Ectopic pregnancy, defined as the implantation of a developing pregnancy outside of the endometrial cavity of the uterus, is the leading cause of early-pregnancy maternal mortality. The majority of ectopic pregnancies implant in a fallopian tube. Acute complications may include rupture of the fallopian tube or rupture of ectopic pregnancy, haemorrhage and hypovolaemic shock, or occur secondary to treatments such as emergency surgery or blood transfusions, and ultimately increase the risk of maternal death. After ectopic pregnancy, patients may experience ongoing morbidity, including chronic pa

An ectopic pregnancy is a pregnancy outside of the uterine cavity. An ectopic pregnancy can rupture and lead to hypovolaemic shock due to intraperitoneal bleeding. We describe 3 cases of women with ectopic pregnancy and discuss the diagnostic and therapeutic options. All women of fertile age with abdominal pain or abnormal vaginal bleeding should get a pregnancy test. Vaginal ultrasound and human chorionic gonadotrophin levels are the most important diagnostic tools. Depending on the findings, management can may be either conservative with repeated follow-up, intramuscular methotrexate or surg

Ectopic pregnancy is a major gynecologic emergency, which results in significant morbidity for the mother and inevitable loss of the pregnancy. Its presentation can be varied from minor symptoms to sudden collapse. It produces a diagnostic dilemma and a management problem. With advances in medical care, most women survive ectopic pregnancy and many will be diagnosed early enough to allow conservative management with the resulting lower morbidity and possible anatomic conservation. This article covers what is currently known about the etiology of this condition, the best approach to the diagnos

Ectopic pregnancy is an important cause of morbidity and mortality worldwide. Use of transvaginal ultrasonography and quantitative measurement of the beta subunit of human chorionic gonadotropin (beta-hCG) has led to a reduction in the need for diagnostic laparoscopy. Furthermore, with earlier diagnosis, medical therapy with methotrexate can be offered and surgery avoided in some women, though the best regimen remains unclear. In the surgical management of ectopic pregnancy, the benefits of salpingectomy over salpingostomy are uncertain. Although there have been advances in the management of e

The incidence of ectopic pregnancy in the United States is increasing annually and is approaching epidemic proportions. Serum progesterone measurements show promise for the early detection of abnormal gestational development. Unfortunately, examination of serum progesterone levels does not allow discrimination between an ectopic pregnancy and an intrauterine abortion. Additionally, discriminatory levels of progesterone fail to include all abnormal gestations in the abnormal group (false-negative result) and classify some normal pregnancies to the abnormal group (false-positive result). As such

The incidence of ectopic pregnancy has been increasing for many years and currently is two to three times higher than it was 20 or 25 years ago. The reasons for this are complex and include increased rates of pelvic and tubal infection, the use of the IUD, surgery in the pelvis for infertility, sterilization and other surgical procedures, in vitro fertilization, and improved diagnosis. Normal tubal physiology, the findings in affected tubes, and the mechanisms of how the ectopic pregnancy develops and involves the tube are considered. The cardinal points of the history and physical examination

The diagnosis of ectopic pregnancy has become precise and reliable. Consequently, the management of ectopic pregnancy has progressed to the point where the physician is often able to preserve fertility. Therefore, conservative surgery is indicated if the patient desires future fertility and conditions are appropriate. The combination of ultrasound, beta-hCG pregnancy testing, and laparoscopy has led to a rising incidence of diagnosed ectopic pregnancy prior to rupture. This has greatly facilitated the use of the conservative approach to the management of tubal pregnancy. Although ectopic pregn

Ectopic pregnancy continues to be a major clinical problem and is the leading cause of death in the first trimester of pregnancy. Diagnosis can be elusive. A thorough knowledge of the clinical spectrum of this disease, as well as the diagnostic tools available to the primary care clinician, provides the opportunity for making an early diagnosis. This is essential if the morbidity and mortality of ectopic pregnancy are to be reduced.

Ectopic pregnancy occurs in 2% of all pregnancies and is a potentially life-threatening emergency. A high level of clinical suspicion is required for any pregnant patient who presents with vaginal bleeding and/or pelvic pain. Workup should begin with immediate triage based on vital signs, a pregnancy test, and transvaginal ultrasound. Ectopic pregnancy can be treated either medically with methotrexate or surgically with either salpingectomy or salpingostomy. Carefully counseled, asymptomatic patients may be candidates for expectant management.

The differential diagnosis in a pregnant patient who presents with pain and bleeding in the first trimester includes normal early pregnancy, spontaneous abortion, molar pregnancy, and ectopic pregnancy. Knowledge of the sonographic appearance of these entities is helpful at arriving at the correct diagnosis. When no intrauterine pregnancy is visualized, careful attention to the adnexa is crucial for finding an extraovarian mass, since the fallopian tube is the most common location for ectopic pregnancy. This review describes and illustrates the sonographic findings of ectopic pregnancy.
