BACKGROUND: Because Myroxylon pereirae (MP), or balsam of Peru, is nowadays almost not used "as such," and fragrance mix 1 (FM1) apparently is more sensitive in detecting fragrance allergy, the usefulness of testing MP in baseline series was recently questioned.
OBJECTIVES: Identification of the number of clinically relevant patch test reactions to MP not detected by FM1.

The Myroxylon pereirae resin (MP; balsam of Peru) is a natural resin used in the local treatment of burns and wounds. M. pereirae extracts and distillates are very often contained in a wide range of cosmetic products and causes frequently allergic contact dermatitis - to the extent of being considered an allergy marker to perfumes. Two patients presented with cheilitis and perioral dermatitis. Patch testing revealed positive reactions to both Myroxylon pereirae (MP; formerly known as Balsam of Peru) and Lucas Papaw Ointment (LPO; Lucas Papaw Remedies, Bisbane, Qld, Australia). MP was not listed as an ingredient in LPO, however the manufacturer confirmed that a small amount of MP was present in LPO and we believe this to be the cause of the patients' dermatitis.

Because of their widespread use, fragrances are among the most common causes of contact allergic dermatitis, second only to nickel.
