Phentolamine continuous infusion in a patient with pheochromocytoma.

PURPOSE: Use of continuous phentolamine infusion therapy for management of serious cardiovascular complications during adrenalectomy for pheochromocytoma is reported. SUMMARY: In preparation for surgical resection of a pheochromocytoma, a 38-year-old woman received outpatient oral therapy with the α-adrenergic-receptor blocker phenoxybenzamine for 25 days with the goal of reducing cardiovascular risks associated with catecholamine surge during surgery. Due to inappropriate dosage adjustment, however, outpatient phenoxybenzamine therapy did not achieve adequate α-adrenergic-receptor blockade; …

[Therapy of primary pulmonary hypertension with phentolamine].

The effect of phentolamine was studied in 5 patients with pulmonary hypertension, of whom 3 had primary pulmonary hypertension and 2 had secondary pulmonary hypertension after pulmonary embolism. They received intravenous phentolamine in doses of 0.5 mg/min to 2 mg/min (i.e. up to a total dose of 27.5 mg in 25 minutes) during diagnostic cardiac catheterization. Under phentolamine infusion only 2 of the 3 patients with primary pulmonary hypertension showed a significant rise in cardiac index, from 1.4 to 2.0 and from 1.8 to 2.2 l/min/m2 respectively, a fall in pulmonary artery mean pressure fro…

The phentolamine test in neurogenic bladder dysfunction.

In an attempt to improve the selection of patients with neurogenic bladder dysfunction likely to benefit from phenoxybenzamine therapy, the effect of intravenous phentolamine (Regitine) on the urinary flow rate was determined in normal and abnormal subjects. A nomogram relating mean flow rate to volume voided was constructed to aid in the interpretation of results. In 16 patients with neurogenic bladder dysfunction it was found that a response of more than 0.8 nomogram units indicates a likelihood of clinical improvement with phenoxybenzamine therapy. The clinical applications of this test are…

Primary pulmonary hypertension treated with oral phentolamine.

A patient with symptomatic primary pulmonary artery hypertension underwent therapeutic trials with parenteral and oral phentolamine. Before drug therapy, he manifested marked increases in pulmonary artery pressure and pulmonary arteriolar resistance during exercise. This hypertensive response to exercise was significantly attenuated by parenteral phentolamine. He also manifested sustained hemodynamic improvement with the same degree of attenuation of exercise-induced increases in pulmonary artery pressure and pulmonary arteriolar resistance 7 months after initiation of therapy with oral phento…

Therapeutic effects of intraarterial phentolamine in "Raynaud's syndrome".

Five patients suffering from "Raynaud's syndrome" were studied as regards the effects of increasing doses of intraarterially administered phentolamine on the blood flow to the digital arteries. Upon angiography they had shown multiple occlusions of their digital arteries. Venous occlusive plethysmography of the forearm, volume pulse amplitude of a finger tip of treated and untreated extremity, respectively, intraarterial pressure in the brachial artery and ECG lead II have been monitored continuously. The blockade of the alpha-adrenoceptors by phentolamine resulted in a favourable effect upon …

Some observations on the gastric effects of phentolamine in rats.

The effects of graded doses of phentolamine on gastric secretion, gastric emptying rate, gastric mucosal mucus content and gastric mucosal lesion incidence were studied in conscious rats 2 h after intramuscular administration. In pylorus-occluded rats, phentolamine (5, 10 or 20 mg/kg) produced dose-dependent decreases in the gastric secretory volume and total acid output. Similar doses of the drug also produced dose-dependent decreases in the gastric emptying rate in animals without pylorus occlusion (intact rats), but did not affect the gastric mucosal mucus content. The incidence of gastric …

[Phentolamine in treatment of acute left ventricular insufficiancies].

Phentolamine (Regitine) at the dose of 0.3 mg/mn behaves as an arterial and, above all venous, vasodilatator agent, resulting in a marked and early lowering of the pulmonary pressures in acute oedema of the lung and in cardiac asthma. It was used alone in 47 attacks of acute severe left ventricular failure with very favourable results in 43 cases, as proved by the rapid improvement of the haemodynamic status and of the aicd-base balance. Under strict observation, tolerance has been excellent. This therapeutic method seems of great interest in the cases of acute pulmonary oedema with a maintain…

Epinephrine-induced vasospasm reversed by phentolamine digital block.

A case of profound digital vasoconstriction caused by the accidental injection of epinephrine from a commercial bee sting kit is reported. One hour later the patient had a cold, painful, blanched finger. A digital block using a 2-mg dose of phentolamine mixed with 2% lidocaine was performed. Thirty minutes after treatment, the finger was pink and warm. The patient reported a marked decrease in pain. Reexamination 12 hours later showed only mild tenderness at the fingertip. No tissue necrosis occurred. One month after injection, there were no apparent sequelae. Although the use of alpha-adrener…

Paradoxical effect of phentolamine on aqueous flow in the rabbit.

PURPOSE: The aim of this study was to assess the effects of acute systemic, nonselective alpha-adrenergic blockade on aqueous flow. METHODS: This study used pentobarbital-anesthetized rabbits (n=7), in which the following parameters were measured: mean arterial pressure, carotid blood flow, heart rate, intraocular pressure (IOP), orbital venous pressure (OVP), ciliary blood flow, and aqueous flow (AqFlow). Measurements were made before and after an intravenous administration of phentolamine (0.1 mg/kg). RESULTS: Phentolamine caused significant decreases in IOP -23%+/-2%; P<0.01), OVP (-28%+/…

Hemodynamics of papaverine- and phentolamine-induced penile erection.

We investigated the hemodynamic changes induced in the canine corpora cavernosa by intracorporeal injection of papaverine or phentolamine. In six dogs studied, papaverine profoundly increased the resistance to venous outflow, whereas phentolamine (an alpha-adrenergic blocker) had little or no effect. In four other dogs, flow within the internal pudendal artery was measured with an ultrasonic flow probe placed directly on the artery. Papaverine increased arterial flow by 300 to 700 per cent over baseline levels, whereas the increase with phentolamine was only 50 to 100 per cent. We conclude tha…

[Effects of norepinephrine and phentolamine on acute intracranial hypertension].

It is accepted that cerebrovascular dilatation is a constant response during the advanced stage of intracranial hypertension. Severe intracranial hypertension ultimately associates with profound vasodilatation and reduces cerebral blood flow to zero. This irreversible state is called cerebral vasomotor paralysis by Langfitt et al (1965). The rich adrenergic nerve supply of the cerebral vessels suggests that pressor amines possibly affect the cerebral circulation and the cerebral vascular tone. This study is to investigate the reactivity of norepinephrine and phentolamine on intracranial pressu…

Use of phentolamine in the treatment of Epipen induced digital ischaemia.

A 33-year-old lady accidentally injected norepinephrine into her right thumb while cleaning the first aid box where a spare Epipen for her son was kept. She presented with pain, pallor and swelling to the distal phalanx of the right thumb. After a prompt literature search and discussing the case with the National Poisons Information Service, local injection of 0.15 ml (1.5 mg) of phentolamine in 1 ml of 2% lignocaine was advised. Peripheral perfusion was restored immediately after injection and fully recovery was noted at the review clinic the following day. The presentation of this case in th…
