Pancreatic Neuroendocrine Neoplasms.

Pancreatic neuroendocrine neoplasms are heterogeneous tumors whose incidence has risen with advances in, and increased use of, cross sectional and endoscopic imaging. They range from well differentiated, often indolent neuroendocrine tumors to aggressive neuroendocrine carcinomas. Most PNENs are non-functional and detected incidentally, whereas functional tumors present with hormone related clinical syndromes. Accurate diagnosis and staging rely on multimodality imaging. Pancreatic protocol CT and MRI remain first-line tools for anatomic assessment, providing information on tumor morphology, v

[Pancreatic cystic neoplasms].

Pancreatic cystic lesions represent a challenging heterogeneous entity with a potential risk of malignant transformation. The diagnostics include in particular medical history taking with collection of relevant clinical information and high-resolution imaging, preferably using magnetic resonance imaging (MRI) with MR cholangiopancreatography (MRCP) and/or endoscopic ultrasonography. A differentiation between different cystic entities and identification of risk factors are crucial for making appropriate treatment decisions. Only a small proportion of pancreatic cystic neoplasms require surgery.

Pancreatic cystic endocrine neoplasms.

A rare case of cystic pancreatic endocrine tumor is presented, and the literature is reviewed. The patient was initially misdiagnosed as having a pancreatic pseudocyst, and that condition was managed accordingly. Persistence of the cystic lesion and reoperation led to the correct diagnosis and management. The neoplasm stained positive for glucagon and pancreatic polypeptide, but there were no clinical abnormalities that suggested hyperfunction. All cystic lesions of the pancreas should undergo biopsy at operation, to avoid an erroneous diagnosis of benign pseudocyst. Neoplastic lesions should

Pancreatic Cystic Neoplasms: An Update.

The incidence of pancreatic cystic neoplasms is rising, in part from detection through the increasing use of high-resolution cross-sectional imaging techniques. Initial diagnosis is generally based on imaging characteristics identified on computed tomography and/or MRI. Endoscopic ultrasound provides further imaging characterization and also enables fluid aspiration and analysis to additionally aid differentiation. The general approach to these lesions includes surgical intervention and/or surveillance imaging. Taking into account diverse presentations, varying malignant potential, and the unc

Pancreatic involvement by plasma cell neoplasms.

INTRODUCTION: Pancreatic involvement by plasma cell neoplasms is an extremely rare event, with only 50 cases described in the literature. They can present as a primary solitary extramedullary plasmacytoma or plasmacytoma secondary to a plasma cell myeloma. Clinical manifestations are due to the presence of a pancreatic mass usually in the pancreas head, which causes extra-biliary obstruction and abdominal pain. METHODS: Abdominal imaging including CT scan or endoscopic ultrasound with fine-needle aspiration tissue sampling is essential for the initial diagnostic procedure. However, immunohist

Imaging of non-ductal pancreatic neoplasms.

A vast majority of pancreatic tumors are pancreatic ductal adenocarcinomas that portend a grave prognosis. Non-ductal neoplasms are a small category of pancreatic tumors with different clinical, imaging, and pathological features and a better prognosis compared to adenocarcinoma of the pancreas. Most of the non-ductal neoplasms are surgically resectable when diagnosed early, while a few of them require expectant management. Knowledge of this spectrum of lesions can aid in providing accurate diagnoses and help with radio-pathological correlation. In this review, we wish to present radiological,

Pancreatic Cystic Neoplasms: Diagnosis and Management.

Pancreatic cancer is one of the most lethal cancers, largely related to the difficulties with early detection, as it typically presents in later stages. Pancreatic cystic neoplasms (PCN) are commonly diagnosed as incidental findings on routine imaging. PCN is becoming more frequently detected with the increasing ease and frequency of obtaining cross-sectional images. Certain subtypes of pancreatic cysts have the potential to progress to malignancy, and therefore, clinicians are tasked with creating a patient-centered management plan. The decision of whether to undergo surgical resection or int

Management Algorithms for Pancreatic Cystic Neoplasms.

CONTEXT.—: The detection of pancreatic cystic neoplasms (PCNs) has increased owing to the advancement and widespread use of imaging modalities, resulting in differences between past and current management methods for PCNs, including intraductal papillary mucinous neoplasms (IPMNs). Therefore, clinicians should accurately diagnose and determine appropriate treatment strategies. However, previously published treatment guidelines for IPMNs present different indications for treatment. OBJECTIVE.—: To review the current status of PCNs, including epidemiologic change, malignancy risk, and factors f

Surgical management of pancreatic neuroendocrine neoplasms.

