Thoracic injuries.

Establishment of adequate ventilation takes the absolute precedence over all other therapeutic measures. Chest trauma has, therefore, the highest priority in a patient with multiple injuries. Bleeding may also be a decisive component. Emergency treatment aims at reestablishing normal physiological functions. Concurrently, diagnostic measures are taken to assess the underlying pathoanatomical changes. Treatment of chest injuries is a team effort of surgical and anaesthetic staff.

[Thoracic injuries].

BACKGROUND: In this study, it was aimed to assess patients who applied to our clinics with the complaints of thorax trauma, consistent thoracal wall injury, complications due to these traumas, and the therapeutical methodology. METHODS: Records of 668 patients, hospitalized at Süreyyapaşa Training and Research Hospital and Yüzüncü Yil University Faculty of Medicine between January 1995 and December 2007, were reviewed retrospectively. Three hundred ninety-nine (59.7%) patients had blunt trauma whereas 269 (40.2%) patients had penetrating trauma. Thoracic wall injury was detected in a total of

Thoracic aortic and thoracic vascular injuries.

Patients with thoracic vascular injuries fall into two groups: those who are exsanguinating and require an empiric operation with a high mortality and those with contained injuries that permit preoperative evaluation. The unstable group requires judgment to determine the appropriate empiric position, exposure, and operation. Unlike abdominal trauma, which is addressed by way of a midline incision, there are multiple thoracic incisions that can be used to access thoracic vascular injuries. Thus, the stable group may benefit from preoperative imaging, which then can suggest a patient position, i

[Penetrating thoracic injuries].

In a period of peace in Germany and other westeuropean countries, penetrating trauma of the thorax (PTT) is much more rarer than blunt trauma. An analysis of 103 patients with PTT, treated in the Surgical Department of the University of Cologne from 1980 to 1990, showed, that these are usually isolated injuries in young patients. These occur usually following violence as stab wounds and less commonly gunshot wounds. Two thirds of the patients could be treated adequately with intensive care therapy and the use of thoracic drains. One third of the patients underwent thoracotomy, which was carrie

Nonpenetrating thoracic injuries.

In patients with nonpenetrating thoracic trauma, the rib fractures and other chest wall lesions may distract the physician from dangerous internal injuries in the chest or abdomen which may not be noted unless looked for very carefully. Early vigorous correction of any ventilatory problem is essential, particularly if there is any evidence of impaired tissue perfusion. Shock is frequently due to extrathoracic injuries, particularly intraabdominal bleeding. The flail associated with multiple rib fractures may seem mild initially, but severe underlying pulponary contusion and/or associated extra

Complex thoracic injuries.

Complex thoracic injuries are a leading cause of death in trauma patients. Four difficult problems of diagnosis and treatment are discussed, including (1) air leak not associated with pneumothorax, (2) management of major thoracic esophageal injuries, (3) penetrating trauma, and (4) retained hemothorax and empyema.

Thoracic Trauma: Aortic Injuries.

Thoracic aortic injuries caused by high impact trauma are life-threatening and require emergent diagnosis and management. With improvement in the acute care services, an increasing number of such injuries are being managed such that patients survive to undergo definitive therapies. A high index of clinical suspicion is required to order appropriate imaging. Computed tomography angiography is used to classify the injuries and guide treatment strategy. While low-grade injuries might be managed conservatively, high-grade injuries require urgent surgical or endovascular intervention. Over the past

Thoracic injuries in children.

The successful management of pediatric thoracic injuries includes immediate and careful evaluation of the injury type and severity. Early treatment can be conservative in nature or require surgical management. Prompt intervention can reduce morbidity and mortality rates, associated complications, and ensure optimal outcomes.

[Thoracic injuries in children].

Chest trauma in children is rare but shows that trauma is severe and the mortality rate is high (30%). Multidisciplinary management of children includes an initial evaluation of respiratory distress, freeing the airways, placing an intercostal tube, stabilizing the chest wall, and analgesia. When vital signs are stable, secondary evaluation includes an etiologic, radiologic and biologic check-up, ending with the therapeutic strategy. Thoracotomy is rarely required, and for most children, only monitoring will be necessary, though this is important because of the risk of secondary decompensation

Thoracic Trauma, Nonaortic Injuries.

