information standards and achieve interoperability. The Chilean government established an inter-agency program to define the current interoperability situation, existing gaps, barriers, and facilitators for interop

Health information systems supporting shared care are going to be distributed and interoperable. Dealing with sensitive personal medical information, such information systems have to provide appropriate security services, allowing only authorised users restricted access rights to the patients' data according to the 'need to know' principle. Especially in healthcare, chip card based information systems occur in the shape of patient data cards providing informational self determination and mobility of the users as well as quality, integrity, accountability, and availability of the data stored on

Interoperability of Health Information Systems (HIS) is critical for improving healthcare delivery, decision-making, and resource management in Low- and Middle-Income Countries (LMICs). However, challenges such as fragmented systems, limited digital infrastructure, and the absence of standardized health records hinder progress. This study proposes the Cluster Care System (CCS), a modular, cluster-based framework designed to address these challenges. Its architecture includes local Cluster Information Systems, a Cluster Master Patient Index for unified patient identification, and a Cluster Ente

BACKGROUND: Unstructured clinical text remains a major barrier to interoperable data reuse and large-scale secondary analysis in health care. Large language models (LLMs) have the potential to automate the extraction of structured clinical information; however, their application is limited by the scarcity of high-quality annotated training data.
OBJECTIVE: To address these limitations, this study aims to develop and validate a scalable, privacy-preserving framework that uses synthetic data generated from structured Fast Healthcare Interoperability Resources (FHIR) to fine-tune open-source LLM

Following the proposal by Tsafnat et al (2024) to converge on three open health data standards, this viewpoint offers a critical reflection on their proposed alignment of openEHR, Fast Health Interoperability Resources (FHIR), and Observational Medical Outcomes Partnership (OMOP) as default data standards for clinical care and administration, data exchange, and longitudinal analysis, respectively. We argue that open standards are a necessary but not sufficient condition to achieve health data interoperability. The ecosystem of open-source software needs to be considered when choosing an approp

The rising prevalence of developmental conditions such as autism, ADHD, and learning disorders underscores the urgent need for an integrated approach to account for the dynamic interaction between genetic, environmental, and societal factors. At present, the information on these factors is scattered and fragmented across a wide range of caregivers using different systems, structures and semantics. As a result, healthcare providers often face excessive administrative burdens to gather relevant information from disparate sources, and in many cases, must repeat diagnostic tests due to a lack of a

BACKGROUND: The use of electronic medical records (EMRs) and clinical registries has transformed health care delivery by improving data management, care coordination, and research capacity. However, the full potential of these technologies can only be realized through effective interoperability, thereby reducing the burden of manual data entry and enhancing the use of real-world clinical data.
OBJECTIVE: This review examines technologies that enable automated data extraction and transfer, which promote interoperability between EMRs and clinical registries.
METHODS: A search of PubMed, CINAHL

BACKGROUND: Although individually uncommon, rare diseases (RDs) collectively affect an estimated 329-624 million people worldwide. There are over 6500 known RDs, 85% of which affect fewer than 1 person per million. Consequently, the critical amount of data necessary to improve knowledge, care, and treatment can only be achieved through cumulative data collection across countries. However, RDs remain underrepresented in medical terminologies and classification systems, hindering data sharing, interoperability, and public health monitoring.
OBJECTIVE: This paper presents the Orphanet Nomenclatu

Transitions in national level health information systems (HIS) require careful assessment of technical, organizational, and policy implications, particularly in low- and middle-income countries. Mozambique is currently transitioning from a long-standing OpenMRS-based HIV electronic medical record (EMR) to a new national python-based EMR. This paper proposes a conceptual framework to guide such transitions, synthesizing digital health standards, enterprise architecture principles, and implementation science. The framework outlines five analytic domains: technical architecture; data governance a

Rare diseases affect an estimated 446 million people worldwide, yet reliable epidemiological data are scarce due to insufficient data sources and lack of standardization. The German National Registry for Rare Diseases (NARSE) aims to establish a comprehensive epidemiological data basis and enable data availability for research. However, limited interoperability between clinical and epidemiological data sources restricts secondary use and cross-registry research. This study assesses the semantic interoperability between NARSE and two core data sets: the German Medical Informatics Initiative (MI

Rare cancers hinder data sharing and secondary use due to low incidence, heterogeneous pathways, and fragmented sources. The IDEA4RC project addresses these issues by creating a federated ecosystem for harmonized data exchange across European Centers of Excellence. This paper presents the IDEA4RC HL7 FHIR Implementation Guide (IG), which operationalizes the IDEA4RC common data model-organizing 279 clinical variables across 19 entities-into computable specifications for semantic and computational interoperability. The IDEA4RC HL7 FHIR IG combines logical models, constrained resource profiles, t

BACKGROUND: Health data management during disasters enables responders to assess the needs of survivors, efficiently allocate resources, and monitor survivors' health conditions. However, government agencies need to interpret health data in real time during disasters because the volume and complexity of such data can hinder timely decision-making.
OBJECTIVE: In this study, we aimed to develop a visual interface for Fast Healthcare Interoperability Resources (FHIR)-based data from various sources, using the daily report form of the World Health Organization (WHO) Emergency Medical Teams Minimu

BACKGROUND: In 2016, the National Health Service (NHS) England sought to drive digital transformation within select NHS trusts through the Global Digital Exemplar (GDE) program. While the program did advance the NHS's integration with digital technologies, disparities in digital maturity persisted between GDE-funded and nonfunded NHS trusts. The Department of Health and Social Care (DHSC) launched a data strategy in 2022 that aimed to develop the appropriate technical infrastructure and data architecture to enable more effective and efficient use of its data. Given the diversity in digital cap

The Swiss Personalized Health Network is a national network aiming to facilitate responsible use of health-related data for research in Switzerland. Central to this effort is the SPHN RDF Schema, a graph-based model leveraging Resource Description Framework (RDF) to encode semantics. The use of RDF and semantic web technologies enable fine-grained representation of health-related entities and facilitates the integration of heterogeneous clinical datasets into interoperable knowledge graphs suitable for research. Fast Healthcare Interoperability Resources (FHIR®) in contrast is optimized for da

Semantic interoperability gaps persist despite standards such as HL7 FHIR, and SNOMED CT. Using a design science approach, our research investigates how a language for the formal specification of semantics can be developed, based on the principles of the ABC4HEDA (Archetype-based Computing for Health Data) model and structured according to the Meta-Object Facility (MOF) standard. The intended contribution is a conceptual basis that may support future work toward more semantically unified medical knowledge graphs. Future work will focus on implementation within the TermX platform to support dis
