There is a high rate of use of antimicrobial drugs for otitis media in children. This article reviews the diagnostic considerations for acute otitis media. An extensive review of literature on this subject has been carried out in order to address the issues of indications, choice, appropriate doses of antimicrobial agents and the duration for which they should be used. It is important to distinguish acute otitis media from otitis media with effusion because antibiotics are seldom indicated for the latter condition. Oral amoxicillin remains first-line therapy for uncomplicated acute otitis medi…

Otitis media is the most common respiratory tract infection of infancy and early childhood that is managed with antibacterial agents. A bacterial pathogen is isolated from the middle ear fluids of approximately two-thirds of children with acute otitis media; S. pneumoniae is the leading bacterial pathogen followed by nontypable strains of H. influenzae and M. catarrhalis. Clearance of bacteria from middle ear fluid without use of antibacterial drugs is evident in studies in which a placebo is used. Whereas pneumococci continued to be isolated from middle ear fluids when the infection was not t…

Earache, a common symptom in children, causes many parents to seek medical attention. Aside from trauma and the discomfort that often accompanies viral infections of the upper respiratory tract, acute otitis media with effusion is the commonest cause of otalgia in infants and children. Proper management requires a team effort between the physician and the child's parents or caretaker. The physician must transmit to the parents a concise but thorough overview of the problem and a plan for management. This should include information on the pathophysiology of ear disease, its incidence, therapy a…

Dysfunction of the eustachian tube is the underlying problem in all types of otitis media. Systemic antibiotics and careful follow-up are the treatment of choice for acute suppurative otitis media, whereas topical antibiotics with saline irrigation are used in patients with a chronic condition. Myringotomy and tube insertion may be required in patients with secretory otitis media that does not undergo spontaneous resolution.

Otitis media (OM) or middle ear inflammation is a spectrum of diseases, including acute otitis media (AOM), otitis media with effusion (OME; 'glue ear') and chronic suppurative otitis media (CSOM). OM is among the most common diseases in young children worldwide. Although OM may resolve spontaneously without complications, it can be associated with hearing loss and life-long sequelae. In developing countries, CSOM is a leading cause of hearing loss. OM can be of bacterial or viral origin; during 'colds', viruses can ascend through the Eustachian tube to the middle ear and pave the way for bact…

Otitis media represents a broad spectrum of disease, which include acute otitis media and otitis media with effusion. As immunization with the pneumococcal conjugate vaccine has become more widespread, the microbiological landscape of otitis media has changed, which affects the treatment options facing clinicians worldwide. This review discusses the diagnosis and medical management of acute and chronic suppurative otitis media, the changes noted over the past decade, and briefly expounds on the surgical management of their severe complications.

Despite extensive research on various aspects of otitis media, this disease remains an important health care problem of childhood. The rapid increase in resistance to penicillin and several other drugs among strains of Streptococcus pneumoniae will most likely have a great impact on the antibiotic treatment of acute otitis media. The increasing antimicrobial resistance has once again brought up the question of whether antibiotics are really needed in the treatment of acute otitis media. Further, the resistance problem underscores the great need for effective methods to prevent otitis media. In…

The spectrum of otitis media includes acute and chronic forms, each of which can be either suppurative of nonsuppurative. In the usual clinical setting distinctions between these several forms can be difficult. Determination of accurate incidence figures is impeded by the unavailability of universally accepted diagnostic criteria. Risk factors include season of the year, genetic factors, race, preceding respiratory tract infections, cleft palate, and others. The effect of household size and allergy are uncertain. The most common infecting organisms are Streptococcus pneumoniae and Hemophilus i…

There are 2 different kinds of chronic otitis media: Otitis media chronica mesotympanalis and otitis media chronica epitympanalis (cholesteatoma). The incidence of chronic otitis media as reported in literature differs in a wide range. The incidence rates vary between 0.45 and 46%. Both, otitis media chronica mesotympanalis and cholesteatoma, lead to eardrum perforation due to lengthy and recurring inflammations. Furthermore, chronic otitis media is characterized by frequently recurring otorrhea and conductive hearing loss.

This overview of the relationship between the nasopharyngeal tonsil and otitis media will review three important concepts: (1) Adenoid inflammation leads to inflammatory obstruction of the Eustachian tube; (2) early colonization of the adenoid with the three major bacterial pathogens of otitis media is the most important factor in the early pathogenesis of otitis media; (3) the local immune system in the adenoid particularly specific secretory IgA directed against both viruses and bacterial pathogens are probably genetically controlled and represent the immunological factor in protecting the h…

Eosinophilic otitis media (EOM) is a rare, treatment-resistant middle ear condition characterised by eosinophilic secretions and a risk of irreversible hearing loss. The condition is often seen in patients with asthma and chronic rhinosinusitis with nasal polyps (CRSwNP) and is driven by type-2 inflammation. We describe two patients in this case with severe EOM and concomitant asthma and CRSwNP, in whom conventional treatment had limited efficacy. Treatment with the IL-5 inhibitor mepolizumab resulted in marked improvement in ear symptoms, improved hearing, and reduced need for systemic steroi…

Chronic otitis media, defined as persistent or recurrent inflammation of the middle ear mucosa, is a frequent entity in primary and secondary care. This disease causes otorrhea and hearing loss that significantly affect the quality of life of patients. In recent years, there have been new developments in its diagnosis, prevention and treatment. In this last aspect, pharmacological treatment, surgical techniques and hearing rehabilitation through implants have shown significant advances. On the other hand, chronic otitis media complications continue to occur in emergency services and have to be…

BACKGROUND: Eosinophilic otitis media (EOM) is a rare middle ear disease that may closely resemble therapy-resistant otitis media with effusion (OME). The diagnosis is made if eosinophil-rich fluid is present with two or more of the following minor criteria: (a) history of nasal polyps or, (b) bronchial asthma, with (c) viscous fluid in the middle-ear, or (d) conventional otitis media treatment has no effect.
CASE DESCRIPTION: A 70-year-old male with a history of the ASA triad (bronchial asthma, nasal/ethmoidal polyposis and aspirin intolerance) presented with progressive mixed hearing loss. …
