Chronic hepatitis is defined as primary necroinflammatory liver disease with a duration of at least 6 months. It is etiologically heterogeneous and may be caused by hepatitis viruses (B, C, D, others?), autoimmune processes, drug reactions and metabolic disorders. A certain percentage of chronic hepatitides is etiologically still uncertain (cryptogenic). Nomenclature and classification of chronic hepatitis proposed in 1968 on the basis of morphologic appearance is still widely used. However, in view of the recent advances in our knowledge of etiology, pathogenesis, course of the disease, and,

Chronic hepatitis may be viral or "lupoid" or may be related to drug use, alcoholism, or Wilson's disease. This article outlines the routine and special laboratory investigations that are indicated when the presence of chronic hepatitis is suspected from symptoms referable to the liver or from biochemical or physical abnormalities in an asymptomatic patient. The importance of needle liver biopsy in diagnosis and in follow-up is emphasized. Special attention is given to the selection of patients for prednisolone therapy.

The term "chronic hepatitis" includes diseases of different etiology, i.e. viral (hepatitis B virus, non-A/non-B hepatitis virus[es]), drug-induced (e.g. oxyphenisatin, alpha-methyldopa), metabolic (Wilson's disease, alpha 1-antitrypsin deficiency) and so-called "autoimmune" hepatitis. In the clinical course, hepatitis running for up to 3 months is considered acute; when lasting for 3-6 months it is termed prolonged, and chronic hepatitis means a duration of more than 6 months by definition. In chronic hepatitis there is international agreement on basing nomenclature on morphologic findings an

Histologic evaluation of the liver is a major component in the medical management and treatment algorithm of patients with chronic hepatitis B (HBV) and chronic hepatitis C (HCV). Liver biopsy in these patients remains the gold standard, and decisions on treatment are often predicated on the degree of damage and stage of fibrosis. This article outlines the clinical course and serologic diagnosis of HBV and HCV for the clinician and the pathologist, who together have a close working relationship in managing patients with acute and chronic liver disease. The salient histologic features are eluci

INTRODUCTION: About 60% to 85% of people infected with hepatitis C virus will go on to develop chronic hepatitis C, which is now believed to affect 3% of the world's population.
METHODS AND OUTCOMES: We conducted a systematic overview and aimed to answer the following clinical questions: What are the effects of interferon-free treatments in treatment-naïve people with chronic hepatitis C infection without cirrhosis? What are the effects of interferon-free treatments in treatment-naïve people with chronic hepatitis C infection with cirrhosis? We searched: Medline, Embase, The Cochrane Library,

The hepatitis E virus is an RNA virus with 4 genotypes. Genotypes 1 and 2 only affect humans and produce epidemic acute hepatitis in endemic regions (Asia and Africa). Genotypes 3 and 4 are considered a zoonosis and produce episodic acute hepatitis in non-endemic areas (America and Europe). However, in the last 3 years there have been cases of chronic HEV hepatitis, mainly in immunosuppressed patients. Here we review the known data of chronic hepatitis E in terms of evolution and treatment.

Among hepatitis A to E viruses, hepatitis B, C, and D viruses can cause chronic hepatitis, in both children and adults. Hepatitis B virus (HBV) infection is the most prevalent and important one. Perinatal transmission accounts for about 40-45% of chronic HBV infection in hyperendemic areas. Horizontal transmission through intramuscular injection using non-sterile needles and intrafamilial spread accounts for the other half of carriers. During the natural course of HBV infection, the host gradually clears HBV and hepatitis B e antigen (HBeAg), liver damage and elevation of aminotransferases occ

Three cases are described in which a chronic relapsing febrile illness was associated with granulomas in the liver. None of the known causes of hepatic granulomas was present in these patients. In two cases there was clinical improvement after corticosteroid treatment and this was associated with resolution of the histological changes on liver biopsy. These three cases support the concept that chronic granulomatous hepatitis of unknown aetiology is a distinct entity.

