AIM: To assess the intraocular pressure (IOP) variability in patients with primary open angle glaucoma (POAG) under clinical treatment who reached an established target pressure based on isolated office readings.
METHODS: Retrospective analysis of 65 eyes from 65 POAG patients under clinical therapy who submitted to modified diurnal tension curve (mDTC) (measurements at every 3 hours between 8 am and 5 pm) followed by a water drinking test (WDT). All subjects had established target IOP < or =15 mm Hg at 11 am or 2 pm. IOP variability during mDTC or WDT was evaluated.
RESULTS: mDTC revealed I

To date, evidence from multiple randomized controlled trials has shown that effective intraocular pressure (IOP)-lowering therapy significantly reduces the risk of glaucomatous progression across all stages of the disease. Changes in IOP have a substantial impact on the load-bearing connective tissues of the optic nerve head (ONH), as well as the overlying neurovascular tissues of the ONH and retina. An initial treatment goal of reducing IOP by 25% to 35%-and possibly more in advanced cases-can help prevent progression. Additionally, maintaining stable IOP by minimizing both diurnal and long-t

In the Miami Intraocular Lens Collaborative Study, 17 patients with glaucoma had a Copeland lens implanted at the time of cataract extraction. Seven additional glaucoma patients received a Binkhorst lens. Lens implantation did not adversely affect intraocular pressure or control of glaucoma, except in one patient who developed central retinal artery occlusion with rubeosis 1 1/2 years postoperatively and in one patient who had the lens removed because of iritis with uncontrolled glaucoma. Of 606 eyes that did not have glaucoma before cataract extraction and Copeland lens implant, many had tran

Over the past decade, results from prospective, randomized, clinical trials have confirmed the value of reducing intraocular pressure (IOP) in patients with ocular hypertension or primary open-angle glaucoma and have outlined the need to consider a target IOP in an individual glaucomatous patient and not an arbitrary value of 21 mm Hg as classically believed. The target IOP corresponds to an estimation of the mean IOP obtained with treatment that is expected to prevent further glaucomatous damage. Target IOP is difficult to assess accurately in advance in every individual patient and eye. More

Bilateral recordings with the applanating suction cup tonograph on eyes of 85 persons with and without glaucoma show that the intraocular pressure pulse amplitudes (IOPA) increase in size with the intraocular pressure (IOP) from an average of 1.85 mmHg at 12 mmHg IOP to 4.8 mmHg at 37 mmHg IOP. This relation resembles the one found between IOP increments and IOP by the injection of 157 equal volumes of saline into 10 eyes of human cadavers. The resemblance indicates that the IOPA versus IOP relation of the live eyes in all essentials is explained by systolic net equal-volume inputs of blood in

PURPOSE: This study aimed to investigate intraocular pressure (IOP) measurements taken using the dynamic contour tonometer (DCT) and the non-contact tonometer (NCT) in subjects with keratoconus.
METHODS: Intraocular pressure was measured in 20 keratoconus subjects and 20 age-matched control subjects using the DCT and NCT instruments. Central and off-centre measures were taken with the DCT in order to highlight any systematic errors associated with corneal biomechanical factors. Measures of anterior and posterior corneal topography and thickness were also taken in each subject.
RESULTS: No si

BACKGROUND: Implantation of rigid gold or platinum lid weights (lidloading) represents the most common method for therapy of patients with lagophthalmus as a result of facial palsy. In a more recent approach also flexible platinum chains have been used. Glaucoma has frequently been described as a contraindication for lidloading. The aim of this study was to proof if lid weights lead to an increase of the intraocular pressure (IOP).
PATIENTS AND METHODS: In a prospective study the IOP has been measured before and after implantation of 46 rigid gold implants and 39 flexible platinum chains in p

Hemodialysis, a useful treatment for patients with severely compromised renal function, has also unfavorable side effects. In the ophthalmologic area, a rise in intraocular pressure (IOP) during dialysis accompanied by ocular pain has been reported. In our study when measuring IOP, as well as serum osmolality and plasma CO2 pressure every 30 minutes during routine hemodialysis in renal failure patients with a normal aqueous outflow facility, the mean percent changes of IOP to the initial value showed no significant difference at any time, although the changes in serum osmolality decreased sign

