Arteriosclerosis, thrombosis, and vascular biology2026eng
Peripheral artery disease commonly refers to diffuse atherothrombotic disease of the arteries supplying the legs. Peripheral artery disease has been relatively understudied and has not been subject to the same intensive scrutiny and research that characterizes coronary artery disease. Moreover, the diagnosis of peripheral artery disease can be complicated by the presence of arterial calcification. Here, we provide a brief report on the current understanding of peripheral vascular calcification to include the following sections: basic mechanisms, anatomic distribution of arterial calcification,

In Japan, the number of diabetes mellitus and chronic kidney disease patients is increasing yearly. Vascular calcification is prevailing in patients suffering from diabetes mellitus and/or chronic kidney disease, and carries a poor prognosis. However, the effective treatment for suppressing the progression of vascular calcification has not been established. On the other hand, treatment of osteoporosis has advanced dramatically as the details of the mechanism become clearer. Bisphosphonates, which are inhibitors of bone resorption that are widely used to treat osteoporosis, also inhibit cholest

Vascular calcification is an independent risk factor for the development of cardiovascular disease and is classified into two types based on the site of calcification : intimal atherosclerotic calcification and Mönckeberg's medial calcification. Matrix vesicles released from macrophages and vascular smooth muscle cells (VSMC) during apoptosis play a pivotal role in formation of fine granular calcification, while osteogenic differentiation of VSMC contributes to progression of advanced calcification. Recent noninvasive imaging studies of atherosclerotic calcification provide robust evidence tha

In patients with chronic kidney disease (CKD), cardiovascular events (CVA) are the main cause of morbidity and mortality. Vascular calcification, linked to bone mineral metabolism disorders such as elevated serum phosphate, parathyroid hormone (PTH), and FGF23, well-known uremic toxins, aggravate this risk. Calcimimetics are allosteric activators of the calcium-sensing receptor (CaSR), a G protein-coupled receptor that regulates PTH secretion and synthesis in response to changes in extracellular calcium in the parathyroid glands. Through direct and indirect mechanisms, they have demonstrated t

Objective- Vascular calcification is a major risk factor for rupture of atherosclerotic plaques. High expression of BMP2 (bone morphogenetic protein 2) in lesions suggests its importance in vascular calcification during atherosclerosis. Teniposide is a Topo II (DNA topoisomerase II) inhibitor and is used for cancer treatment. Previously, we reported that teniposide activated macrophage ABCA1 (ATP-binding cassette transporter A1) expression and free cholesterol efflux indicating Topo II inhibitors may demonstrate antiatherogenic properties. Herein, we investigated the effects of teniposide on t

The link between chronic kidney disease(CKD)and cardiovascular diseases has recently been attracting attention as cardiorenal connection. Vascular calcification due to down-regulation of klotho is partly involved in the pathogenesis of cardiorenal connection. Klotho protein which is mainly expressed in the distal convoluted tubule, is directly correlated with renal function. Of note, its expression has already been decreased in patients with estimated glomerula filtration rate(eGFR)of 60~90 mL/min/1.73 m2 compared with those with eGFR of more than 90 mL/min/1.73 m2. In early CKD, compensatory

A variety of potential therapies for vascular calcification, based either on the underlying biology or physical chemistry or solely on empiric observations in patients, may be effective but lack rigorous testing. Pasch et al. provide convincing evidence that sodium thiosulfate prevents medial vascular calcification in uremic rats. Although this provides some scientific basis for the clinical use of thiosulfate, uncertainty about mechanism of action and safety still remains.

Vascular calcification is a major feature of cardiovascular disease, but mechanistic studies are limited by slow and variable animal models and by ex vivo systems that insufficiently reflect physiological flow. Here, we established an ex vivo vascular calcification perfusion system and compared perfusion with static culture using an in vivo rabbit abdominal aortic calcification model as a pathological reference. Low serum combined with interleukin-17A rapidly induced vascular calcification within 7 days. Under normal culture conditions, the perfusion system preserved endothelial and smooth mus

Cinacalcet acts on calcium receptors (CaR) expressed on chief cells of the parathyroid gland to inhibit the secretion of parathyroid hormone (PTH) . This drug inhibits PTH secretion without causing an elevation of serum calcium and phosphorus, unlike active vitamin D. Several experimental studies demonstrated an inhibitory effect of calcimimetics on the progression of vascular calcification in animals with chronic kidney disease (CKD), in keeping with the expression of the calcium-sensing receptor (CaSR) in vascular tissue. The EVOLVE, evaluated in patients with CKD 5D the effects of the cinac

BACKGROUND: Arteriovenous (AV) access dysfunction is a common complication in hemodialysis patients. Markers of vascular calcification are associated with cardiovascular outcomes and mortality in this population, but their association with vascular access outcomes is unknown. In this study, we aimed to evaluate the association between selected vascular calcification makers and vascular access complications in a cohort of hemodialysis patients.
METHOD: Fetuin-A, osteopontin (OPN), osteoprotegerin (OPG), and bone morphogenetic protein-7 (BMP-7) were measured in blood samples from 219 dialysis p

Numerous epidemiological studies link bone loss with vascular calcification and cardiovascular disease. Osteoporosis and atherosclerosis share risk factors and are likely to share pathogenic mechanisms as well. On the other hand, drugs used in the treatment of osteoporosis could influence cardiovascular risk, just as those used in cardiovascular disease could affect bone mineral density and the development of fractures.

