Monitoring of brainstem structures is requested of the neurophysiologist for both intra-axial and extra-axial surgeries of the posterior fossa. A variety of techniques to include upper extremities somatosensory evoked potentials, short latency auditory evoked potentials, spontaneous and evoked electromyographic activity, and recordings from the cochlea and the eighth nerve are available. The indication, implementation, and interpretation of each of these modalities are delineated. The efficacy and utility of these techniques are also discussed.

There has been a tremendous growth in the use of evoked potential techniques in the orthopedic and neurosurgical operating rooms. The popularity of evoked potential monitoring has evolved from the surgeon's need to know the impact of surgical manipulations on the central nervous system. It is safe and practical to monitor the electrophysiological function of the cortex, brainstem, cranial nerves, and motor and sensory pathways in the spinal cord utilizing evoked potential techniques. This paper presents a review of these techniques.

The purpose of intraoperative monitoring is to preserve function and prevent injury to the nervous system at a time when clinical examination is not possible. Cranial nerves are delicate structures and are susceptible to damage by mechanical trauma or ischemia during intracranial and extracranial surgery. A number of reliable electrodiagnostic techniques, including nerve conduction studies, electromyography, and the recording of evoked potentials have been adapted to the study of cranial nerve function during surgery. A growing body of evidence supports the utility of intraoperative monitoring

Successful parathyroidectomy for primary hyperparathyroidism has traditionally consisted of bilateral neck exploration with identification of all parathyroid glands and excision of grossly abnormal glands. Recently it has been reported that preoperative localization combined with intraoperative monitoring of parathyroid hormone enables performing directed unilateral cervical explorations while maintaining similar cure rates. This new surgical approach, feasible under local anesthesia as an outpatient procedure, decreases operative time, improves cosmetic results and may reduce morbidity. We re

A variety of benign and malignant neoplasms occur in the superior cervical neck, parapharyngeal space or the infratemporal fossa. The surgical resection of these lesions may result in postoperative iatrogenic injury to the vagus nerve with associated dysfunctional swallowing and airway protection. Anatomic and functional preservation of this critical cranial nerve will contribute to a favorable surgical outcome. Fourteen patients with tumors of the cervical neck or adjacent skull base underwent intraoperative vagal nerve monitoring in an attempt to preserve neural integrity following tumor rem

Visual evoked potential (VEP) is recorded from the back of the head, which is elicited by retinal stimulation transmitted through optic nerve, optic chiasm, optic tract lateral geniculate body, optic radiation and finally cortical visual area. VEP monitoring did not prevail since 1990s because marked intra-individual difference and instability of VEP recording limited the clinical usefulness under inhalation anesthetic management and techniques of VEP monitoring at the time. However, recent advances in techniques including a new light-stimulating device consisting of high-luminosity LEDs and i

Neurophysiologic intraoperative monitoring, using somatosensory, brainstem auditory, and visual evoked potentials, transcranial electric motor stimulation, and electromyography, is typically used during complex surgeries involving the motor and sensory cortex, brainstem, cranial nerves, spinal cord, nerve root, peripheral roots, brachial plexus, lumbar plexus, and peripheral nerves. The particular type of surgery and the neurologic structures at risk determine the type of monitoring chosen. Although many methods are the same in adult and pediatric patients, some differences in the pediatric po

The electrocardiogram (ECG) is used as a standard monitoring method during anesthesia and operation. But during the operation of severely burnt patients, the electrodes for ECG cannot be placed on the ideal points for the standard limb leads. We tried the indirect monitoring of the ECG. We placed the electrodes on the sheet over the operating table, and connected the patient and the electrodes with water. By this way the ECG similar to the standard limb leads could be recorded. This method is useful for the patients, with such diseases as severe burn, severe atopic dermatitis and epidermolysis

Intraoperative facial nerve monitoring is performed by electromyographic recording of motor action potentials generated by facial nerve stimulation or mechanical damage of the facial nerve, and helps the surgeon to localize the course of this nerve, reduce the possibility of its injury during surgery, and assure its anatomical and functional integrity. We report our experience on the intraoperative facial nerve monitoring in 35 patients, ten of which belong to absolute indications for monitoring, and to relative indications the remaining cases. In every case, we used the Nerve Integrity Monito

Children, like adults, are at risk for neurological injury during a variety of surgical procedures. Intraoperative electrophysiological monitoring (IOM) provides information about the functional integrity of the nervous system during surgery. This information may determine the mechanism of injury and prevent damage by identifying nerves and detecting dysfunction at a reversible stage. Electrophysiological techniques may also help direct therapy by improving injury localization, detect the presence or absence of axonal integrity in peripheral nerve lesions, and identify rootlets with the greate

Intraoperative nerve monitoring has become common for surgical procedures in which cranial or peripheral nerves may be compromised. Intraoperative monitoring of recurrent laryngeal nerve function can be accomplished by recording electromyographic activity from fine-wire electrodes placed in the vocalis muscle. The technique and instrumentation for this are adapted from those used in intraoperative facial nerve monitoring for acoustic neuroma excision. Specifically, a commercially available instrument, the XOMED-NIM provides the capability for monitoring the vocalis muscle electromyogram by mea

During microvascular decompression(MVD)for hemifacial spasm(HFS), trigeminal neuralgia(TN), and glossopharyngeal neuralgia(GPN), brainstem auditory-evoked potential monitoring is widely used to preserve hearing function. In MVD for HFS, abnormal muscle response monitoring is useful for identifying the offending vessels compressing the facial nerve and confirming the completion of decompression intraoperatively. The amplitude of facial motor-evoked potential by transcranial electrical stimulation in the orbicularis oculi muscle is reported to decrease after completing MVD. The Z-L response(ZLR)

Anatomic preservation of the facial nerve, with maximal facial function, is one of the goals of acoustic neuroma surgery. Application of electrophysiologic monitoring techniques is useful in achieving this goal. Preoperative electromyography and nerve conduction studies provide important prognostic information for preservation of the nerve and postoperative function. Intraoperative electromyography alerts the surgeon to facial nerve proximity and potential injury. Direct nerve stimulation is utilized to confirm the location and integrity of the nerve. Matched-pair analysis of two groups of pat

Intraoperative measurements are valuable tools in cochlear implant (CI) surgery, enabling the determination of implant functionality while the patient is still in the operating room. Standard measurements include impedances for stimulating and non-stimulating contacts, triggering of the stapedial reflex with or without estimating the threshold, and assessing cochlear nerve function by measuring electrically evoked compound action potentials. Additional measurements can also be performed. For example, electrocochleography can be used to observe and help preserve residual hearing in patients. Us

Necessary nonobstetric surgical procedures should not be withheld from pregnant women for fear of risks to the women and their pregnancies; however, careful preoperative planning should be undertaken to mitigate risks that may be present. Fetal monitoring recommendations will be dependent on the woman's preferences, gestational age of the pregnancy, and situational-specific risks (including anticipated risk of cardiovascular instability). Some fetal heart rate changes (lower baseline, less variability) can be anticipated, depending on anesthetic agents utilized during the procedure, and should

BACKGROUND: Vagus nerve stimulation is a palliative treatment for patients with refractory epilepsy; however, the misplacement of electrodes may cause complications and thus needs to be avoided.
METHODS: We herein report an intraoperative monitoring technique to prevent the misplacement of electrodes. Endotracheal tube electrodes were inserted to record electromyographic activity from the vocal cords and identify the vagus nerve. Electromyography electrodes were placed on the sternomastoid muscle, sternohyoid muscle, geniohyoid muscle, and trapezius muscle to record muscle activities innervat