Thirty-one patients (61 eyes) had a rise in intraocular pressure averaging 8.2 mm Hg following the use of succinylcholine as a muscle relaxant prior to general anesthesia. Just prior to the start of surgery (an average of 14 minutes following Anectine administration), the tension had returned to the control level. Ten patients (20 eyes) had pancuronium used without succinylcholine. No change in intraocular tension was noted with this agent and pressure remained unchanged at the start of surgery. Work of other investigators, using the "spring back balance" test and strain gauge force measuremen

Glaucoma is a leading cause of irreversible blindness. Intraocular pressure (IOP) is the only modifiable risk factor for glaucoma, yet there is little known about the molecular events that regulate IOP. Genetic and genomic studies have helped identify genes that influence IOP and could lead to the identification of biological pathways that serve as targets for novel pressure-modifying therapies. Genetic linkage studies resulted in the identification of several genes that cause Mendelian (autosomal dominant or autosomal recessive) forms of high-pressure glaucoma, including MYOC. The relationship between intraocular pressure (IOP) and arterial blood pressure (BP) was analyzed in response to two pressor agents (norepinephrine and angiotensin II) and two depressor agents (methacholine and isoproterenol) in pentobarbital anesthetized cats. Both IOP and BP were measured manometrically in the same animals. Intravenous norepinephrine and angiotensin II produced dose-dependent increases, whereas intravenous methacholine and isoproterenol produced dose-dependent decreases of both BP and IOP. The IOP response to methacholine was biphasic, with an initial decrease followed by an

We found intraocular pressures of less than 7 mm Hg in 11 of 72 patients (15%) with early Hansen's disease. Furthermore, significant postural changes in intraocular pressure (greater than 30%) occurred in 43 of 72 patients (60%). This loss of intraocular pressure homeostasis, which results from reduced aqueous humor production or increased uveoscleral outflow with reduced local adrenergic control in the ciliary body-iris-trabecular meshwork region, suggests that early autonomic neuropathy of the eye may occur in Hansen's disease. The only predictive value for significant postural change in int

PURPOSE: Assessment of intraocular pressure (IOP) after vitreoretinal surgery is important to ensure functionality of the eye. Incidences and risk factors for early postoperative IOP elevation were evaluated.
METHODS: In a prospective case series of 210 vitreoretinal cases, IOP-lowering treatment was performed at IOP values of ≥30 mmHg. Differences in IOP elevation in relation to surgical procedures and tamponades were evaluated.
RESULTS: Sixty-two patients required treatment (29.5%). Encircling bands were associated with a high risk for IOP elevation when combined with pars plana vitrectomy

Intraocular pressure (IOP) is the cardinal and only modifiable risk factor for glaucoma, the leading cause of irreparable blindness worldwide. Twin and family studies estimate the heritability of IOP to be 40-70%, and linkage studies for IOP have identified numerous loci. Mutations in MYOC can cause markedly elevated IOP and aggressive glaucoma often requiring surgical intervention. However, the majority of the genetic basis for raised IOP and glaucoma in populations is complex, and recent large genome-wide association studies (GWASs) have identified over 100 common variants that contribute to

Cataract surgery is one of the most performed surgeries in the developed world. In addition to its significant impact on visual acuity, phacoemulsification has been hailed as a potential intraocular pressure (IOP)-lowering procedure. While current evidence suggests an overall significant and sustained decrease in IOP to exist after cataract surgery, the specific ocular characteristics that could help predict which patients are likely to benefit from this IOP-lowering effect remain unclear. This definition is important in glaucoma patients if this surgery is to be used in the treatment for this

OBJECTIVE: To measure the intraocular pressure (IOP) in different sleep positions and compare with IOP in the sitting position.
METHODS: Perspective study. Thirty-two patients with primary open angle glaucoma (POAG) and 28 normal subjects had IOP measured in the sitting position and then again in the supine, right lateral decubitus (right-lateral) and left lateral decubitus (left-lateral) positions with a Perkins applanation tonometer. IOPs then were measured again in the sitting postion after 1, 5 and 10 minutes. Two-way ANOVA was used to compare IOP readings in the different positions among
