Pediatric postoperative cardiac care.

Postoperative care of cardiac patients requires a comprehensive and multidisciplinary approach to critically ill patients with cardiac disease whose care requires a clear understanding of cardiovascular physiology. When a patient fails to progress along the projected course or decompensates acutely, prompt evaluation with bedside assessment, laboratory evaluation, and echocardiography is essential. When things do not add up, cardiac catheterization must be seriously considered. With continued advancements in the field of neonatal and pediatric postoperative cardiac care, continued improvements

Postoperative care after lymphaticovenous anastomosis.

In this report, we discuss the postoperative protocol for patients undergoing lymphaticovenous anastomosis (LVA) in our unit. Immediately after LVA, the incision site is closed over a small Penrose drain and a simple gauze dressing is applied without compression. In the first 5 days, ambulation is allowed, but limb elevation is actively encouraged to promote lymphatic flow across the newly formed anastomosis. Prophylactic antibiotics are routinely given to prevent infection because this patient group is susceptible to infections, which could trigger thrombosis in the anastomosis.

Postoperative care following pituitary surgery.

Patients undergoing surgery for pituitary tumors represent a heterogeneous population each with unique clinical, biochemical, radiologic, pathologic, neurologic, and/or ophthalmologic considerations. The postoperative management of patients following pituitary surgery often occurs in the context of a dynamic state of the hypothalamic-pituitary-end organ axis. Consequently, a significant component of the postoperative care of these patients focuses on vigilant screening and observation for neuroendocrinologic perturbations such as varying degrees of hypopituitarism and disorders of water balanc

Postoperative care in cataract surgery.

PURPOSE OF REVIEW: The purpose of this study is to provide a summary of current trends and recent developments in postoperative care after cataract surgery. RECENT FINDINGS: There is new evidence challenging the routine use of a protective eye shield after uncomplicated cataract surgery. Nonsteroidal anti-inflammatory drugs (NSAIDs) are effective in decreasing the risk of cystoid macular edema (CME) in high-risk eyes, but must be used with caution in patients with prior corneal disease. Pre-existing ocular comorbidities can have significant effects on postoperative outcomes. Management of pos

[Postoperative care after cataract surgery].

The postoperative treatment of cataract surgery is an important element for success of the surgery. The postoperative therapy primarily aims at the prevention of intraocular bacterial infection by administration of antibiotics. Anti-inflammatory drugs can suppress the operation-related inflammation. In addition to fundamental risks of topical eye drop application, this review article discusses the current therapeutic strategies for the prevention of postoperative bacterial infection and suppression of surgically induced inflammation.

Postoperative care after liver transplantation.

The proliferation of liver transplant services across the country during the next 5 years will be an interesting phenomenon to observe. An enlightened approach to the process require answers to several essential questions, many of which are discussed elsewhere in this issue of Seminars. Regardless of how the future distribution of services sorts out, many physicians will face an increasing likelihood that their services will be requested in the long-term management of liver transplant recipients. In this report, the care of liver transplant patients is discussed, including that involving the i

Postoperative care of the gynecology patient.

This chapter has dealt with some of the general issues that impact upon postoperative care. Within the context of this volume, a detailed analysis of each subject is not feasible. Some subjects were deliberately deferred to other contributors. My intent was to review some basic physiology and patho-physiology to emphasize the complex physical and metabolic alterations produced by abdominal pelvic surgery and to emphasize my personal belief that an uncomplicated postoperative recuperation begins with appropriate presurgical care.

Systems of Care Delivery and Optimization in the Postoperative Care Wards.

A third of all patients are at risk for a serious adverse event, including death, in the first month after undergoing a major surgery. Most of these events will occur within 24 hours of the operation but are unlikely to occur in the operating room or postanesthesia care unit. Most opioid-induced respiratory depression events in the postoperative period resulted in death (55%) or anoxic brain injury (22%). A future state of mature artificial intelligence and machine learning will improve situational awareness of acute clinical deterioration, minimize alert fatigue, and facilitate early interven

Aortic Dissections-Postoperative Care and Considerations.

Postoperative care following aortic dissection is a multidisciplinary effort, which is enhanced by the presence of an experienced team. Overall outcomes, including survival, are correlated with the early postoperative course and presence of complications such as malperfusion. Despite improvements, in-hospital mortality remains high. Patients who do survive to hospital discharge have a positive prognosis; however, additional interventions for progression of aortic disease may be required. Discharge recommendations for patient care are largely institution and provider dependent due to limited da

Targeted nutritional strategies in postoperative care.

Immunonutrition, which uses specific nutrients to modulate the immune response, has emerged as a vital adjunct to perioperative care. Surgery-induced stress triggers immune responses that can lead to complications, such as infections and delayed wound healing. Traditional nutritional support often overlooks the immunological needs of surgical patients. Immunonutrition addresses this oversight by providing key nutrients, such as arginine, omega-3 fatty acids, glutamine, nucleotides, and antioxidants (vitamins C and E) to enhance immune function and support tissue repair. This review examined th

Postoperative care in pediatric liver transplantation.

In the last 25 years, liver transplantation in children has become an effective, definitive, and universally accepted treatment for terminal liver diseases. Long-term survival exceeds 80% and improves each year as the result of constant technical advancements and improvements in immediate postoperative intensive care and clinical control.

Phonomicrosurgery III: pre- and postoperative care.

This article discusses the important aspects of pre- and postoperative care for phonomicrosurgical procedures. Making an accurate diagnosis is paramount to any surgical decision, which involves interdisciplinary evaluations by a laryngologist, voice pathologist, singing voice specialist, and other specialists. Patient selection and candidacy for phonomicrosurgery are discussed, including preoperative vocal rehabilitation, compliance, and pre- and postoperative timing issues. Postoperative voice rest issues are covered, and suggestions for postoperative voice rehabilitation are made. Risks of s

[Postoperative care of continent urinary reservoirs].

Continent urinary reservoirs like the Kock pouch, Indiana pouch or Mainz pouch are constructed from the detubularized intestine. These types of urinary diversion are good in regard to the quality of life, but meticulous postoperative care is required to avoid complications. Continent urinary reservoirs have common problems of mucous secretion and absorption of urinary constituents. Irrigation of the reservoir to remove the mucous debris is important to prevent stone formation and infections. The patient should be educated to perform the self catheterization properly and the danger of over-dist

Congenital choanal atresia: the postoperative care.

A case of congenital bilateral bony choanal atresia treated by surgery is reported. A new method of insertion and retention of the modelling tubes to prevent stenosis is described. This method has the following advantages: 1. it provides a good insertion and retention of the modelling tubes; 2. it doesn't provoke any trauma of the nasal septum, soft palate and nostril; 3. the insertion of the tubes doesn't require general anesthesia; 4. it's well tolerated.
