Prune belly syndrome is a rare congenital disorder of the urinary system, characterized by a triad of abnormalities. The aetiology is not known. Many infants are either stillborn or die within the first few weeks of life from severe lung or kidney problems, or a combination of congenital anomalies.

The majority of paediatric surgeons will encounter a patient with prune belly syndrome (PBS) only a few times in their clinical practice. There have been many opposing views in the literature regarding the pathogenesis and management of this complex condition. Prune belly is a lax, wrinkled abdominal wall and frequently is associated with other anomalies. The prune belly syndrome is a specific constellation of anomalies consisting of an abdominal wall deficient in muscular tissue, dilated urinary tract, and bilateral cryptorchidism. This group of anomalies is also called the Eagle-Barrett syndrome and the triad syndrome. In about 3 of 4 patients with the prune belly syndrome, there are associated malformations of the cardiopulmonary, gastrointestinal, and orthopedic systems.

Referring to a patient operated on the third day of life for pyeloureteral stenosis the 'prune belly' syndrome is described. The syndrome is delineated by urinary tract anomalies, undescended testicles and a prune like abdominal wall. Several other anomalies are commonly seen. In this patient the megaureters, the vesicoureteral reflux and megacystis are considered part of the syndrome. The pyeloureteral stenosis was only seen once before in a succumbed newborn.

A large sonolucent mass was first revealed by the ultrasonic echography examination of a fetus at 18 1/2 weeks' gestation, but was no longer present at 28 1/2 weeks' gestation. The fetus was subsequently born with the "prune belly" syndrome. The case is described, and the possible causes, and possibilities of prevention, of "prune belly" are discussed.
