INTRODUCTION: Renal colic accounts for 5-10% of all emergency department visits, making it a common condition in acute medicine. The typical clinical presentation is an early indication of urolithiasis.
DIAGNOSIS: Diagnostic measures include laboratory tests, ultrasound, and low-dose noncontrast computed tomography (CT) scans. Kidney, ureter, bladder (KUB) plain film radiography has been widely replaced by low-dose noncontrast CT with similar radiation dosage. In special patient groups such as children or pregnant women, ionizing radiation should be avoided if possible.

A positive diagnosis of urinary stone was made in 183 of 407 patients (44.9%) with renal colic seen in the emergency room. 182 (44.7%) had normal intravenous pyelograms, 26 (6.4%) had evidence of dilatation of the urinary system, possibly due to the passage of a stone, and 16 (3.9%) had other pathological urinary conditions, such as double collecting system or cysts. UNLABELLED: Renal colic in pregnant women is a serious condition, mainly when is associated with fever.
MATERIAL AND METHOD: Our retro-prospective study analyzes 111 cases managed conservatively or with endourological procedures for renal colic--insertion of JJ stents and percutaneous nephrostomy.
RESULTS: Clinical evolution determined the insertion of JJ stents in 60 cases and the failure of this procedure imposed percutaneous nephrostomy in 5 cases. In 56 cases urinary tract infection was associated and in 2 cases, despite all efforts, the patients deceased due to sever sepsis.

Acute ureteral obstruction is always associated with high intrapelvic hydrostatic pressure. Objective diagnosis of renal colic can be made by direct measurement of intrapelvic pressure (IPP). We propose a very simple device for estimation of IPP consisting of ureteral catheter and polyethylene tube 150 cm long. The device gives intrapelvic pressure in centimeters of urinary column. We discuss the case of a young man presenting with "renal colic" whose flank pain however, turned out to be the result of renal infarction due to dissection of an accessory renal artery with fibromuscular dysplasia. The condition was diagnosed after exclusion of nephrolithiasis and work-up of new onset arterial hypertension developing in the later course. He was successfully treated by ethanol ablation (renal ethanol angioinfarction) and coiling of the accessory renal artery with resolution of hypertension in the absence of any antihypertensive medication.

Spontaneous renal pelvic haematomas are rare, often mimicking symptoms of other pathologies such as a renal tumour or renal calculi. Spontaneous renal haematoma was first reported by Bonet in 1679 and later described by Wunderlich in 1856. We present the case of a young female patient with no known comorbidities who presented with spontaneous renal pelvis haematoma. Misinterpretation of this finding can lead to erroneous diagnoses.

A prospective randomized study was performed to compare the effect of acupuncture and intramuscular Avafortan injection in the treatment of renal colic. Our results showed that acupuncture is as effective in relieving renal colic as Avafortan but it had a more rapid analgesic onset (3.14 +/- 2.88 minutes versus 15.44 +/- 7.55 minutes, p less than 0.05). A Mesenteric panniculitis is an uncommon disorder with unknown etiology. It may cause nonspesific abdominal or systemic symptoms. Radiological modalitis especially computed tomography are very helpful to make a diagnosis. Hence, the radiologist should be familiar with its tomographic findings to avoid further examinations and unnecessary surgery. We report computed tomography findings of mesenteric panniculitis in an adult patient with renal colic.

OBJECTIVE: To determine the role of intrarenal Doppler ultrasound (US) in patients with renal colic and to establish the usefulness of this diagnostic method.
MATERIALS AND METHODS: In 121 patients with renal colic and 70 healthy individuals, 382 kidneys were examined with color duplex US. Mean intrarenal-arterial resistive index (RI), and the difference of mean RIs (dRI) between both kidneys were determined.