Intimal and medial calcification are increased with type 1 and type 2 diabetes, metabolic syndrome, chronic kidney disease, and ageing. There are several biological mechanisms through which vascular calcification increases all-cause mortality and atherosclerotic plaque rupture. Arterial medial calcification increases arterial stiffness that causes systolic hypertension and diastolic dysfunction and heart failure. In contrast, arterial intimal calcification is strongly associated with atherosclerotic plaque burden, predicting adverse arterial events. In particular, micro-calcifications within t

Vascular calcification is associated with adverse cardiovascular events that increase the risk of cardiovascular death. Unfortunately, the pathogenesis of vascular calcification is complex and incompletely understood. As important intercellular signaling molecules, the role of extracellular vesicles (EVs) in vascular calcification has attracted wide attention in recent years. This review will briefly describe the role of EVs (mainly including exosomes and microvesicles) in the process of vascular wall calcification focusing on the specific mechanisms of smooth muscle cell (SMC) differentiation

Vascular calcification is highly prevalent in patients with coronary artery disease and, when present, is associated with major adverse cardiovascular events, including an increased risk of cardiovascular mortality. The pathogenesis of vascular calcification is complex and is now recognized to recapitulate skeletal bone formation. Vascular smooth muscle cells (SMC) play an integral role in this process by undergoing transdifferentiation to osteoblast-like cells, elaborating calcifying matrix vesicles and secreting factors that diminish the activity of osteoclast-like cells with mineral resorbi

Chronic kidney disease (CKD) is characterized by phosphate retention and reduced synthesis of 1.25(OH)2-vitamin D stimulating parathyroid hyperplasia. These changes cause a complex osteopathy, defined as renal osteodystrophy, and vascular calcification. Renal osteodystrophy increases the risk of fracture and causes deformities and disability. Vascular calcification occurs in a large proportion of hemodialysis patients and is a marker of arteriopathy. Calcifying arteriopathy induces arterial stiffness and contributes to the high cardiovascular mortality and morbidity among CKD patients. Vascula

The articles providing answers to the questions on vascular calcification of most interest from a clinical point of view were selected. 1. How is it measured?: Studies showing the clinical utility of different tools to quantify it were analyzed. 2. What does it measure?: Both in dialysis patients and the general population, vascular calcification and arterial stiffness are prognostic factors for morbidity and mortality. Other markers such as fetuin-A are associated with mortality in patients on hemodialysis but not in patients in early stages of chronic kidney disease. 3. What causes it?: In t

Vascular calcification is an active, cell-mediated process that results from an imbalance between the promoters and inhibitors of mineralization. The process of vascular calcification shares many similarities with that of skeletal mineralization. However, while skeletal mineralization is a regulated process induced by complex, well-timed developmental cues, vascular calcification is a pathological process, occurring in response to dysregulated/inappropriate environmental cues. Damage inducing agents present in the uremic milieu such as a mineral imbalance, induce vascular smooth muscle cell (V

Vascular calcification increasingly afflicts our aging and dysmetabolic population, predisposing patients to cardiovascular mortality and lower extremity amputation. Active osteogenic processes are evident in most histoanatomic variants, including elaboration of BMP2-Msx2 signals required for craniofacial bone formation. We developed an animal model of diet-induced diabetes, dyslipidemia, and vascular calcification. High-fat diets promote vascular calcification in male low-density lipoprotein receptor (LDLR)-deficient mice, with concomitant upregulation of aortic BMP2 and Msx2 gene expression.

PURPOSE OF REVIEW: Vascular calcification is a common and important cardiovascular risk factor in patients with chronic kidney disease (CKD). Recent advances in the understanding of the biology of vascular calcification implicate vitamin K-dependent proteins as important regulators in this process. This review highlights recent key advances in vascular biology, epidemiology, and clinical trials in this rapidly evolving field.
RECENT FINDINGS: Vitamin K deficiency is associated with increasing severity of vascular calcification among patients with CKD, but the relationship with cardiovascular

Renal osteodystrophy, vascular disease and mortality are believed to be linked in patients with chronic kidney disease (CKD), although to date most of the evidence is based only on statistical associations. The precise pathophysiology of vascular calcification in end stagerenal disease (ESRD) is unknown, but risk factors include age, hypertension, time on dialysis, and, most significantly, abnormalities in calcium and phosphate metabolism. Prospective studies are required before 'cause and effect' can be established with certainty, but it is an active metabolic process with inhibitors and prom
