PET/CT and SPECT/CT imaging of 90Y hepatic radioembolization at therapeutic and diagnostic activity levels: Anthropomorphic phantom study.

PURPOSE: Prior to 90Y radioembolization procedure, a pretherapy simulation using 99mTc-MAA is performed. Alternatively, a small dosage of 90Y microspheres could be used. We aimed to assess the accuracy of lung shunt fraction (LSF) estimation in both high activity 90Y posttreatment and pretreatment scans with isotope activity of ~100 MBq, using different imaging techniques. Additionally, we assessed the feasibility of visualising hot and cold hepatic tumours in PET/CT and Bremsstrahlung SPECT/CT images. MATERIALS AND METHODS: Anthropomorphic phantom including liver (with two spherical tumours)

Evaluating therapeutic efficacy of extended shelf-life 90 Y glass microspheres in transarterial radioembolization for colorectal cancer: a quantitative FDG PET/CT analysis.

OBJECTIVES: There is a lack of sufficient evidence regarding the use of extended shelf-life (ExSL) Yttrium-90 ( 90 Y) glass radiomicrospheres in metastatic colorectal cancer (mCRC) patients. We aimed to investigate the efficacy of ExSL 90 Y glass radiomicrospheres with a personalized treatment approach by analyzing 18 F-FDG PET/CT quantitative parameters [metabolic tumor volume (MTV) and total lesion glycolysis (TLG)] separately before and after the treatment. METHODS: A total of 93 radioembolization sessions involving 77 patients were included. Simplicit 90 Y software was utilized to perform

Radioembolization (Selective Internal Radiation Therapy-SIRT). Changes in clinical practice, points of our experience.

Radioembolization (SIRT) was initially introduced in the early 21st century as a whole-liver, primarily palliative treatment, for hepatocellular carcinoma (HCC) and liver metastatic colorectal cancer (mCRC). Over time, its application expanded to include other primary and secondary liver tumors. Clinical experience and emerging studies have demonstrated a clear relationship between higher tumor-absorbed radiation doses and improved tumor response. Such dose escalation can be achieved through advanced, more precise, and synchronous dosimetric approaches (personalized dosimetry), or in selected

Transarterial Therapy for Liver Tumors.

Primary and secondary liver tumors represent a significant global health challenge and are often diagnosed at stages where curative options are not feasible. Transarterial therapies (TATs) have emerged as key locoregional treatment options for patients with unresectable liver tumors, using the liver's unique dual blood supply to selectively target tumor tissue while sparing healthy parenchyma. These therapies include transarterial embolization, transarterial chemoembolization, and transarterial radioembolization, each with distinct mechanisms, technical considerations, and clinical application

Recent developments and prospects in locoregional therapy of hepatocellular carcinoma.

This invited review discusses recent advancements and prospects in locoregional therapy (LRT) for hepatocellular carcinoma (HCC), a leading cause of cancer mortality globally. The epidemiology of HCC is shifting toward nonviral etiologies, prompting a focus on early surveillance and personalized treatment approaches. Updated international guidelines, such as the Barcelona Clinic Liver Cancer (BCLC) staging system, American Association for Study of Liver Disease (AASLD) guideline, and European Association for the Study of the Liver (EASL) now recognize LRT-including transarterial embolization,

The use of locoregional therapies in cholangiocarcinoma.

The standard of care for patients with unresectable or advanced cholangiocarcinoma (CCA) is systemic chemotherapy combined with immunotherapy, which achieves a median overall survival (OS) of 13 months and a 3-year OS of 15%. This provides the benchmark against which additional locoregional therapies are evaluated. This narrative literature review examined locoregional therapies, their clinical outcomes, and recommendations for their role in the management of patients with CCA. The review focused on intrahepatic (iCCA) and perihilar CCA (pCCA) with locoregional disease in patients who were ine

Dosimetric Development of Yttrium-90 Resin Microspheres for Selective Internal Radiation Therapy: Current Status and Perspectives.

