[Concomitant strabismus: of strabismus in strabismus syndromes].

PURPOSE: To show how to progress from the deviation of the visual axis (provided the diagnosis of concomitant strabismus is certain) to the classification of the squint in one out of the different squint syndromes and consequently to adopt the appropriate therapeutic strategy. METHOD: Every sign correlated with the deviation contributes to progress step by step to the diagnosis of a given squint syndrome. The age on onset of strabismus, either convergent or divergent, and its characteristic, intermittent or constant, allow in a first step to evaluate the potential binocularity, as well in ear

Strabismus surgery in the treatment of paralytic strabismus.

Strabismus surgery may restore limited binocular function and improve cosmesis in patients with paralytic strabismus. Evaluation of the amount of residual function of the affected extraocular muscles is essential to determine which surgical procedure will yield the best results. Standard muscle resection techniques are effective for patients who have paretic extraocular muscles with residual function. Muscle transposition procedures are indicated for patients with total paralysis of an extraocular muscle. Autogenous and alloplastic materials may be required to fix the eye in primary position i

Strabismus.

Defining the type of strabismus creates a framework for work-up and management. Comitant esotropia is most commonly a childhood condition treated with glasses and surgery. Comitant exotropia is often a childhood condition that may require surgical correction. Microvascular disease is the most common cause of ocular cranial nerve palsies in adult patients.

Single-stage adjustable strabismus surgery for restrictive strabismus.

PURPOSE: To evaluate the feasibility and stability of ocular alignment of single-stage adjustable strabismus surgery (SSASS) in restrictive strabismus. METHODS: This was an observational case series comprising 12 patients with restrictive strabismus (mean age, 54.8 years) who were treated with SSASS using intravenous midazolam, fentanyl, and topical anesthesia. All were studied in a retrospective institutional manner. The refractive strabismus in 7 patients was caused by dysthyroid orbitopathy. Five patients had undergone previous ocular surgery, and 4 had undergone previous strabismus surger

Paralytic strabismus.

During the past year, many papers described new surgical approaches to correct extraocular muscles paralysis. New advances have been made in the knowledge of ptosis and superior oblique muscle myochymia. Moreover, the author reports sensory problems concerning subjective cyclorotation and binocularity that arise with macular translocation.

Cyclic strabismus.

An unusual age of presentation of cyclic strabismus, which became constant in a relatively short period of time without a previous history of surgery, trauma, or strabismus, is reported. The mechanism of cyclic strabismus is still unknown; however, most cases are treated successfully with strabismus surgery. Cyclic strabismus is distinct from other rhythmic diseases in that peripheral manipulation of the target organ results in resolution of the deviation.

Compartmental Strabismus.

Recent reports confirm innervational compartments of select rectus extraocular muscles as well as the superior oblique.1 Histopathological and orbital imaging studies demonstrate well defined compartmental innervation of the horizontal rectus muscles with less differentiation in the vertical rectus muscles. Acquired vertical misalignment not associated with cyclovertical muscle dysfunction has been associated with horizontal rectus muscle compartment dysfunction. Pattern and other forms of strabismus have been associated with segmental or compartmental abnormal innervation of the extraocular m

Comitant strabismus.

A review of the literature on comitant strabismus of the period from April 1999 until April 2000 is presented. A rather new and increasingly important issue is the psychosocial aspect of strabismus. Two studies have demonstrated that strabismus creates a significant negative social prejudice on the patients and that it can significantly reduce an applicant's ability to obtain employment. Subsequently, strabismus surgery can no longer be called "cosmetic". Concerning the timing of surgery in congenital esotropia, it was reported that early surgery does not ensure continued alignment, but freque

Paralytic strabismus.

In the past 12 months there have been many interesting publications on paralytic strabismus. Many investigators have reported on various imaging techniques helping both the clinician and the basic scientist better evaluate the causes and effects of paralytic strabismus on the extraocular muscle environment. Some interesting cases of neurologic and systemic associations have also been reported, including a unilateral pupil-sparing third nerve paralysis developing after the use of sildenafil citrate (Viagra; Pfizer, New York, New York). Advances in genetics are now helping us further understand

Paralytic strabismus.

This review of the year's literature on paralytic strabismus contains some especially interesting contributions on epidemiology of ischemic ocular motor palsies, new imaging diagnostic techniques in sixth and fourth nerve palsy and in Graves' ophthalmopathy, and bilateral histological findings in one case of clinically monolateral Duane's retraction syndrome. Some reports concern the reliability of diagnostic tests routinely used for measuring cyclotorsion in superior oblique palsies.

Comitant strabismus.

This article briefly overviews the recent literature regarding comitant strabismus. The evaluation and management of esodeviations and exodeviations, which are still topical questions today, are discussed. Several relevant basic studies focus on the prevalence and the role of sensory mechanisms in the clinical findings of essential infantile esotropia. Data reported on by the authors reinforce the validity of early surgery in congenital strabismus. Other articles deal with the accommodative esotropia. The beneficial effect of adequate corrective eyeglasses in accommodative esotropia is emphasi

Strabismus in craniosynostosis.

Strabismus is common in craniosynostosis, with rates from 39% to 90.9% in Crouzon, Apert, Pfeiffer, and Saethre-Chotzen syndromes. This article reviews the epidemiology of strabismus in these disorders and discusses competing theories of the mechanism, including absent muscles, excyclorotation of muscles, and instability of muscle pulleys. The authors then review options for surgical treatment of the often complex ocular misalignment in these disorders.

The genetics of strabismus.

Strabismus (misalignment of the eyes; also known as "squint") comprises a common heterogeneous group of disorders characterised by a constant or intermittent ocular deviation often associated with amblyopia (uniocular failure of normal visual development) and reduced or absent binocular vision. The associated poor cosmetic appearance may also interfere with social and psychological development. Extensive twin and family studies suggest a significant genetic component to the aetiology of strabismus. The complexity of the molecular basis of strabismus is now beginning to be elucidated with the i

The heritability of strabismus.

The etiology of strabismus has long been observed to have a genetic component. Recent advances in genetic methodology may provide insight into the genetic basis for several types of inherited strabismus, including those associated with genetic multisystem disorders such as Moebius syndrome, Prader-Willi syndrome, craniofacial dysostoses, and mitochondrial myopathies. Inheritance of primary forms of strabismus, such as congenital ocular fibrosis, Brown syndrome and Duane syndrome, has been reported, but less is known of the defective genetic sites. The genetic basis for isolated strabismus that

Strabismus in ptosis.

Two hundred twenty-six consecutive surgical ptosis cases were carefully evaluated to establish the incidence of strabismus in ptosis. The overall incidence of strabismus in surgical ptosis patients was found to be 36%. Most remarkable were the findings in 113 congenital ptosis patients; 32% of these patients were found to have strabismus. Only 44% of these patients had entities previously described associating strabismus with ptosis. It is postulated that in four of the congenital ptosis patients (3.5%), strabismus developed as a result of their ptosis. In one of these patients, strabismus and
