Disorders of iron metabolism.

Although iron in minute amounts is necessary for the metabolism of most cells, it produces damage if present in excess. Clinical tools for the evaluation of iron stores include serum iron concentration, transferrin saturation, deferoxamine test, Prussian blue stains of liver and marrow, and serum ferritin concentration. Serum ferritin concentration is an excellent screening test for iron deficiency or excessive iron stores. If excessive iron stores is diagnosed, the iron should be removed with phlebotomies or chelation therapy.

Inherited disorders of iron metabolism.

PURPOSE OF REVIEW: To discuss inherited iron disorders, their pathophysiology and clinical implications in the light of the recent advances in our knowledge of iron metabolism and its regulation. RECENT FINDINGS: In previous years the molecular mechanisms of cellular iron uptake and release and the cellular and systemic iron homeostasis have been substantially clarified. New proteins (hepcidin, hemojuvelin, HFE, TFR2 and ferroportin), mutated in hereditary hemochromatosis, have been identified with a crucial role in iron regulation. These advances have modified our understanding of the pathop

Inherited disorders of iron metabolism.

Recent molecular studies have resulted in the identification of genetic alterations underlying hereditary disorders of iron metabolism. One example is the discovery of the HFE gene that is mutated in patients suffering from hereditary hemochromatosis. This autosomal recessive disorder has an estimated carrier frequency that varies between 0.07 and 0.13, thus representing one of the most common genetically determined metabolic disorders. The identification of the hemochromatosis mutations has encouraged efforts to investigate other conditions with iron overload for a putative interaction with t

Hepcidin and disorders of iron metabolism.

The hepatic peptide hormone hepcidin is the principal regulator of iron absorption and its tissue distribution. Pathologically increased hepcidin concentrations cause or contribute to iron-restrictive anemias including anemias associated with inflammation, chronic kidney disease and some cancers. Hepcidin deficiency results in iron overload in hereditary hemochromatosis and ineffective erythropoiesis. The hepcidin-ferroportin axis is the principal regulator of extracellular iron homeostasis in health and disease, and is a promising target for the diagnosis and treatment of iron disorders and a

[Treatment of disorders of iron metabolism].

When prescribing oral iron preparations, consideration must be given in particular to the question of bioavailability. In this connection, ferrous sulfate preparations are to be preferred. The simultaneous ingestion of tea, coffee or dairy products reduces the absorption of iron, while ascorbic acid increases it. Gastrointestinal side effects of oral iron substitution are usually dose-dependent. If absorption or mobilization disorders, gastrointestinal diseases or signs of intolerance to oral administration are present, iron must be administered via the parenteral route. To avoid toxicity, acc

Brain iron metabolism and neurodegenerative disorders.

Iron, an essential element for central nervous system (CNS) function, has frequently been found to accumulate in brain regions that undergo degeneration in neurological diseases such as Alzheimer disease, Parkinson disease, Friedreich ataxia and other disorders. However, the precise role of iron in the cause of many neurodegenerative diseases is unclear. To assist in understanding the potential importance of iron in CNS disease, this review summarizes the present knowledge in the areas of CNS iron metabolism, homeostasis and disregulation of iron balance caused by mutations in genes encoding p
