[Functions of the thalamus in psychiatry: the relationship between the thalamus and schizophrenia].

The thalamus involves two electrophysiological systems of the specific projection system and the non-specific projection system. The electrical stimulation of the former induces the augmenting response in the cerebral cortex and the same of the latter induces the recruiting response in the cortex. The non-specific projection system is also known as the diffuse thalamo- cortical projection system and it is concerned with the maintenance mechanism of consciousness such as sleep and wakefulness. The thalamus is concerned in the higher nervous functions such as language, cognition, memory and inte…

The Cerebellar Thalamus.

The thalamus is a neural processor and integrator for the activities of the forebrain. Surprisingly, little is known about the roles of the "cerebellar" thalamus despite the anatomical observation that all the cortico-cerebello-cortical loops make relay in the main subnuclei of the thalamus. The thalamus displays a broad range of electrophysiological responses, such as neuronal spiking, bursting, or oscillatory rhythms, which contribute to precisely shape and to synchronize activities of cortical areas. We emphasize that the cerebellar thalamus deserves a renewal of interest to better understa…

Coherence between cerebellar thalamus, cortex and muscle in man: cerebellar thalamus interactions.

Local field potentials (LFPs) were recorded in seven unanaesthetized patients between the four adjacent contacts of a macroelectrode stereotactically implanted for the treatment of tremor. The LFPs were presumed to arise predominantly from the nucleus ventralis intermedius (Vim) of the thalamus, the implantation target. They were recorded simultaneously with the ipsilateral EEG and contralateral EMG during an isometric contraction or at rest. The patients had a history of either isolated tremor (essential tremor, n = 2; benign tremulous Parkinson's disease, n = 1) or tremor with signs of a cer…

[Vascular syndromes of the thalamus].

The thalamus comprises numerous nuclei that can be grouped into five major functional domains: (1) the reticular and intralaminar nuclei influence arousal and nociception, (2) sensory nuclei handle afferent pathways, (3) the effector nuclei are involved in motor function and language, (4) associative nuclei participate in higher cognitive functions and (5) limbic nuclei influence mood and motivation. The thalamic nuclei are mainly supplied by the following four cerebral arteries: paramedian artery, anterior thalamoperforating artery, thalamogeniculate artery and posterior choroidal artery (lat…

Pain and the primate thalamus.

Noxious stimuli that are perceived as painful, are conveyed to the thalamus by the spinothalamic tract (STT) and the spinotrigeminothalamic tracts (vSTT), arising from the dorsal horn of the spinal cord and medulla, respectively. Most investigators have concluded that the thalamic terminus of these pathways include several nuclei of the somatosensory and intralaminar thalamus. Non-noxious stimuli are carried by the dorsal column/medial lemniscal or the trigeminothalamic pathways which terminate in much more restricted regions of the thalamus than do the STT and vSTT systems. Lesions of compone…

Lesioning the thalamus for dyskinesia.

Recent advances on understanding the pallidothalamic relation lead us to perform Vim-Vo thalamotomy (combined thalamic lesion in ventralis intermedius nucleus and ventralis oralis nucleus) for cases with dyskinesia. In our recent series of thalamotomies, there are 12 cases of dyskinesia caused by various etiologies. Therefore the clinical manifestation of the involuntary movement was different in each case, including, more or less, some elements of irregular involuntary hyperkinetic movement. Stereotactic operation was performed using Leksell's apparatus aided by Surgiplan and MRI. The Vim nuc

[A case of hemangioblastoma in the thalamus].

Supratentorial hemangioblastoma is encountered very rarely. About 80 cases in all have been reported. The authors present a case of supratentorial hemangioblastoma in the thalamus with gradually enlarging cysts. The patient was a 45-year-old man with complaints of left hemiparesis and headache. Computed tomographic scans of the brain showed a cystic mass with mural nodule in the right thalamus. Left vertebral angiography demonstrated a vascular tumor fed by a thalamogeniculate artery. A right temporo-parietal craniotomy was performed and the tumor was totally resected through the transcortical…

The role of the thalamus in schizophrenia.

BACKGROUND: Explaining the diversity of symptoms that occur in schizophrenia is a major conceptual challenge. Perhaps the most powerful strategy is to identify a fundamental cognitive process and/or a fundamental neural circuit. METHODS: Convergent data from our research group in Iowa and from investigators in other centres are summarized. RESULTS: The thalamus plays a key role in information processing. A defect in circuitry connecting the thalamus, frontal cortex, and cerebellum could explain a wide range of symptoms. Neuropathology and imaging studies suggest that patients with schizophre…

Infiltrative astrocytomas of the thalamus.

