Hemobilia is an uncommon medical problem that presents in a varied fashion and is increasingly of iatrogenic origin. The diagnosis of hemobilia needs to be considered in patients presenting with upper gastrointestinal bleeding, particularly if they are jaundiced with abdominal pain in the setting of recent or previous percutaneous liver intervention or abdominal trauma. Multislice computed tomographic angiography is increasingly being used in the investigation, but transcatheter arterial embolization remains the cornerstone of managing those patients requiring intervention. The majority of pat…

Biliary tract hemorrhage is increasingly reported as a complication of diagnostic and therapeutic interventions on the liver and the bile ducts. Major hemobilia with profuse, even life-threatening, hemorrhage which necessitates emergency surgical intervention is uncommon. Minor hemobilia with melena or clot production in the ducts, although more common, may cause diagnostic difficulties. Diagnosis is obtained either endoscopically to demonstrate bleeding through the papilla of Vater, or through hepatic arteriography, which has the advantage of indicating the location of the lesion. Operative t…

Hemobilia, or hemorrhage into the biliary system, is an unusual cause of gastrointestinal bleeding most commonly seen in accidental or iatrogenic trauma. We present the rare case of a 43-year-old gentleman who presents with an intrahepatic pseudoaneurysm caused by cholecystitis. The management of the hemobilia was technically challenging requiring multiple interventional procedures. We review the pathophysiology and treatment strategies for this rare case of gastrointestinal hemorrhage.

Hemobilia is a syndrome caused by hemorrhage into the biliary ducts. It is characterized by colicky pains, jaundice, and intestinal bleeding; a correct diagnosis of this syndrome is made rather rarely. The most common cause is trauma; inflammatory or tumorous disease of the liver and bile duct system, bile stones and aneurysms of the hepatic artery are rarer causes. The diagnosis can be established and the arterio-biliary fistula localized by selective arteriography. A case is described, which shows, that, in addition, duodenoscopy with retrograde cholangiography is of diagnostic value. Other …

AIM OF THE STUDY: Hemobilia, defined as hemorrhage arising from pathological changes in the biliary tract, represents a rare entity. In a retrospective study the presentation, diagnosis, and management of hemobilia were investigated.
PATIENTS AND METHODS: Since January 1991 six patients suffering from severe hemobilia presented to our department. Jaundice, right upper quadrant abdominal pain, hematemesis and melaena were the most frequent symptoms leading to admission. Hemobilia originated from iatrogenic injury following percutaneous liver biopsy and endoscopic removal of common bile duct st…

A case of postbiopsy hemobilia is presented in which computed tomographic (CT) scanning showed blood within the gallbladder appearing as high-density material measuring 67-91 HU. Residual clots were seen by CT and ultrasound 8 days after the acute episode. These findings were confirmed by serial CT scans in two monkeys in whom blood was experimentally injected into the gallbladder. When the cystic duct is patent, the diagnosis of hemobilia may be excluded if bile of normal density (0-20 HU) is demonstrated by CT scanning. However, when homogeneous or inhomogeneous material of high attenuation …

In 100 patients before and after a percutaneous blind puncture of the liver after Menghini biliary juice was received with the help of a duodenal sound and this was investigated for blood admixtures microscopically and chemically. In 21 out of 100 punctures patients blood was proved in the duodenal juice: In 11 cases the proof of blood was positive before and after the puncture and in 10 patients only after the puncture blood was found in the duodenal juice. A macrohaemobilia was never observed. The influence of thromboplastin time, number of thrombocytes, time of haemorrhage, age and basic di

A 4-year-old boy was hit by a car travelling at 40 km/h and was admitted 3.5 h later to the department of paediatric surgery. Because he was … patient was re-admitted with abdominal pain, again with haematemesis and melaena. The diagnosis of hemobilia was obtained with MRI and angiography, which revealed a ruptured pseudoaneurysm of the ramus dexter of the proper hepatic artery. The patient was successfully treated with embolization. The diagnostic delay was two months, which illustrates the impo …

Haematobilia is caused by a pathological linking of the arterial vascular system and intrahepatic or extrahepatic bile ducts. This report presents the case of a traumatic haematobilia in a child. Possibilities to achieve safe diagnosis are discussed and suggestions for treatment are considered. In the case presented here, occlusion of the arteriobiliar shunt is effected by means of a rejectable balloon catheter according to Serbinenko. This method has been successfully tried in neurosurgery and developed further, and can be applied to similar disease patterns in disciplines other than neurosur
