Perfusion Imaging for the Heart.

The use of myocardial perfusion imaging during a stress cardiac magnetic resonance (CMR) examination for the evaluation of coronary artery disease is now recommended by both US and European guidelines. Several studies have demonstrated high diagnostic accuracy for the detection of hemodynamically significant coronary artery disease. Stress perfusion CMR has been shown to be a noninvasive and cost-effective alternative to guide coronary revascularization.

MR Perfusion Imaging for Gliomas.

Accurate diagnosis and treatment evaluation of patients with gliomas is imperative to make clinical decisions. Multiparametric MR perfusion imaging reveals physiologic features of gliomas that can help classify them according to their histologic and molecular features as well as distinguish them from other neoplastic and nonneoplastic entities. It is also helpful in distinguishing tumor recurrence or progression from radiation necrosis, pseudoprogression, and pseudoresponse, which is difficult with conventional MR imaging. This review provides an update on MR perfusion imaging for the diagnosi

Regional perfusion imaging using pTILT.

PURPOSE: To demonstrate the feasibility that the pseudo-continuous transfer insensitive labeling technique (pTILT) sequence can be modified to achieve regional perfusion imaging by either vessel-encoded or vessel-selective pTILT, similar to the regional perfusion imaging sequences using pseudo-continuous arterial spin labeling (pCASL). MATERIALS AND METHODS: Static phantom experiments were carried out to investigate the selectivity and efficiency of region-specific labeling by pTILT. The ability of the modified pTILT sequences to separate perfusion territories was also examined at two levels

Pharmacologic stress myocardial perfusion imaging.

Pharmacologic stress agents (dipyridamole, adenosine and dobutamine) allow virtually all patients to be safely assessed for ischemic heart disease. These agents have mild but significant side effects, mandating a thorough knowledge of indications, contraindications, side effects and management before their use. Adjunctive exercise improves image quality in vasodilator pharmacologic myocardial perfusion imaging. Diabetics, especially women, have a much higher cardiac event rate than nondiabetics for an equal amount of ischemia. They also have a higher incidence of asymptomatic ischemia. There i

Perfusion imaging of cerebrovascular reserve.

Cerebrovascular reserve reflects the capacity of the brain to maintain adequate blood flow in the face of decreased perfusion pressure. Perfusion imaging, combined with a physiologic or pharmacologic challenge, is a direct method of measuring cerebrovascular reserve. The authors discuss the strengths and drawbacks of each of the methods of cerebrovascular reserve assessment. They review the applications of cerebrovascular reserve testing, particularly in the assessment of stroke risk in the setting of chronic stenosis or occlusion of vessels in the head and neck.

Cardiac Positron Emission Tomography Myocardial Perfusion Imaging: Seeing Beyond Perfusion.

Positron emission tomography (PET) myocardial perfusion imaging (MPI) is a highly accurate noninvasive modality for evaluating coronary artery disease due to its excellent sensitivity and specificity, excellent image quality, efficient imaging protocol, and significantly lower radiation exposure. Its ability to absolutely quantify myocardial blood flow (MBF), MBF-R, and left ventricular ejection fraction-reserve also strengthens its diagnostic and prognostic properties and guides physicians about the justification for coronary revascularization. Presence of coronary artery calcification on the

Magnetic Resonance Imaging of Lung Perfusion.

"Lung perfusion" in the context of imaging conventionally refers to the delivery of blood to the pulmonary capillary bed through the pulmonary arteries originating from the right ventricle required for oxygenation. The most important physiological mechanism in the context of imaging is the so-called hypoxic pulmonary vasoconstriction (HPV, also known as "Euler-Liljestrand-Reflex"), which couples lung perfusion to lung ventilation. In obstructive airway diseases such as asthma, chronic-obstructive pulmonary disease (COPD), cystic fibrosis (CF), and asthma, HPV downregulates pulmonary perfusion

Perfusion Imaging of Traumatic Brain Injury.

The mechanisms for regulating cerebral blood flow (CBF) are highly sensitive to traumatic brain injury (TBI). The perfusion imaging technique may be used to assess CBF and identify perfusion abnormalities following a TBI. Studies have identified CBF disturbances across the injury severity spectrum and correlations with both acute and long-term indices of clinical outcome. Although not yet widely used in the clinical context, this is an important area of ongoing research.

Myocardial perfusion imaging with contrast echocardiography.

Advances in myocardial perfusion imaging have firmly established the use of noninvasive techniques capable of providing useful information over a broad range of diagnostic and therapeutic cardiovascular problems. Evaluating regional myocardial perfusion abnormalities is a cornerstone for the diagnosis of coronary artery disease, risk assessment in those with known disease, and determination of myocardial viability. The clinical use of myocardial perfusion imaging and the current limitations of existing techniques continue to promote the development of new technologies capable of assessing micr

Brain MR perfusion imaging in humans.

