The heart rate of Romano-Ward syndrome patients.

The Romano-Ward prolonged QT syndrome is associated with an abnormality of ventricular recovery, rendering the patients susceptible to life- … fore, be manifest by slower heart rates. To examine this possibility, the resting heart rates of 58 Romano-Ward syndrome patients were compared to those of 255 age-matched normal controls. A significant difference in resting heart rate was observed between the Romano-Ward syndrome patients and the normal controls in newborns and children up through age 3 years. No dif …

[Clinical and electrophysiological observations in a case of Romano-Ward syndrome ].

Syncope associated with atypical ventricular tachycardia of the "torsades de pointes" type was observed in a 16 year-old girl with hereditary QT prolongation. The arrhythmia occurred only during maximal prolongation of the QTc to 0.77 sec, which had possibly been aggravated by exercise and hypokalaemia. Electrophysiological studies were performed when the QTc was 0.59 sec. Incremental as well as premature ventricular pacing with single or double premature ventricular beats did not initiate ventricular arrhythmias, but revealed AV nodal and bundle branch reentry. It is postulated that these typ

Molecular genetic aspects of the Romano-Ward long QT syndrome.

The Romano-Ward long QT syndrome, an autosomal dominant inherited disorder, is an increasingly recognized cause of sudden cardiac death in children and adults. Sudden death may or may not be preceded by a history of "seizures" and recurrent syncope; the diagnosis relies on electrocardiographic evidence, including prolongation of the QT interval corrected for heart rate, T wave abnormalities, sinus bradycardia, and polymorphous ventricular tachycardia (torsades de pointes). Recently, a gene responsible for causing long QT syndrome was localized (using molecular genetic methods) to the short arm

[Hereditary prolonged QT interval (Romano Ward syndrome) in obstetric management].

In the Romano-Ward syndrome, an inherited form of abnormally prolonged QT duration is a matter for concern. The frequency-corrected QTc value in the ECG amounts to more than 0.44 s. Persons suffering from Romano-Ward syndrome are vitally endangered by attacks of arrhythmia, syncope and cardiac arrest which are triggered or aggravated by stress or certain drugs. In obstetrical anaesthesia, attention should be paid to the foetus, which, due to the genetic dominance, will have a 50% probability of suffering from the disorder. A 28-year-old para I was admitted with cervical dilation on her predete…

Romano-Ward syndrome diagnosed as moderate fetal bradycardia. A case report.

BACKGROUND: Romano-Ward syndrome is a rare, autosomal-dominant cardiac conduction defect associated with syncope and life-threatening ventricular arrhythmia in children and adults. CASE: A fetus was noted to have moderate fetal bradycardia in the antenatal period. When studied postnatally, the child was found to have Romano-Ward syndrome. CONCLUSION: This is the first reported case of Romano-Ward syndrome diagnosed prenatally as fetal bradycardia. This report confirms that moderate fetal bradycardia does not indicate fetal distress, but it does indicate that fetuses should be studied for car…

Functional Characterization of a Spectrum of Novel Romano-Ward Syndrome KCNQ1 Variants.

The KCNQ1 gene encodes the α-subunit of the cardiac voltage-gated potassium (Kv) channel KCNQ1, also denoted as Kv7.1 or KvLQT1. The channel … gram (ECG). Autosomal dominant loss-of-function variants in KCNQ1 result in the LQT syndrome called Romano-Ward syndrome (RWS), while autosomal recessive variants affecting function, lead to Jervell and Lange-Nielsen syndrome (JLNS), associated with deafness. The aim of this study was the characterization of novel KCNQ1 variants identified in patients with RWS to wide …

[A child affected by the Romano-Ward syndrome born of a mother with the Jervell and Lange-Nielsen syndrome].

We report a child affected with the Romano-Ward syndrome (long QT, normal hearing), born from a mother affected with the Jervell and Lange-Nielsen syndrome (long QT plus deafmutism). We discuss the possible pathogenetic reasons for this uncommon association.

Syncope and sudden arrhythmic death complicating pregnancy. A case report of Romano-Ward syndrome.

