Orbital implants.

Several aspects of orbital implants are reviewed. Hydroxyapatite has become one of the most commonly used implants for anophthalmic patients and has many of the characteristics desirable in an implant, although reports of significant complication rates are noted. Recent editorials acknowledge the advancement that hydroxyapatite has given in treating anophthalmic patients but add a note of caution in its extensive use. It may be reasonable to limit the use of hydroxyapatite to patients who are highly motivated. Other recent advances in surgical technique, evaluation, and treatment of complicati…

Patient-Specific Orbital Implants Vs. Pre-Formed Implants for Internal Orbital Reconstruction.

PURPOSE: To compare the outcome of orbital blowout fracture repair by means of pre-formed porous-polyethylene titanium implants (PFI) vs patient-specific porous-polyethylene implants (PSI). METHODS: Retrospective cohort study. Baseline characteristics, ophthalmic examination results, ocular motility, fracture type, the timing of surgery, implant type, and final relative enophthalmos of all patients operated on for blow-out fractures in a single center were collected and analyzed. RESULTS: Twenty-seven patients (mean age 39 years, 9 females) were enrolled. Sixteen underwent fracture repair wi

Complications of supramid orbital implants.

PURPOSE: To determine the incidence and risk factors of complications associated with Supramid orbital implants. METHODS: A retrospective chart review was performed to document complications in a series of 41 patients receiving Supramid implants during orbital reconstruction or fracture repair. Implant size, duration of time between trauma and implant insertion, and history of prior orbital surgery were examined as possible risk factors for the development of complications. RESULTS: Four patients had hemorrhage within the implant capsule. An orbital abscess developed in a single patient. All…

Orbital implants for fracture repair.

PURPOSE OF REVIEW: To review the latest advancements in implant materials for orbital fracture repair, including analysis of the last year in research. RECENT FINDINGS: Advances in the manufacture of highly biocompatible alloplastic materials offer an alternative to traditional autologous implants for orbital fracture repair. The emergence of the composite implant offers the stability and strength of traditional titanium mesh, while the porous polyethylene shell allows for greater fibrovascular integration and a potential for decreased risk of postoperative complications. Development of copol

PURPOSE: To present the authors experience with the nylon foil (Supramid) implant as a safe and effective method to repair pediatric orbital wall fractures. METHODS: A retrospective chart review of all pediatric patients (≤18 years) that underwent orbital wall fracture repair with an unsecured 0.4-mm Supramid implant between 2007 and 2010. Outcome variables were diplopia and surgical complications. This study was carried out with IRB approval. RESULTS: A total of 59 orbits in 57 patients underwent orbital fracture repair using solely the 0.4-mm Supramid implant that were included in this ret

Alloplastic implants for orbital wall reconstruction.

Nonabsorbable alloplastic implants for orbital wall reconstruction have been widely accepted by surgeons because of their ready availability, stability, and biocompatability. Many complications have arisen with this class of implants because the lack of host tissue integration allows for implant migration, implant extrusion, recurrent hemorrhage, and infection. Porous polyethylene implants provide a welcome alternative as they have the unique properly of supporting tissue ingrowth in vivo. Their semirigid structure provides structural stability when used around the orbit, and their malleabilit

Management of exposed hydroxyapatite orbital implants.

BACKGROUND: Porous coralline hydroxyapatite orbital implants have gained significant popularity for use in anophthalmic sockets. Several reports have noted increased rates of early implant exposure. METHODS: Six patients with exposure complications of primary hydroxyapatite implants after enucleation or evisceration were studied. RESULTS: The hydroxyapatite implant exposures occurred 2 to 8 weeks after implantation. Magnetic resonance imaging scans with gadolinium enhancement obtained in two patients demonstrated a "cold" zone anteriorly with failure of fibrovascular ingrowth in the region o…

Patient-specific implants in orbital reconstruction.

PURPOSE OF REVIEW: Advances in the use of patient-specific implants (PSIs) and virtual surgical planning (VSP) for reconstruction of primary and secondary traumatic orbital defects are explored. RECENT FINDINGS: PSIs and VSP are emerging technologies that promise to make complex orbital reconstructions safer and more predictable for patients. Recent studies highlight principles of implant design, the novel use of multiunit implant constructs, and utility of intraoperative imaging adjuncts to achieve favorable outcomes. SUMMARY: This article summarizes recent developments in PSIs for orbital

Computer-assisted design of orbital implants.

PURPOSE: The purpose of this study is to report on the use of a computer-assisted design (CAD) system for predictable preoperative planning of orbital implant surgery (determination of the optimal number and position of orbital implants). MATERIALS AND METHODS: Preoperative computed tomographic data were processed by interactive software for predictable surgical planning of orbital implant placement. Reformatted images from axial scans were used to analyze the structure of orbital bone and to plan the number of implants to place and the sites in which to place them. RESULTS: Surgeries to cor…

Patient-specific implants for orbital fracture surgery.

Orbital wall fractures are among the most common maxillofacial injuries and require precise reconstruction to restore orbital volume, prevent diplopia, and avoid long-term sequelae such as enophthalmos or facial asymmetry. Traditional reconstruction techniques have relied on autologous grafts and preformed alloplastic implants; however, these approaches are limited by intraoperative variability, imprecise contour restoration, and increased operative time. Over the past decade, the emergence of patient-specific implants (PSIs), designed through advanced three-dimensional (3D) imaging, computer-

Autologous pericranium grafts for large orbital implants.

INTRODUCTION: Insufficient orbital volume in an anophthalmic socket is a major problem for the placement of an ocular prosthesis. This study reports the outcomes of the use of autologous pericranium graft in association with a large primary or secondary orbital implant in patients with a contracted socket and large orbital volume deficit. METHODS: This was a retrospective single-institution study. Participants were 13 patients with contracted socket, volume deficit, and insufficient conjunctiva to cover the new implant divided into two groups, A (n = 3) and B (n = 10), according to the baseli
