Cryptococcosis in the Northern Territory.

This report represents a review of cryptococcosis in the Northern Territory from 1957 to 1975. There were 26 cases over a 19-year period; 25 of these were in full-blooded Aborigines. The disease occurred throughout the rural areas of the Territory as isolated cases. There were 24 cases of cryptococcal meningitis and only two with solitary lung involvement. The overall mortality was 50%. In all five untreated cases the disease was fatal. There were eight deaths among the 20 patients receiving chemotherapy, a mortality of 40%. Lung resection was performed in six cases of localized pulmonary cryp

Melioidosis in the Northern Territory of Australia.

Melioidosis is being diagnosed with increasing frequency in the northern part of the Northern Territory, but the mortality rate remains high in the acute septicaemic form of the disease largely because of associated chronic debilitating illnesses. This paper reviews epidemiological and clinical features of human melioidosis in 37 cases seen between 1960 and 1979. Infection with Pseudomonas pseudomallei is most often contracted during the wet season by persons who have regular contact with soil or ground water, probably through pre-existing skin lesions or penetrating wounds and, occasionally,

Crocodile attacks in the Northern Territory of Australia.

OBJECTIVE: To examine crocodile attacks in the Northern Territory with particular reference to risk factors, range of injuries, microorganisms isolated from wounds, and surgical management; and to make recommendations for optimal treatment. DESIGN AND SETTING: The case notes of patients treated at the Royal Darwin Hospital within the last decade were reviewed retrospectively. Autopsy and newspaper reports for the same period were also reviewed. RESULTS: There were 16 reported crocodile attacks in Northern Territory waters from June 1981 to June 1991. Four of these were fatal. Most attacks re

Hookworm in the Northern Territory: down but not out.

OBJECTIVES: To determine the prevalence and trends of human hookworm infection (HWI) in the Northern Territory over the past 10 years, and to assess the influence of the community children's deworming program (CCDP). DESIGN, PATIENTS AND SETTING: A retrospective observational analysis of consecutive microbiologically confirmed cases of HWI in patients diagnosed at NT government health care facilities and the main private laboratory servicing the NT between January 2002 and July 2012. MAIN OUTCOME MEASURES: Annual prevalence of HWI (2002-2011); age, sex, Indigenous status, residence, haemoglo

Prevalence of mental illness in Northern Territory prisoners.

BackgroundPrisoners experience disproportionately high rates of mental illness. The incarceration rate in the Northern Territory (NT) is five times the national average but there is no existing research regarding the prevalence of mental disorder in this population.ObjectivesThis cross-sectional observational study aimed to examine the prevalence of mental illness and substance use disorders among NT prisoners.Methods310 adult prisoner files were analysed, using four electronic medical record databases which captured a person's lifetime contact with healthcare services in the NT. Treponemal infection in the Australian Northern Territory aborigines.

For many years, a moderate to high prevalence of positive reactions to standard tests for syphilis (STS) has been found in sera from the aboriginal inhabitants of the Northern Territory of Australia. In the survey reported here, people living in 18 areas of the Northern Territory were sampled to determine if the positive STS results were due to treponemal infection and, if so, to what extent this was endemic syphilis, yaws, or venereal syphilis. The results of the Treponema pallidum immobilization test were taken to indicate the presence or absence of treponemal disease in the subject. Clinica

Survey for hydatidosis in cattle bred in the northern region of the Northern Territory of Australia.

OBJECTIVE: To determine whether hydatidosis occurs in cattle bred in the northern region of the Northern Territory. DESIGN: A survey utilising two stage sampling was designed to provide 95% confidence of detecting hydatids in cattle with a herd prevalence of 10% and animal prevalence of 10%. PROCEDURE: For a 1 year abattoir killing season, lesions from the liver and/or lung were collected from 4348 cattle being slaughtered at a Northern Territory abattoir and examined for hydatid cysts. The origin of cattle was established through identification of the brand. RESULTS: Sample sizes of 41 or

Neurosyphilis in the Northern Territory of Australia: a clinical guideline.

BACKGROUND: The Northern Territory (NT) of Australia is currently experiencing a syphilis epidemic. Neurosyphilis is commonly considered in the differential diagnosis for patients presenting with neurologic conditions such as dementia and stroke in the NT. AIMS: To explore the local epidemiologic, diagnostic and treatment complexities of neurosyphilis in the NT and produce a guideline for clinical practice. METHODS: A database search was undertaken and local and global neurosyphilis guidelines were analysed. A guideline was created based on findings of the critical review and consultation wi

A retrospective search for lyssavirus in humans in the Northern Territory.

BACKGROUND: Following the 1996 discovery of a rabies-like lyssavirus in Australian flying foxes, it was unclear whether this was a new epizootic or an unrecognised, previously existing disease. OBJECTIVE: To review cases of unexplained encephalitis in the Northern Territory (NT) to test available clinical specimens for lyssavirus and survey the use of diagnostic tests by clinicians. METHODS: The NT hospital morbidity database was searched from January 1992 to September 1996 for all Royal Darwin Hospital (RDH) cases with an ICD-9 code encompassing encephalitis or viral meningitis. Final diagn

An unusual presentation of secondary syphilis in the Northern Territory.

OBJECTIVE: To present a case which demonstrates the unusual clinical features of secondary syphilis that may be encountered in tropical Australia. CLINICAL FEATURES: A syphilitic aetiology was initially missed in a Caucasian female presenting with a rare form of syphilis, "lues maligna", characterised by nodulo-ulcerative skin lesions, fever, meningism and a relapsing course. CONCLUSION: Secondary syphilis is usually manifest in the Northern Territory by a characteristic palmo-plantar psoriasiform eruption with variable involvement of skin in other body areas. The disease is most commonly se

Equine onchocerciasis in Queensland and the Northern Territory of Australia.

Investigations were conducted on the taxonomy, distribution in the carcase, pathology and transmission of Onchocerca spp. in equids from Queensland and the Northern Territory. Examination of small groups of horses and ponies revealed high infection rates with O. cervicalis, while lesser numbers were infected with O. gutturosa. O. reticulata was not found. Neither of the Australian species is likely to be of economic importance to the horsemeat industry. The findings support the belief that O. cervicalis is a pre-disposing factor in the aetiology of equine nuchal disease, most commonly seen cli

Burkholderia pseudomallei Genotype Distribution in the Northern Territory, Australia.

Melioidosis is a tropical disease of high mortality caused by the environmental bacterium, Burkholderia pseudomallei. We have collected clinical isolates from the highly endemic Northern Territory of Australia routinely since 1989, and animal and environmental B. pseudomallei isolates since 1991. Here we provide a complete record of all B. pseudomallei multilocus sequence types (STs) found in the Northern Territory to date, and distribution maps of the eight most common environmental STs. We observed surprisingly restricted geographic distributions of STs, which is contrary to previous reports

Case report: Sporotrichosis from the Northern Territory of Australia.

We report three cases of lymphocutaneous infection caused by the thermally dimorphic fungus, Sporothrix schenckii from Australia's tropical Northern Territory. Two cases were acquired locally, making them the first to be reported from this region. All three cases presented with ulceration in the limb; however, the classical sporotrichoid spread was present only in the first two cases. Their occurrence within several weeks of each other was suggestive of a common source of environmental contamination such as hay used as garden mulch. Diagnoses were delayed in each case, with each patient having