Clinical characteristics and outcome in 57 patients with infiltrative thalamic astrocytomas were analyzed retrospectively. The median patien … eatures; in multivariate analysis, only open biopsy was favorable. Infiltrative astrocytomas of the thalamus carry a dismal prognosis, regardless of the type of treatment. Hyperfractionated RT does not increase toxicity but its benefit over conventional RT remains unproven.

Reduced Thalamus Volume and Enhanced Thalamus and Fronto-Parietal Network Integration in the Chess Experts.

The ability of chess experts depends to a large extent on spatial visual processing, attention, and working memory, all of which are thought to be mediated by the thalamus. This study explored whether continued practice and rehearsal over a long period of time results in structural changes in the thalamic region. We found smaller gray matter volume regions in the thalami of expert Chinese chess players in comparison with novice players. We then used these regions as seeds for resting-state functional connectivity analysis and observed significantly strengthened integration between the thalamus…

The thalamus: Structure, function, and neurotherapeutics.

The complexity and expansive nature of thalamic research has led to numerous interventions for varied disease states. At the same time, this complexity along with siloed areas of study can hinder a comprehensive understanding. The goal of this paper is to give the reader a broader and more detailed perspective on the thalamus. In order to accomplish this goal, the paper begins with a summary of the function, electrophysiology, and anatomy of the normal thalamus. With this foundation, thalamic involvement in neurological diseases is discussed with a focus on epilepsy. Therapeutic interventions …

The Thalamus in Perioperative Neurocognitive Disorders.

Thalamus function and structure are known predictors of individual differences in the risk of age-related neurocognitive disorders (NCD), such as dementia. However, to date, little is known about their role in the perioperative setting. Here, we provide a narrative review of brain-imaging studies of preoperative and postoperative thalamus scanning parameters associated with risks of developing perioperative NCD, such as postoperative delirium (POD) and postoperative cognitive dysfunction (POCD) during the postoperative phase. These findings are discussed in light of the concept of reserve capa…

Primary intracranial cholesteatoma in the thalamus.

BACKGROUND: Primary intracranial cholesteatomas are rare, benign lesions of keratinizing squamous epithelium, often arising from middle ear extension. However, their occurrence in the thalamus is exceptionally rare, with only one prior case reported. CASE DESCRIPTION: A 48-year-old male presented with progressive headaches over 3 years and difficulty with upward gaze. Neurological examination revealed bilateral upward gaze palsy. Magnetic resonance imaging demonstrated an intra-axial cystic mass with a solid component in the right thalamus extending to the midbrain, causing obstructive hydroc…

The mediodorsal thalamus in executive control.

Executive control, the ability to organize thoughts and action plans in real time, is a defining feature of higher cognition. Classical theories have emphasized cortical contributions to this process, but recent studies have reinvigorated interest in the role of the thalamus. Although it is well established that local thalamic damage diminishes cognitive capacity, such observations have been difficult to inform functional models. Recent progress in experimental techniques is beginning to enrich our understanding of the anatomical, physiological, and computational substrates underlying thalamic…

Nuclei and tracts in the thalamus of crocodiles.

The thalamus is one of the most important divisions of the forebrain because it serves as the major hub for transmission of information between the brainstem and telencephalon. While many studies have investigated the thalamus in mammals, comparable analyses in reptiles are incomplete. To fill this gap in knowledge, the thalamus was investigated in crocodiles using a variety of morphological techniques. The thalamus consists of two parts: a dorsal and a ventral division. The dorsal thalamus was defined by its projections to the telencephalon, whereas the ventral thalamus lacked this circuit. T…

Orofacial proprioceptive thalamus of the rat.

The ascending pathway mediating proprioception from the orofacial region is still not fully known. The present study elucidated the relay of jaw-closing muscle spindle (JCMS) inputs from brainstem to thalamus in rats. We injected an anterograde tracer into the electrophysiologically identified supratrigeminal nucleus (Su5), known to receive JCMS input. Many thalamic axon terminals were labeled and were found mainly contralaterally in a small, unpredicted area of the caudo-ventromedial edge (VPMcvm) of ventral posteromedial thalamic nucleus (VPM). Electrical stimulation of the masseter nerve an…