BACKGROUND: Infection with the hepatitis C virus (HCV) is a leading cause of chronic liver disease worldwide. Epidemiologic and natural history studies have helped to define the clinical impact of HCV infection, and molecular diagnostic assays have established clinical endpoints against which therapeutic regimens are currently tested. The absence of definitive therapy has generated controversy regarding indications and optimal candidacy for currently approved treatment. This paper reviews the epidemiology, natural history, clinical manifestations, diagnostic modalities, and current treatment o

Formerly the diagnosis of acute and chronic non-A, non-B hepatitis was made by the exclusion of other causes. However, in 1989 cloning of an antigenic component of the hepatitis C virus (HCV) was reported. This led to first- and second-generation tests for antibody to HCV (anti-HCV) in serum. HCV has been associated with acute and chronic posttransfusion and sporadic non-A, non-B hepatitis, and with hepatocellular carcinoma. Viral HCV RNA can be estimated with the polymerase chain reaction test, but this technically difficult test is not generally available. The entire viral genome has been se

Chronic viral hepatitis is a common clinical problem; it must be differentiated from other forms of chronic liver disease by history, laboratory data, and liver biopsy. This article reviews the treatment of chronic hepatitis B, delta hepatitis, and nonA and nonB viral hepatitis and emphasizes the controlled trials when available.

Hepatitis A virus (HAV) infection is a well-recognized cause of acute hepatitis with jaundice in adults. However, chronic sequelae of HAV infection are so rare and so poorly defined that their existence remains speculative. In this report we describe a middle-aged woman in whom serologically defined acute hepatitis A appeared to trigger the onset of autoimmune chronic active hepatitis. This and two previously reported cases suggest that in certain individuals, presumably with a genetic predisposition to the development of autoimmune hepatitis, HAV infection may be the precipitating event in th

Two male homosexual patients, one an intravenous (IV) drug abuser, had hepatitis B surface antigen-positive fulminant hepatitis. On the basis of tests for IgM antibody to hepatitis B core antigen (HBcAg) and for IgM and IgG antibodies to delta agent, acute delta hepatitis in chronic hepatitis B infection was diagnosed. The delta agent appeared to have a role in producing fulminant hepatic failure. Accurate diagnosis of fulminant hepatitis may be impossible without recourse to tests for IgM antibody to HBcAg and antibodies to delta agent. Although delta hepatitis in the United States primarily

It has been suggested that hepatitis C virus (HCV) is selectively transmitted to a new host as an infectious clone from multiple HCV variants (quasispecies) in the donor. Most individuals with HCV infection develop chronic hepatitis, but approximately 15%-40% of them clear the virus spontaneously and the hepatitis is resolved in a self-limiting manner in the acute phase of infection. This difference in the outcome of acute hepatitis C is attributable to both viral characteristics and genetic regulation of infection. In particular, the evolutionary dynamics of the infecting virus and host genet

Chronic hepatitis comprises a group of disorders of the liver exhibiting a chronic necroinflammatory process that differs in etiology, clinical course and treatment strategies. A diagnosis of chronic hepatitis is usually made when inflammation and liver cell necrosis persist for longer than 6 months. Clinical manifestations range from asymptomatic patients to those with advanced hepatic failure. Both sexes and all age groups are affected. Chronic hepatitis may emerge as a sequelae of hepatitis C and less often after hepatitis B. Both diseases are treatable and require rapid and exact diagnosis

The clinical expressions, courses and consequences of hepatitis caused by different viruses (A,B,C,D,E,G) are different. Diagnosis of hepatitis is incomplete unless its etiology is specified and for chronic hepatitis, the etiology is apparent in almost all cases when autoimmune and metabolic diseases are also included. Hence, the classification based only on histology is not adequate and emphasis should also be on the etiology. The prognostic indices governing a response to interferon therapy in patients with chronic viral hepatitis have advanced with the knowledge of role played by viral geno

Autoimmune chronic active hepatitis, now often refereed to as simply autoimmune hepatitis has been recognised for nearly 50 years. Typically, the disease arises in a young woman who presents with an acute hepatitic illness and complains of lethargy, arthralgia, oligomenorrhoea, and fluctuating jaundice. For the purposes of clinical trials and research, guidelines for establishing the diagnosis have been published recently, but in clinical practise it is diagnosed when there is a histological picture of chronic active hepatitis together with immunological features (high levels of immunoglobulin