Trauma is one of the leading causes of death worldwide. Approximately two-thirds of trauma patients have thoracic injuries. Nonvascular injury to the chest is most common; however, while vascular injuries to the chest make up a small minority of injuries in thoracic trauma, these injuries are most likely to require intervention by interventional radiology (IR). IR plays a vital role, with much to offer, in the evaluation and management of patients with both vascular and nonvascular thoracic trauma; in many cases, IR treatments obviate the need for these patients to go to the operating room. Penetrating thoracic vascular injuries.

1. Known injuries to the ascending aorta and arch are exposed by median sternotomy. Known injuries to the descending thoracic aorta can be exposed through a posterolateral thoracotomy. These injuries are more commonly diagnosed through emergent exploration by means of anterolateral thoracotomies. 2. Preoperative arteriography is extremely useful in managing penetrating injuries to the thoracic outlet, as it allows the choice of appropriate incisions for exposure and control. 3. Innominate artery, right common carotid or subclavian artery, as well as left intrathoracic common carotid artery inj

[Thoracic trauma with injuries of the thoracic spine].

From a major series of surgically treated patients with fractures and fracture dislocations of the thoracic spine 4 cases are presented exhibiting different trauma mechanisms and presenting with a variety of associated acute and chronic intrathoracic lesions. Surgical stabilisation and treatment of additional intrathoracic injury (vascular, lymphatic duct, empyema, posttraumatic fibrothorax) necessitated a transpleural approach, effective for the spine as well. Diagnostic radiology was restricted to minimal requirements due to urgency and/or difficulties of exposure.

Videothoracoscopic evaluation of thoracic injuries.

Residual thoracic collections after thoracic traumas occur in 5-30% of patients and are a major risk factor for development of empyema. Management with a thoracostomy tube is the traditional treatment but it requires a prolonged hospital stay and 20% of patients treated continue to have a residual clot. Forty-two haemodynamically stable patients with chest trauma (36 blunt and 6 penetrating) were examined thoracoscopically. Indications for videothoracoscopy included suspected diaphragmatic injury (8 patients), persistent pneumothorax (8), continued haemorrhage (6) and clotted haemothorax (20).

Thoracic and Lumbar Vertebral Injuries.

Injuries to the thoracic and lumbar vertebral column are not uncommon and are the result of increased loading on structures endowed with little motion or flexibility. Conventional radiographs, when carefully analyzed for the ÒfootprintsÓ and ÒfingerprintsÓ of the injury, will define its nature and, to a degree, its extent. A system of radiologic analysis is described and the parameters for diagnosis and differentiation are given. CT and MR imaging will provide further information regarding the full extent of injury and involvement of the spinal cord, if any. The importance of the radiographic

[Practical aspects of thoracic injuries].

While mortality is about one per hundred in patients with injuries to the chest wall it is one in five in patients with internal thoracic injuries. The mortality is dependent on the total severity of injuries, and upon age. Closed, blunt thoracic trauma may initially present few signs and symptoms. Internal injuries, such as pneumothorax and haemothorax may be present. Decompression by thoracic drainage improves oxygenation. This may be crucial for the prognosis in multitrauma, particularly in cases of concomitant head injury. Treatment and diagnosis of thoracic injuries have first priority in

Penetrating and perforating thoracic injuries.

Five hundred and eleven patients with penetrating or perforating chest injuries were admitted to the Department of Thoracic and Cardiovascular Surgery, University Central Hospital, Helsinki, during the 25-year-period 1952-77. There were 433 stab wounds, 59 gunshot wounds and 19 other penetrating injuries. The organs most often involved were lungs (major haemo- or pneumothorax in 385 patients), heart (63 patients) and liver (61 patients). About one third of the patients were in profound shock on admission. The treatment was immediate thoracotomy in 176 (35%) and laparotomy in 123 cases (24%). T

[Current problems in thoracic injuries].

The authors stress the increased frequency in the recent years of single thoracic trauma and of thoracic trauma in the frame of polytraumatic lesions. The fact is stressed that the severity of thoracic trauma is usually determined by pathophysiological syndromes generated sometimes by minor anatomical lesions. An original classification is presented, of thoracic trauma, the present etiopathogenetic concept is discussed, as well as pathophysiologic syndromes consecutive to thoracic trauma. The major types of lesions are illustrated by clinical cases. The treatment of thoracic trauma should resp
