Sulfasalazine hepatotoxicity.

Hepatotoxicity has been recently described as a rare complication of sulfasalazine therapy. Two patients with hepatic damage after sulfasalazine treatment are reported. Liver biopsy performed in one patient revealed granulomatous hepatitis. On rechallenge with 5 aminosalicylic acid enema the second patient developed a hypersensitivity reaction without hepatic involvement.

Sulfasalazine pneumonitis.

A man with alleged Crohn's disease of the terminal ileum was started on sulfasalazine. Five weeks later he developed progressive cough, shortness of breath, and fever associated with peripheral eosinophilia and bilateral pulmonary infiltrates. After withdrawal of sulfasalazine all abnormalities returned to normal. This case supports the conviction that there is an entity of sulfasalazine-induced lung disease.

[Sulfasalazine in rheumatology].

Sulphasalazine (SLZ) has proved to be a drug effective in inducing clinical improvement or remission in chronic inflammatory rheumatic diseases (other than inflammatory bowel diseases) such as rheumatoid arthritis (RA) and some seronegative spondyloarthropathies, in a fashion similar to that of long-acting, second-line drugs in RA. Several clinical studies agree upon the efficacy of the compound employed at a dose of 2-3 g/day and upon the incidence of side effects, which appear to be equivalent to or lower than those related to traditional disease-modifying drugs employed in RA. The following

Sulfasalazine-induced agranulocytosis.

We report a patient with ulcerative colitis in whom agranulocytosis was diagnosed 5 wk after the initiation of sulfasalazine therapy. Concomitant features included fever and skin rash, and bone marrow examination revealed severe myeloid hypoplasia. Antineutrophil antibodies were absent from the serum. Recovery began during the 2nd wk and was characterized by a leukemoid reaction with thrombocytosis. Subsequently, the patient received 5-aminosalicylate enemas without adverse effects. Agranulocytosis, a rare effect of sulfasalazine, appears to occur almost invariably during the first 2 months of…

Sulfasalazine for ankylosing spondylitis.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease of unknown cause and affects mainly the spine, but can also affect other joints. Disease progression may result in loss of mobility and function. Sulfasalazine is a disease-modifying antirheumatic drug used in the treatment of AS. However, its efficacy remains unclear. This is an update of a Cochrane review first published in 2005. OBJECTIVES: To evaluate the benefits and harms of sulfasalazine for the treatment of ankylosing spondylitis (AS). SEARCH METHODS: We searched for relevant randomized and quasi-randomized tri…

Sulfasalazine and dermatitis herpetiformis.

Dermatitis herpetiformis that is unable to be controlled using dapsone and a gluten-free diet presents a therapeutic challenge. Three cases that responded well to sulfasalazine are presented. Two cases, who were unable to tolerate dapsone, had a rapid response to sulfasalazine, without apparent side-effects. The third case with dapsone-responsive blistering dermatoses, presumed to be dermatitis herpetiformis on the basis of serology, showed an excellent clinical response to sulfasalazine, but after 6 weeks of therapy had to cease it because of side-effects. Sulfasalazine is metabolized variabl…

Agranulocytosis associated with sulfasalazine.

Agranulocytosis is a rare but potentially lethal adverse effect of sulfasalazine. We report a case of sulfasalazine-associated agranulocytosis that occurred in a 79-year-old woman who had been taking the drug for approximately seven weeks. The patient had discontinued the drug on her own initiative nine days prior to admission. The patient was admitted with complaints of hoarseness, fever, odynophagia, and malaise. The total white blood cell count was 600/mm3 with a differential of 0% neutrophils, 8% bands, 67% lymphocytes, and 25% monocytes; a bone marrow aspirate and biopsy revealed maturati…

Sulfasalazine for ankylosing spondylitis.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease of unknown cause and belongs to a group of diseases known as spondyloarthropathies (SpA), which includes reactive arthritis, arthritis/spondylitis in inflammatory bowel disease, psoriatic arthritis/spondylitis and undifferentiated SpA. Non-steroidal anti-inflammatory drugs (NSAIDs) have been the main treatment for AS. For those refractory or intolerant to NSAIDs, the disease modifying antirheumatic drugs (DMARDs) have been used as a second line approach. Sulfasalazine (SSZ) is the best studied DMARD in AS, but its effica…

Sulfasalazine for alopecia areata.

Sulfasalazine is used as a therapy for various autoimmune conditions, including psoriasis; its effectiveness is presumed to be the result of its immunomodulatory effects. We have treated patients with severe alopecia areata with sulfasalazine as part of our dermatology practice and have noticed cosmetically acceptable regrowth in 23% of patients in whom a response could be determined. In view of its good safety profile, sulfasalazine may be considered for systemic treatment of severe alopecia areata.

[Sulfasalazine in rheumatoid arthritis].

Sulfasalazine (SLZ) is now a second-line treatment in rheumatoid arthritis (RA), as well as in psoriatic arthritis and ankylosing spondylitis. 54 patients with active RA were treated with enteric coated SLZ (2 g daily) in a 24-week open study. Clinical and laboratory parameters of disease activity were monitored initially and thereafter at 2-week intervals for the first 2 months, and then at 4-week intervals until the end of the 24-week study. There was significant clinical improvement at the end, as shown by scores for joint pain (Ritchie), grip strength and morning stiffness (p less than 0.0…

Sulfasalazine-induced aseptic meningitis.

Aseptic meningitis is a clinical syndrome of meningeal inflammation without clear infectious aetiology. Drug-induced aseptic meningitis is a well-documented but poorly understood side effect of a variety of medications, and has rarely been described with the immunosuppressive and anti-inflammatory drug sulfasalazine. This report describes the case of a man in his 60s presenting with aseptic meningitis after commencing sulfasalazine for seronegative arthritis 1 month prior. Analysis of cerebrospinal fluid showed a predominantly mononuclear leucocytosis, slightly elevated protein, and normal glu…

Sulfasalazine lung. Desensitization to sulfasalazine and treatment with acrylic coated 5-ASA and azodisalicylate.

A patient with ulcerative colitus developed fever, dyspnea, eosinophilia, and pulmonary infiltrates while taking sulfasalazine. The abnormalities resolved when the sulfasalazine was stopped but the colitus recurred. Desensitization to sulfasalazine was unsuccessful, but treatment with azodisalicylate (olsazine) controlled the colitus and did not cause pulmonary problems.

Case report: Sulfasalazine-induced hypersensitivity.

Drug-induced hypersensitivity syndrome (DiHS)/drug reaction with eosinophilia and systemic symptoms (DRESS) is a systemic inflammatory condition that is characterized by multisystemic involvement (liver, blood, and skin), heterogeneous manifestations (fever, rash, lymphadenopathy, and eosinophilia), and an unpredictable course; cases of DiHS/DRESS caused by sulfasalazine are rare in children compared to adults. We report a case of a 12-year-old girl with juvenile idiopathic arthritis (JIA) and sulfasalazine-related hypersensitivity who developed fever, rash, blood abnormalities, and hepatitis …

Sulfasalazine decreases mouse cortical hyperexcitability.

OBJECTIVE: Currently prescribed antiepileptic drugs (AEDs) are ineffective in treating approximately 30% of epilepsy patients. Sulfasalazine (SAS) is an US Food and Drug Administration (FDA)-approved drug for the treatment of Crohn disease that has been shown to inhibit the cystine/glutamate antiporter system xc- (SXC) and decrease tumor-associated seizures. This study evaluates the effect of SAS on distinct pharmacologically induced network excitability and determines whether it can further decrease hyperexcitability when administered with currently prescribed AEDs. METHODS: Using in vitro c…
