This article discusses pathologies found in monozygotic twinning. Detailed information is provided regarding the development during monozygotic twin formation: embryo development, twin-to-twin transfusion syndrome, acardiac twinning, vanishing twins, conjoined twins, and Beckwith-Weidmann syndrome twins. An algorithm describing the approach for identifying pathology in a placenta with multiple pregnancies is presented.

OBJECTIVE: To report two cases of monozygotic twinning after IVF-blastocyst transfer.
DESIGN: Case report.
SETTING: Private practice in an assisted reproductive technology clinic.
PATIENT(S): Two women treated with IVF-ET at the blastocyst stage.
INTERVENTION(S): Pituitary down-regulation with luteal leuprolide acetate, ovulation induction with gonadotropins, IVF, sequential culture, blastocyst transfer, and P for luteal support.

An excess of structural defects occurs in monozygotic twins compared in dizygotic twins or singletons. The excess is composed of three categories of defects. Recent changes in the estimated incidence of monozygotic twinning in 15 European populations are described. Amyoplasia is a very specific, nongenetic clinically recognizable form of arthrogryposis, representing about one-third of individuals with arthrogryposis surviving the newborn period. We describe a case of concordant body stalk anomaly in a monozygotic twin. Autopsy of the fetus showed abnormalities compatible with the maldevelopment of embryonic folding. Abdominal viscera were in a sac covered by the amnion and were attached directly to the placenta. The anus was not visible and no discernible external genitalia were noted. A case is presented of a monozygotic twin pair, discordant for phenotypic sex, in which the female member showed gonadal dysgenesis and chromosomal mosaicism. Review of the pertinent literature reveals that in monozygotic twin pairs, phenotypic and karyotypic concordance is the usual occurrence for Down's and Klinefelter's syndromes, whereas discordance often accompanies gonadal dysgenesis. Mosaicism is a frequent concomitant of gonadal dysgenesis in monozygotic twins. Our case strengthens the probability of a real association between mosaicism and monozygotic twinning in gonadal dysgenesis.