Yttrium-90 resin microspheres selective internal radiation therapy ( 90 $$ {}^{90} $$ Y-SIRT) has been increasingly adopted worldwide as a locoregional treatment option for appropriately selected patients with liver malignancies. The key to ensuring that the tumor receives an adequate radiation dose while minimizing the dose to normal tissues is to optimize the trade-off between tumor control and the probability of normal tissue complications. To accurately determine a patient's internal radiation dose, pre-treatment dose planning and post-treatment dose verification using dosimetric met

Real-Life Experience in the Treatment of Intrahepatic Cholangiocarcinoma by 90Y Radioembolization: A Multicenter Retrospective Study.

Limited treatment options in patients with intrahepatic cholangiocarcinoma (iCCA) demand the introduction of new, catheter-based treatment options. Especially, 90Y radioembolization may expand therapeutic abilities beyond surgery or chemotherapy. Therefore, the purpose of this study was to identify factors associated with an improved median overall survival (mOS) in iCCA patients receiving radioembolization in a retrospective study at 5 major tertiary-care centers. Methods: In total, 138 radioembolizations in 128 patients with iCCA (female, 47.7%; male, 52.3%; mean age ± SD, 61.1 ± 13.4 y) wer

Locoregional therapies for intrahepatic cholangiocarcinoma: a stage-stratified, biology-informed multidisciplinary framework-a narrative review.

BACKGROUND AND OBJECTIVE: Intrahepatic cholangiocarcinoma (iCCA) is an aggressive primary liver malignancy with rising global incidence; most patients present with unresectable disease, and intrahepatic recurrence is common after curative-intent surgery. Locoregional therapies (LRTs), including future liver remnant (FLR) augmentation, percutaneous ablation, transarterial chemoembolization (TACE), transarterial radioembolization (TARE), hepatic arterial infusion chemotherapy (HAIC), and stereotactic body radiotherapy (SBRT), are increasingly applied across the disease continuum. This narrative

Challenging the Mirror Image: Radiation Segmentectomy in Situs Inversus Totalis With Pretreatment Embolization of Aberrant Gastroesophageal Arteries.

Situs inversus totalis (SIT) is a rare congenital anomaly characterized by the mirror-image transposition of the normal anatomical positions of major visceral organs and vascular structures. The reversed placement of cardiovascular structures observed in SIT may lead to additional technical challenges in endovascular treatment procedures. This paper presents the transarterial radioembolization (TARE) treatment applied to a 59-year-old man with SIT diagnosed with liver metastasis from rectal cancer. A literature review revealed no publications regarding the application of TARE in patients with

Impact of catheter angle on microsphere distribution in transarterial radioembolization: an in silico study based on ex vivo porcine liver data.

Computational fluid dynamics (CFD) simulations were conducted to explore the impact of catheter angle on microsphere distribution in transarterial radioembolization (TARE). The models were designed to closely replicate conditions from previously conducted ex vivo machine-perfused porcine liver experiments and therefore improve the perspective on microsphere distribution patterns. TARE with holmium-loaded microspheres was performed on an ex vivo machine-perfused porcine liver. Catheter position was imaged using angiography and ultrasound, the hepatic arterial vasculature using 3D cone beam CT,

Radioembolization in the treatment of unresectable liver tumors: experience across a range of primary cancers.

Radioembolization is a proven treatment to slow disease progression and improve survival in patients with colorectal cancer liver metastases and hepatocellular carcinoma. Accumulating evidence supports its use in metastases from neuroendocrine tumors and breast cancer. Cancers with radiobiologic profiles similar to those of colorectal and breast cancer, including melanoma, lung cancer, and nodular cholangiocarcinoma, are being studied as candidates for radioembolization. This treatment modality has also been shown to downsize hepatic tumors for potentially curative ablation in patients with br