PURPOSE: To evaluate any possible advantages of high-concentration gadolinium chelates in the human brain thin-slice MR. MATERIAL AND METHODS: Thirty-six patients were allocated into six groups. Three groups had perfusion imaging performed using 7-mm-thick slices and the other three groups had 4-mm-thick slices. One group who had 7-mm slices and one group of 4-mm slices had 0.5 M gadolinium (20 ml) whilst other groups received the high-concentration gadolinium (1.0 M: 10 ml and 20 ml). The time-intensity curves were analysed for maximum signal reduction and signal-to-noise measurements. RESU

Perfusion imaging in ischemic heart disease.

Myocardial perfusion imaging with thallium-201 has been used extensively for many years in the evaluation of ischemic heart disease. It is a valuable adjunct to exercise treadmill testing and is especially helpful in evaluating cardiac perfusion in patients who have abnormal resting electrocardiograms, who are unable to exercise, and whose clinical history and exercise test results are discordant. Perfusion imaging is useful for both diagnosis and prognosis in patients with coronary artery disease. In patients unable to exercise, the coronary vasodilators dipyridamole and adenosine may be used

MR Perfusion Imaging of the Lung.

Lung perfusion assessment is critical for diagnosing and monitoring a variety of respiratory conditions. MRI perfusion provides a radiation-free technique, making it an ideal choice for longitudinal imaging in younger populations. This review focuses on the techniques and applications of MRI perfusion, including contrast-enhanced (CE) MRI and non-CE methods such as arterial spin labeling (ASL), fourier decomposition (FD), and hyperpolarized 129-Xenon (129-Xe) MRI. ASL leverages endogenous water protons as tracers for a non-invasive measure of lung perfusion, while FD offers simultaneous measur

Myocardial perfusion imaging using contrast echocardiography.

Microbubbles are an excellent intravascular tracer, and both the rate of myocardial opacification (analogous to coronary microvascular perfusion) and contrast intensity (analogous to myocardial blood volume) provide unique insights into myocardial perfusion. A strong evidence base has been accumulated to show comparability with nuclear perfusion imaging and incremental diagnostic and prognostic value relative to wall motion analysis. This technique also provides the possibility to measure myocardial perfusion at the bedside. Despite all of these advantages, the technique is complicated, techni

Perfusion magnetic resonance imaging in psychiatry.

INTRODUCTION: Cerebral perfusion imaging using magnetic resonance imaging (MRI) is widely used in the research and clinical fields to assess the profound changes in blood flow related to ischemic events such as acute stroke, chronic steno-occlusive disease, vasospasm, and abnormal vessel formations from congenital conditions or tumoral neovascularity. With continuing improvements in the precision of MRI-based perfusion techniques, it is increasingly feasible to use this tool in the study of the subtle brain perfusion changes occurring in psychiatric illnesses. This article aims to review the e

Myocardial perfusion imaging with thallium-201.

Over the past 15 yr, numerous clinical studies have validated the use of myocardial perfusion imaging with 201Tl for detection and evaluation of coronary artery disease (CAD). In addition, 201Tl scintigraphy plays a valuable role in the risk stratification of patients with suspected or known CAD to determine prognosis. Typical protocols involve a comparison of stress and rest images to locate regions of myocardial ischemia. Results are comparable from protocols that employ either exercise stress or pharmacologic stress. By combining 201Tl scintigraphic criteria with electrocardiographic (ECG)

Quantitative aspects of myocardial perfusion imaging.

Computer quantitation of myocardial perfusion images has enhanced the detection of thallium perfusion abnormalities compared to visual analysis. Computer analysis is more specific than visual analysis for detection of initial defects and more sensitive for detection of redistribution. Computer analysis is equally good for detecting thallium abnormalities in the distribution of the three major coronary arteries. Measurement of absolute clearance of thallium results in an unacceptable high false-positive rate. However, when clearance in a myocardial segment is compared to the fastest clearing se

Hybrid Assessment of Myocardial Ischemia Using Stress-Only Nuclear Myocardial Perfusion Imaging and Rest Computed Tomography Perfusion Imaging.

BACKGROUND: Sequential assessment using CT coronary angiography (coronary CT) and nuclear myocardial perfusion imaging (MPI) is considered an anatomical and functional evaluation of coronary artery disease (CAD). However, there can be unexpected radiation exposure. Hybrid MPI with stress-only nuclear MPI and rest CT-MPI using coronary CT may contribute to reducing the radiation dose in sequential assessment with nuclear MPI after coronary CT. We analyzed the diagnostic performance and total radiation dose of hybrid MPI for detection of significant CAD compared with sequential assessment using