Romano-Ward syndrome is a subtype of prolonged QT syndrome with autosomal dominant inheritance. Stress-induced syncope and sudden death are secondary to ventricular tachydysrhythmias. A case report of Romano-Ward syndrome complicating pregnancy is presented. Successful therapy with propranolol for life-threatening dysrhythmias was achieved. An excellent neonatal outcome occurred.

The QT prolongation syndrome: long-term follow-up study of 13 families with Romano-Ward syndrome.

To investigate the long-term course or natural history of Romano-Ward syndrome, 131 cases from the affected side of 13 families with the syndrome were examined by electrocardiography and a review of the histories. The patients were followed up for 2-13 years, with an average of 7.2 years. Five patients (3.8%) died suddenly, four with QT prolongation and one with a normal QT interval. Fourteen patients (10.7%) with QT prolongation developed episodes of syncope, whereas 58 patients (44.3%) showed only QT prolongation without syncopal episode. Three of the thirteen families were asymptomatic, wit…

Consanguinity, cardiac arrest, hearing impairment, and ECG abnormalities: counselling pitfalls in the Romano-Ward syndrome.

Genetic counselling in the autosomal dominant condition of the Romano-Ward syndrome might be assumed to be relatively straightforward. The problems posed by consanguinity, deafness, and subclinical gene carriers in a pedigree with this condition have caused us to reevaluate this view. The diagnostic and management difficulties which may attend this potentially fatal condition are highlighted by our experience with this family.

Mutational and phenotypic spectra of KCNE1 deficiency in Jervell and Lange-Nielsen Syndrome and Romano-Ward Syndrome.

KCNE1 encodes a regulatory subunit of the KCNQ1 potassium channel-complex. Both KCNE1 and KCNQ1 are necessary for normal hearing and cardiac … ariants of KCNE1 and KCNQ1 have prolonged QT intervals, a dominantly inherited phenotype designated Romano-Ward syndrome (RWS), which is also associated with arrhythmias and elevated risk of sudden death. Coassembly of certain mutant KCNE1 monomers with wild-type KCNQ1 subunits results in RWS by a dominant negative mechanism. This paper reviews variants of KCNE1 and t …

Recessive Romano-Ward syndrome associated with compound heterozygosity for two mutations in the KVLQT1 gene.

We describe a Swedish family with the proband and his brother suffering from severe Romano-Ward syndome (RWS) associated with compound heterozygosity for two mutations in the KVLQT1 (also known as KCNQ1 and KCNA9) gene (R518X and A525T). The mutations were found to segregate as heterozygotes in the maternal and the paternal lineage, respectively. None of the heterozygotes exhibited clinical long QT syndrome (LQTS). No hearing defects were found in the proband. The data strongly indicates that the compound heterozygosity for R518X and A525T is the cause of an autosomal recessive form of RWS in

[Sympathetic ganglionitis in the hereditary syndrome of QT prolongation (Romano-Ward syndrome); intracardiac registration of after-potentials (author's transl)].

A 20-year-old woman, admitted to an intensive care unit in ventricular fibrillation, had marked QT prolongation in the ECG after successful defibrillation. Family study revealed ECG signs of QT prolongation, as seen in the Romano-Ward syndrome, in four generations. Syncopal attacks only occurred in the third generation, affecting four siblings. In the described patient the intraventricular ECG revealed additional depolarizations (after-potentials) during and following repolarization (ST-T) in the surface ECG. The patient died later as a result of irreversible hypoxic cerebral damage. Invasive …

Heart rate pattern during sleep in an infant with congenital prolongation of the Q-T interval (Romano-Ward syndrome).

Heart rate and the variability of the heart rate, indices of autonomic control, were studied during sleep in an infant with prolonged Q-T interval (Romano-Ward syndrome) and were compared to the heart rate and variability of heart rate in 18 normal infants studied at monthly intervals during the first four months of life. The overall variability and beat-to-beat variability in the infant with Romano-Ward syndrome were significantly below the median in the normal infants at each age and sleep state. This decrease in overall and beat-to-beat variability persisted after normalization by the absol…