Pancreatic neuroendocrine neoplasms are relatively rare entities, representing approximately 1% to 2% of all pancreatic tumors. Owing to their rarity as well as their relatively indolent natural history, treatment approaches are not yet standardized. A formal pancreatic resection is usually mandatory for large and localized sporadic pancreatic tumors or in the presence of symptoms. However, in small and asymptomatic lesions, a conservative approach consisting in a careful wait-and-see policy is going to appear as more appropriate, particularly when, to remove the lesion, an aggressive surgical

[Pancreatic neoplasms and risk of thrombosis].

Among all cancers of the pancreas, the liver and the bile ducts with icterus terminal lung embolism ist seldomer then among the same cancers without icterus. The risk of terminal lung embolism is among all tumours of the abdominal region higher than among all tumours of the thorax region, because the compression of the vena cava inferior increases the risk of thrombosis with lung embolism. The prevalence of adenocarcinoma among all cases with lung embolism is caused by the fact, that cancers of the abdomen are more frequent adenocarcinomas then all other tumours.

Primary pancreatic cystic neoplasms revisited. Part III. Intraductal papillary mucinous neoplasms.

Intraductal papillary mucinous neoplasms (IPMNs) represent about 25% of all primary pancreatic cystic neoplasms and are increasingly recognized during the last two decades. They are characterized by intraductal proliferation of neoplastic mucinous cells forming papillary projections into the pancreatic ductal system, which is typically dilated and contains globules of mucus. IPMNs may be multifocal and have malignant potential. Modern imaging is essential in establishing preoperative diagnosis and in differentiating different subtypes of IPMNs (i.e., main-duct vs. branch-type disease). Endosco

Pancreatic Mucinous Cystic Neoplasm Communicating with Main Pancreatic Duct: An Unrecognized Presentation of Pancreatic Mucinous Neoplasm?

Mucinous cystic neoplasms (MCNs) and intraductal papillary mucinous neoplasms (IPMNs) are two well recognized entities of precursor cystic lesions of pancreatic duct adenocarcinoma. The characteristic features of MCNs are the lined mucinous epithelium with underlying ovarian-type stroma, but without communication with the ducts, while that for IPMNs are the communication with the ducts but without the underlying ovarian-type stroma. Here we report a case of MCN communicating with the main pancreatic duct in a 68-year-old woman. The initial radiographic diagnosis was pancreatic IPMN with main p

Discordant risk factors between pancreatic neuroendocrine neoplasms and pancreatic ductal adenocarcinoma.

Pancreatic neuroendocrine neoplasm (panNEN) is a rare malignancy and the second most common type of pancreatic cancer after pancreatic ductal adenocarcinoma (PDAC), but its etiology is poorly understood. We investigated whether the risk factors of panNEN are concordant with those known for PDAC. We performed the largest case-control study to date on panNENs, comprising 927 sporadic nonfunctional panNEN cases and 1807 frequency-matched controls, using data from the Mayo Clinic Biospecimen Resource for Pancreas Research. We assessed associations for obesity, first-degree family history of pancre

Differentiation Versus Grade for Pancreatic Neuroendocrine Neoplasms.

CONTEXT.—: Pancreatic neuroendocrine neoplasia (panNEN) is a tumor disease with distinctive morphology and often poses diagnostic challenges. OBJECTIVE.—: To present and discuss the current diagnostic criteria of PanNEN. DATA SOURCES.—: PanNENs are classified by the World Health Organization (WHO) criteria into well-differentiated neuroendocrine tumor (panNET) and poorly differentiated neuroendocrine carcinoma (panNEC) of large or small cell type. panNETs are graded as G1-G3 on the basis of their proliferation capacity by mitotic count and/or Ki-67 proliferation index. Differentiation and gr

[Pancreatic cystic neoplasms: work-up and surveillance].

Whereas pancreatic masses are often difficult to detect with transabdominal ultrasound, cross-sectional imaging features high sensitivity for the pancreatic tumors. However, increasing availability of magnetic resonance imaging (MRI) has led to a surge in the detection of benign or precancerous pancreatic lesions. The medical history is characteristic only for two entities. A history of acute or chronic pancreatitis favors a benign pseudocyst while weight loss or jaundice are suggestive of malignancy. More challenging is the classification of asymptomatic cysts, which often require extensive w

Duodenum-preserving total pancreatectomy for pancreatic neoplasms.

BACKGROUND/PURPOSE: Total pancreatectomy (TP) is sometimes performed to treat low-grade malignant neoplasms that are spreading to the entire pancreas. However, TP impairs quality of life, due to the resulting loss of pancreatic exocrine and endocrine function, and an organ-preserving procedure should be chosen to minimize the impact of pancreatic dysfunction. Recently, we performed four duodenum-preserving TPs (DPTPs) on patients with low-grade malignant neoplasms of the entire pancreas and we introduce our operative technique and results herein. METHODS: DPTP is performed with the objective
