[Silicosis].

BACKGROUND: Silicosis is a potentially severe but preventable occupational lung disease caused by inhalation of silica particles. There is a wide application in the usage of silica especially in lesser known industries. This disease has yet not been eradicated due to insufficient application of protective measures. CASE DESCRIPTION: A 57-year-old patient presents to the pulmonary outpatient clinic with progressive dyspnea d'effort. The professional history states that he worked as a sandblaster 10 years ago. The accompanying protective measures were not properly followed by the patient at the…

Silicosis.

Silicosis is a fibrotic lung disease caused by inhalation of free crystalline silicon dioxide or silica. Occupational exposure to respirable crystalline silica dust particles occurs in many industries. Phagocytosis of crystalline silica in the lung causes lysosomal damage, activating the NALP3 inflammasome and triggering the inflammatory cascade with subsequent fibrosis. Impairment of lung function increases with disease progression, even after the patient is no longer exposed. Diagnosis of silicosis needs carefully documented records of occupational exposure and radiological features, with ex…

Update on Silicosis.

Although silicosis has been an established disease with a recognized cause for more than 100 years, many workers continue to be exposed to silica and new outbreaks of disease continue to occur. This article describes some of the well-established and new exposures, including denim sandblasting, artificial stone cutting, and some forms of "coal worker's pneumoconiosis." The authors review the imaging and pathology of acute silicosis (silicoproteinosis), simple silicosis, and progressive massive fibrosis and summarize known and putative associations of silica exposure, including tuberculosis, lun…

[Silicosis and hemoptysis].

INTRODUCTION: Pneumoconiosis is defined as a diffuse interstitial lung disease secondary to inhalation of mineral particles. Haemoptysis is common in pneumoconiosis. But it is usually secondary to bronchiectasis, bronchitis or pulmonary tuberculosis occurring with silicosis. OBSERVATION: A 74-year-old Portuguese man, known to have chronic respiratory failure secondary to chronic silicosis, was treated for moderate haemoptysis. CT angiography revealed bilateral fibrosing peri-hilar masses, inferior lobe nodules and previously known mediastinal lymphadenopathy, without active bleeding. Fibreopt…

HL-A and silicosis.

The search for a link between HL-A antigens and silicosis among 75 patients revealed no significant association. However, we did note a decrease in the frequency of B7; This change could be linked to a greater sensitivity to silicosis among persons who carry this antigen. There was also an increase of B8 among those silicotic patients with tuberculosis, although only a small number of patients were affected.

Silicosis and tuberculosis: Part 2--a radiographic presentation of nodular tuberculosis and silicosis.

Indolent nodular pulmonary tuberculosis (TB) in workers exposed to silica dust may go undetected clinically and radiographically, especially in the absence of identification of tubercle bacilli in sputum. Illustrative cases demonstrating the radiographic manifestations of coexistent pulmonary silicosis and the indolent form of nodular TB are presented. Alterations in the usual chronologic progress, a rapid advance in nodular profusion or size outside the expected time frame, and distinct pattern alterations are features indicating the presence of TB associated with silicosis.

Elimination of tuberculosis requires prior control of silicosis including sub-radiological silicosis.

India is committed to the elimination of tuberculosis by 2025. But its achievement appears to be difficult as India has a huge burden of silicosis as well as sub-radiological silicosis, which was never given its required attention. Silicotic subjects are highly vulnerable to pulmonary tuberculosis due to the progressive decline of lung immunity. A study among vulnerable glass factory workers in Firozabad, Uttar Pradesh, revealed that silicotic workers were 7.5 times more at risk of pulmonary tuberculosis compared to non-silicotic subjects. Since India has a huge burden of silicosis and sub-rad…

Silica, Silicosis, and Autoimmunity.

Inhalation of dust containing crystalline silica is associated with a number of acute and chronic diseases including systemic autoimmune diseases. Evidence for the link with autoimmune disease comes from epidemiological studies linking occupational exposure to crystalline silica dust with the systemic autoimmune diseases systemic lupus erythematosus, systemic sclerosis, and rheumatoid arthritis. Although little is known regarding the mechanism by which silica exposure leads to systemic autoimmune disease, there is a voluminous literature on silica exposure and silicosis that may help identify …

[Silicosis and systemic diseases].

We present case histories of 5 patients with silicosis who developed systemic lupus erythematosus and microscopic polyarteritis in two cases each, and rapidly progressive glomerulonephritis (limited Wegener's granulomatosis) in one case. An association between silica exposure and autoimmune disease is emphasized.

Predictors of mortality in silicosis.

The mortality of a cohort of 1487 male patients with silicosis in a population-based register followed up from 1980 to 1986 was evaluated with reference to the mortality rates of the general male population. A striking excess of deaths from all causes (observed 368, standardized mortality ratio, SMR 3.00) was noted. Seventy-four percent of the deaths were due to respiratory conditions and complications directly or indirectly related to silicosis. The risk of death was especially higher than expected in younger patients under 45 years of age. Patients with simple silicosis of profusion category…

Silicosis in the 1980's.

At the request of the Mine Safety and Health Administration (MSHA), the National Institute for Occupational Safety and Health (NIOSH) recently investigated the health of workers at two silica flour mills. Both mills have a long history of failure to maintain workplace concentrations of dust containing free silica below the MSHA exposure standard. Thirty-seven percent of sixty-one workers and ex-workers with one or more years of silica dust exposure had chest radiographic evidence of silicosis. Because of the high prevalence of silicosis in workers at these two silica flour producers, MSHA sili…

A case of accelerated silicosis.

Silicosis, caused by inhaling dust containing free crystalline silica, typically has a chronic course, with the numbers of silicosis patients declining globally. Much rarer are the acute and subacute forms. Presented is a case of severe subacute (accelerated) silicosis. The condition resulted from ~2 years of very intense exposure without appropriate personal protective equipment while sandblasting. The patient's initial symptoms were progressive cough, dyspnoea and weight loss. Given his occupational history, typical clinical manifestations and radiological findings, an initial diagnosis of a…
